inquantic 1es of special form radioactive materials and are packed for shipmentSpecification 7A Type A (general prekaging) containers (US Title 49 CFR 178.350).

These containers meet the requirements of CFR 173.24 and are designed andconstructed to meet the standards as specified in CFR 173.389(j) and 173.398(b).

They are marked USA DOT 7A TYPE A, RADI0 ACTIVE MATERIALS in conformance with.

CFR 173.24.

Thess regulations are in conformance with the requirements (Marginal C-6) of'

the International Atomic Energy Agency's " Regulations for the Safe Transport ofRadioactive Materials", Safety Series No. 6, 1973, revision edition. Theseregulations do not require competent authority for international shipment as '

icovered in Section VIII, paragraph 801.

For re-shipment, the shipper must maintain a copy of this Certification on filefor a period of one year in accordance with CFR 173.394 (a) (1). Note that;

DUTIES & RESPONSIBILITIES

General Responsibilities.

A. Coordinate the safe use of the surface moisture - density gauges.

B. Ensure compliance with the requirements of Title 10, Code of FederalRegulations, Parts 19, 20, 30, 71 and applicable 0.0.T. regulations.

Typical Duties

A. To assure that byproduct material possessed under the license conformto the materials listed on the license.

B. To assure that use of the device, particularly in the field, is onlyby authorized individuals.

C. To assure that all users wear personnel monitoring equipment, such as

|[ film badges or thermoluminescence dosimeters (TLD), when required.

D. To assure that gauges are properly secured against unauthorized removalat all times when they are not in use.

E. To serve as a point of contact and give assistance in case of energency(gauge damage in the field, fire, theft, etc.) to assure that properauthorities; for example NRC, local police, and state personnel, arenotified promptly in case of accident or damage to gauges.

F. To assure that the terms and conditions of the license, such as periodicleak test, are met and that the required records, such as personnelexposure records, leak test records, etc. are periodically reviewed forcompliance with Nuclear Regulatory Commission regulations, requirements,and license conditions.

G. To provide individual users reports on their exposure levels.

CONSOL RADIATION SAFETY~

& EMERGENCY PROCEDURES.

1. Transportation,

A. The device shall be fully secured within the transportationvehicle and placed as far away from passengers as possible.

B. The source rod lock -should be in place and the containercarried in a vehicle which can be locked.

2. Storage

A. When not in transit, the instrument should be stored in asecured place.

B. The instrument should not be stored for more than 8 hours atless than 1 meter from undeveloped film.

C. During construction inspection jobs the gauge shall be storedin a locked, secured place at the site.

- 3. Incidents

A. The user shall imediately report any theft or loss of thelicensed gauge to the Radiation Protection Officer for sub-sequent reporting to the regulatory authority.

5. Maintenance

A. Any maintenance on the gauge involving dismantling or removalof source holders must not be perfomed by the user and mustonly be perfomed by the manufacturer of the gauge.

B. To insure that the source is not leaking radioactive materiala leak test will be taken every 6 months with Troxler's 3880 '
