Single Use Endodontic Files By Dr Matthew Gentner - BDSc MDSc (QLD) MRACDS (ENDO)

For many years now, the endodontic group have enforced the policy of single use of endodontic files. There are protocols for cleaning and sterilising of endodontic instruments, however they are quite cumbersome and it is difficult to get consistency in results from these procedures. There is also the risk of sharps injuries to staff in performing the procedure.

Several government agencies around the world, for example in the United Kingdom recommend the single use endodontic files. This action has arisen out of concern for the difficulties encountered in their cleaning and sterilization after use and the possibility that they may act as vehicle for disease transmission when re-used. Of particular concern is the transmission of prion protein, a pathogenic isoform of a common host cell receptor, which causes acquired iatrogenic Creutzfeldt-Jakob Disease (iCJD), a type of the spongiform encephalopathy. No treatment or prophylaxis is available for this disease and its acquisition nearly always proves fatal. Disease transmission associated with the re-use of endodontic hand and rotary files and reamers has not yet proven itself to be a clinical problem in endodontic practice although there is still concern in the literature.

The other risk is instrument separation. This risk increases with more use of files. It can be very difficult to track the number of times instruments have been used (and do you count number of teeth or canals?). It is also difficult trying to take into account the type of canals they were used in – highly dilacerated or relatively straight? Studies also demonstrate the decline in efficiency of endodontic files when machining dentine. This tends to lead to pushing harder, exacerbating the risk of instrument separation.

When initialling making this decision we asked ourselves “What would we like used on ourselves.” Once you answer this question, the decision seems quite simple. There are cost implications for patients; however most are quite happy to pay more for the use of new instruments. This ensures the highest possible care available.

Fiji trip to the Suva Dental SchoolDrs Ross Applegarth and Bayardo Martell visited Fiji in September to present a series of lectures at the Suva Dental School and the Nandi University campus. They enjoyed the

warmth and hospitality of the Fijian dental community, and took the opportunity to dive in the crystal clear waters of the Mamanuca Island. As you would expect, the equipment at the Fijian Dental School is very basic. The endodontic group have made a donation to the Suva Dental School that includes equipment, instruments and textbooks in an effort to improve the quality of endodontic training.
