(Tong MJ, Co RL, Bellak C: Hepatitis A vaccination. West J Med 1993; 158:602-605)The safety and immunogenicity of an inactivated hepatitis A virus vaccine were assessed in 101 healthyadults. Seronegative persons with normal serum aminotransferase levels were grouped according to age:Group 1 (n = 24) and group 3 (n = 22) were between 18 and 40 years of age, and group 2 (n = 25) andgroup 4 (n = 30) were older than 40 years. Groups 1 and 2 received vaccine on a 0-, 1-, and 2-monthschedule (schedule A), and groups 3 and 4 received the vaccine on a 0-, 1-, and 12-month schedule(schedule B). Of the 101 vaccinated subjects, 98 (97%) seroconverted with antibody titers to hepatitis Avirus of .20 IU per liter after the first dose, and all subjects seroconverted after the second dose. The geo-metric mean titers a month after the third vaccine dose were significantly greater (P< .03) on bothschedules for younger subjects (schedule A, 1,743 IU per liter, and schedule B, 7,882 IU per liter) than forolder subjects (schedule A, 826 IU per liter, and schedule B, 4,279 IU per liter). Also, the differences ingeometric mean titers a month after the third dose were significantly greater (P< .001) for subjects inboth age groups on schedule B (group 3, 7,882 IU per liter, and group 4, 4,279 IU per liter) than for thosein schedule A (group 1, 1,743 IU per liter, and group 2, 826 IU per liter). The hepatitis A virus vaccine waswell tolerated, with mild discomfort at the injection site being the main side effect. This vaccine is bothsafe and highly immunogenic.

Manuscript InformationManuscripts should be sent to the Editor, Linda Hawes Clever, MD,The Western Journal ofMedicine, 221 Main Street, San Francisco,Califomia 94105. Manuscripts must be original, not previouslypublished, and not under consideration by another publication. Ifpreliminary data were included in another presentation or publica-tion, this should be noted in the cover letter and a copy of thatpublication should be included.

All manuscripts will be subject to peer review.

Letter of TransmittalAll authors must sign the letter of transmittal, with one au-

thor designated as correspondent and his or her name, address, andtelephone number included.

The order of authorship is determined by the authors. All au-thors should meet the basic criteria for authorship (as stated below).Because order of authorship is assigned in different ways, its mean-ing cannot be inferred accurately unless it is stated by the authors.Authors may wish to add an explanation ofthe order ofauthorship ina footnote. In deciding on order, authors should be aware that manyjournals limit the number of authors listed in the table of contentsand that the National Library of Medicine lists in MEDLINE onlythe first 10 authors.

A letter of transmittal, except for one accompanying a manu-script from US Government employees whose work was done aspart of their official duties, must include a paragraph that transferscopyright: "In consideration for reviewing and editing my submis-sion, the author(s) undersigned hereby transfers, assigns, or other-wise conveys all copyright ownership to 77Te Western Journal ofMedicine, in the event that such work is published by the journal."

The letter of transmittal must include a statement of authorresponsibility: "I have participated in the conception and design ofthis work and in the writing of the manuscript and take publicresponsibility for it. I have reviewed the final version of the manu-script and approve it for publication. I attest to the validity andlegitimacy of data in the manuscript and agree to be named as thelead author or coauthor of the manuscript."

The letter of transmittal must include a statement of financialdisclosure: "I warrant that I have no financial interest in the drugs,devices, or procedures described in the enclosed manuscript (exceptas disclosed herewith)."

Research or project support should be included as a footnote tothe manuscript or in the accompanying cover letter.

Manuscripts concerned with experimental investigation on hu-man subjects must include in the text a statement that all subjectsgave their informed consent and that approval of the appropriateethics committee was obtained.

Acknowledgments will be printed on a select basis-to acknowl-edge those who contributed to but were not involved in the writingor review ofthe paper. They should not include persons whose workon the manuscript was a part of their regular duties.

Preparation of ManuscriptsEverything, including references, tables, figure legends, and

abstracts, must be double-spaced.The original manuscript and two copies should be submitted on

standard-sized heavy bond paper. Allow a margin of 2.5 to 4 cm (1to 1.5 in) on both sides. Manuscripts transmitted by facsimile ma-chine are not acceptable unless requested by the Editors.

All values should be in Systeme International (SI) units, withmetric equivalents following in parentheses. Temperature readingsshould be given in Celsius. Use the generic name of drugs, with thesalt or ester given when first used.

Figures (illustrations) should be submitted in triplicate in blackand white, unmounted 12.7 x 17.8 cm (5 x 7 in) glossy prints, withthe figure number, author's name, and top of the photo indicatedwith a label on the back. Patients (and relatives) have a right toanonymity in published clinical documentation. Details that mightidentify patients should be avoided unless essential for scientificpurposes. Masking ofthe eye region in photographs of patients maybe inadequate protection ofanonymity. If identification ofpatients isunavoidable, informed consent should be obtained, and this shouldbe clearly stated in the article. Changing data on patients should notbe used as a way of securing anonymity. Figure legends, to be typedin sequence on the same page, should be in complete sentences. Anyfigures or tables previously published must be accompanied by aletter of permission to borrow from the copyright holder (the pre-vious publisher). The permission of the authors should also beobtained. The Editors discourage the use of borrowed figures ortables.

ReprintsA form to order reprints will be sent to the author when the

article is published.

ReferencesAll references should be cited in the text using superscript num-

bers and listed in their order of citation. They should be pertinent tothe text and not simply a listing of the results of a computerizedsearch. They should be in WJM style (see previous copy ofjournal).

All references should be medical, scientific, or scholarly publi-cations. Others-newspapers, magazines, etc-should be kept to aminimum and included in the text.
