Bradley 12/20 Alusaine Jalloh 12/26 Extended Care Chronicles DECEMBER · ISSUE 12 · VOLUME 4 Some of you may have heard this quote and the term “Person Centered Care.” This approach is moving front and center to the work we do in Long Term Care. Person-centered care means that nursing home residents are supported in achieving the level of physical, mental and psychosocial well-being that is individually practicable. This goal honors the importance of keeping the person at the center of the care planning and decision-making process. In the reality of our work day what does this mean? It means we have to look at our processes, our schedules, our planning and ask ourselves – Is what/when/how we are doing X what the resident wants or is it because it fits better with our workday? Everyday things we do are often based on our schedules not what the resident would like. For instance, a resident who wants to sleep later but “can’t” or doesn’t because if they sleep later they will miss breakfast. We have to re-tool our processes. This is not to say the kitchen becomes a restaurant but we can accommodate (person centered approach) that late riser by having breakfast substitutes, like yogurt, hard boiled eggs, cold cereal, toast and the like. Same thing with a resident who would prefer baths but we encourage a shower because it is easier on us. You will hear more and more of the term Person Centered Approach as time goes on as it is a definite direction of that the regulatory agencies are moving. I think we all can understand that when we put the resident in the center, the outcomes are better and people are generally happier. We have to make it work. It is the HOLIDAY SEASON. The Sebo’s Nursing & Rehabilitation Admissions and Marketing Team is led by Shameia Pope & LaShanda Berry. Shameia has done an outstanding job externally showcasing all the great things Sebo’s Nursing & Rehab has to offer, including their 4-Star Recipe for Success! Our 4-Star Recipe is a combination of 1 Part Quality Care, 1 Part Innovative Memory Support Care, 1 Part Compassion, and 1 Part Highly Trained and Dedicated Staff! She continues to lead our forces externally while our internal Admissions Director, LaShanda Berry owns our internal process! LaShanda is new to the Admissions position but she continues to SHINE! Shameia & Lashanda make an excellent team as they managed to admit 21 patients in the month of October, a Sebos ALL-TIME HIGH!! In the month of October, they converted 40% of their Medicare Referrals and closed on nearly 50% of their tours this month! We congratulate this team on their daily hustle and efforts and they aren’t stopping now, this team is on FIRE! Munster Med-Inn took the month of October and led the entire company with a grand total of 44 Admissions, the highest to date since 2013 for the Munster Team! This team had close to 100 referrals this month! The Admissions & Marketing team is led by Chauntisha Jakes, Yvonne Wiejak, and Michael Siegal, together their team is dynamite in their ability to convert 61% of their Medicare admissions and closing nearly 70% of their tours! This team has done great

things and the best is yet to come! This team comes with longevity and the drive and determination! Chauntisha is a strong force externally and with her team of Yvonne and Michael executing and owning it internally, creating a recipe for success! Wheaton Care Center mourns the loss of one of their beloved nurses.

Fides Ann Dela Cruz (July 26, 1980 -October 06, 2017)Fides passed away after a very brief, yet courageous fight with cancer.Her first degree was a Bachelors in Psychology and then went on to earn her Bachelor’s in Nursing.

She worked at Jecson Medical Center in the Philippines and then she worked two years in Japan prior to moving to the United States. She started at Wheaton Care Center in 2013, working in the kitchen as she studied for the NCLEX exam. When she completed the exam, she moved to Miami to work. Fides returned to Wheaton Care Center in 2015 working part-time on nights until a full time day position became available. She was a hard-working, down-to- earth, private person who will be remembered as someone who was always helping people.She is survived by her parents, Edna and June Dela Cruz, four sisters, Dabing, Paton, Cacay and ME, and her niece Carlee. She is also survived by her close friends, the Wheaton Care Center staff and residents who will never forget her smile and her caring touch.

In recognition of Breast Cancer Awareness Month, Estates held their own awareness walk, to honor the residents and employee breast cancer survivors. The initial inspiration for the walk came from Laquina Lane (known as Q), our Admissions Director who is a survivor herself. Laquina participates in the local breast cancer awareness walk every year and wanted to celebrate the residents, who may not be able to participate in a local walk.
