FMP RISK ASSESSMENT

This document is intended as a basic template to assist in the completion of a risk assessment for film production activities.Detailed information is available from a variety of risk management and risk assessment specialists. Useful information can also be obtained from the Health & Safety Executive www.hse.gov.uk

When conducting a risk assessment, consider all the likely hazards; who may be harmed, property which may be damaged, and the controls already in place. Then, for each likely hazard, bearing in mind the controls already in place, record the ‘risk assessment’ level. The further action(s) to be considered will be determined by the level of risk assessment.

HAZARD IN WHICH SCENE IS THIS HAZARD PRESENT?

FURTHER ACTION TO BE TAKEN

Working Alone Ending up without a

direction to work or a necessary helping hand

Patryk or Taylor None Being able to contact with the

work partner through many long distance means as

well as being able to relay work to each

other through cloud sharing services

Moderate/Moderate Develop a plan for a situation when one of us is not present

so that we are always on track

Equipment Lost/Malfunction

Progress being slowed down/lost due to equipment

being lost or malfunctions

Patryk and Taylor Computers, Graphical Tablets,

Computer Accessories, General

Electronics

Replacement equipment and the ability to repair the broken equipment if

it’s all possible

Low/High Regularly check equipment for any wear as to prevent any malfunctions

Data Loss Ending up with lost files that then need to be

Patryk and Taylor Hard Drive and/or Computers

Backups made on several different

hard drives

Moderate/Extreme Use other services to store data such as cloud storage

Page 2: Risk Assessment (Project: Cosmic Constructor)

recreatedStress Overworked, may

cause us to be less efficient as well as create additional problems (such as creating more game bugs due to less concentration)

Patryk and/or Taylor

None Being able to take breaks when

necessary

Moderate/Low Have a plan of how many hours to work

for and when to take breaks

Problem with working

environment (Fire/Building

Structural Failure)

Renders the workspace unusable as well as creates a lot of other problems (serious health hazard, equipment and/or data loss)

Everyone in and around the building

Anything within the building, including

workspaces, electronics, as well

as any other equipment required

to work (pencils, papers, planners

etc.)

Evacuation, Equipment designed to fight the hazard (fire extinguishers

etc.) and being able to call emergency

services in an instant

Low/Extreme Make sure the building and

workspaces are being well

maintained as well as identify and

destroy any hazard that may cause things like fire

Unexpected Game Problems

Progress being slowed down

Patryk and Taylor None Good planning, several backups and version of the game

to go back to

Moderate/High Plan ahead and have several version of the game ready

to back up into

Risk Assessment Chart

Hazard likelihood / Consequences RISK ASSESSMENTVERY LIKELY TO HAPPEN or SEVERE CONSEQUENCES (ie. Someone could get seriously hurt)

EXTREME

Could happen or mild consequences (i.e. someone could suffer a minor injury)

HIGH

Unlikely to happen or minor consequences (i.e. production could be slightly delayed)

MODERATE

Rarely happens and very insignificant consequences LOW
