� Oral exams (including evaluations by a specialist) are payable twice per calendar year. Limited oral evaluationsfor a specific problem or complaint are also payable twice in the same calendar year.

� Prophylaxes (cleanings) are payable twice per calendar year. Two additional periodontal maintenanceprocedures are payable per calendar year for individuals with a documented history of periodontal disease. Fullmouth debridement is payable once in a lifetime.

� Fluoride treatments are payable once per calendar year for people up to age 19.� Bitewing X-rays are payable once per calendar year and full mouth X-rays (which include bitewing X-rays) are

payable once in any five-year period.� Sealants are payable once per tooth per two-year period for the occlusal surface of first and second permanent

molars up to age 16. The surface must be free from decay and restorations.� Composite resin (white) restorations are Covered Services on posterior teeth.� Porcelain and resin facings on bridges are Covered Services on posterior teeth.� Implants and implant related services are payable once per tooth in any five-year period.

No Deductible.

Maximum Payment – $1,000 per person total per Benefit Year on all services, except cephalometric films, photos, diagnostic casts and orthodontic services (including surgical repositioning of teeth). $1,500 per person total per lifetime on cephalometric films, photos, diagnostic casts and orthodontic services (including surgical repositioning of teeth).

Dependent Age Limit – Dependents are covered up to age 26.

Eligible People – The subscriber (you) is eligible for dental benefits when your employer or organization notifies Delta Dental.

Also eligible at your option are your legal spouse and your children who meet the age requirements noted above. You and your eligible dependents must enroll for a minimum of 12 months. If coverage is terminated after 12 months, you may not re-enroll prior to the open enrollment that occurs at least 12 months from the date of termination. Your dependents may only enroll if you are enrolled (except under COBRA) and must be enrolled in the same plan as you. Plan changes are only allowed during open enrollment periods, except that an election may be revoked or changed at any time if the change is the result of a qualifying event as defined under Internal Revenue Code Section 125.

If you and your spouse are both eligible under this Contract, you may be enrolled as both a Subscriber on your own application and as a dependent on your spouse's application. Your dependent children may be enrolled on both applications as well. Delta Dental will coordinate benefits.

Benefits will cease on the last day of the month in which the employee is terminated.

Rates (per pay period):

This Summary of Dental Plan Benefits should be read along with your Certificate. Your Certificate provides

additional information about your Delta Dental plan, including information about plan exclusions and limitations.

If a statement in this Summary conflict with a statement in the Certificate, the statement in this Summary applies

to you and you should ignore the conflicting statement in the Certificate. The percentages above are applied to

Delta Dental's allowance for each service and it may vary due to the dentist's network participation.*

Health Screening Benefit $50 per covered person per calendar year

Provides a benefit if the covered person has one of the health screening tests performed. This benefit is payable once per calendar year per person and is subject to a 30-day waiting period.

EXCLUSIONS We will not pay benefits for losses which are caused by: intoxicants, narcotics and hallucinogenics, dental procedures, elective procedures, cosmetic surgery, felonies or illegal occupations, pregnancy of a dependent child, psychiatric or psychological conditions, suicide, intentional injuries, war, armed forces service or giving birth within the first nine months after the certificate effective date. We will not pay benefits for hospital confinement of a newborn who is neither injured nor sick. We will not pay benefits for loss during the first 12 months after the effective date due to a pre-existing condition, which means a sickness or physical condition for which a covered person was treated, had medical testing, received medical advice or had taken medication within the 12 months before the certificate effective date.

WAITING PERIODThe policy and its riders may have a waiting period. Waiting period means the first 30 days following the policy’s coverage effective date during which no benefits are payable. If your cancer has a date of diagnosis before the end of the waiting period, coverage for that cancer will apply only to losses commencing after the policy has been in force for two years, unless it is excluded by name or specific description in the policy.

EXCLUSIONSWe will not pay benefits for cancer or skin cancer:

■ If the diagnosis or treatment of cancer is received outside of the territorial limits of the United States and its possessions; or

■ For other conditions or diseases, except losses due directly from cancer.

■ Initial diagnosis of cancer progressive payment rider — Provides a lump-sum payment of $50 for each month the rider has been in forceafter the waiting period and before cancer is first diagnosed.

■ Specified disease hospital confinement rider — Pays $300 per day if you or a covered family member is confined to a hospital for treatment for one of the 34 specified diseases covered under the rider.

EXCLUSIONS AND LIMITATIONS

If the insured commits suicide within two years (one year in ND) from the coverage e$ective date or the date of reinstatement (not applicable in AR), whether he is sane or insane (not applicable in AZ), we will not pay the death benefit. We will terminate this policy and return the premiums paid, minus any loans and loan interest to you. Product may vary by state. Additional features

Paid-up at age 65

The premium payments for the juvenile policy stop at age 65 even though coverage continues

on the insured child up to age 100.

$3,000 immediate claim payment

This payment can help meet immediate needs, such as funeral costs, by providing an initial

death benefit payment of $3,000 to the designated beneficiary.

Accelerated death benefit

If the insured child is diagnosed with a terminal illness, you can o*en request up to 75 percent

of the policy’s death benefit, up to $150,000.

Optional rider

Accidental death benefit rider

When you purchase this rider, it pays an additional benefit if the insured dies as a result of

an accident before age 70. The benefit doubles if the accident occurs while the insured is a

fare-paying passenger. An additional 25% of the accidental death benefit will be paid if the

insured dies due to an injury sustained while driving or riding in a private vehicle and wearing

a seat belt.

Guaranteed purchase option

When you purchase the policy, your child has the option to purchase additional whole life

coverage – without having to answer health questions – at age 18, 21 and 24. They may

purchase up to the initial face amount, not to exceed a total combined maximum of $100,000

for all options.

Spouse term life rider

Your child can purchase term life coverage for a spouse, with a maximum death benefit of up to

$50,000. 10-year and 20-year coverage periods are available. They can choose to convert this

coverage to a cash value policy within certain time periods later on – without having to answer

health questions.

Children’s term life rider

Your child may purchase up to $10,000 in term life coverage for all of their eligible dependent

children and pay one premium. They can later convert this coverage to a cash value life

insurance policy – without having to answer health questions – upon their 70th birthday or the

child’s 25th birthday, whichever comes first. They can add this additional coverage to either

the primary or the spouse policy, but not both.To learn more,
