A Vision for Primary Care
and Prevention
The Access to Care project will position Seven Oaks as a centre for research and innovation in primary care and prevention with strong partnerships and opportunities for effective collaboration between private practice doctors, and both community and hospital services.

doctors, including current residents in the Family Medicine program, into the new clinic. The clinic will be a state of the art practice with an emphasis on illness prevention, utilizing Electronic Medical Records and “advanced access” – a scheduling system that allows patients to be seen within 48 hours of making an appointment. Advanced access has been successfully implemented at Kildonan Medical Centre (KMC), a teaching clinic which is also integrated within Seven Oaks. KMC will continue to operate as a teaching clinic, but hasn’t been able to take on new patients for quite some time, and is fully supportive of the new clinic.
Seven Oaks hopes to negotiate arrangements with the new physicians to see their own patients in hospital. In some ways Seven Oaks wants to reintroduce a model of care that was typical 30 years ago when family doctors routinely treated their own patients in hospitals. Family doctors gradually separated into either hospital or office practices, partly because of the demanding schedule from maintaining both. With better attention to work life balance and with enough doctors
The hospital is planning to spend up to $7 million to create a new family medicine clinic and other hospital services as part of a bright, attractive expansion and renovation of its front entrance and main level.
According to Chief Medical Officer Dr. Ricardo Lobato de Faria, the north end of Winnipeg is under served with family doctors because several practices have closed with no new clinics to replace them.
“We have lost our access to primary care in the community. Patients shouldn’t have to travel to St. Vital for regular doctor visits, and patients shouldn’t have to rely on walk-in clinics and Emergencies for their primary care. We just don’t have enough doctors in northwest Winnipeg,” he said.
Seven Oaks would also like to increase the number of doctors in the area available to treat patients in hospital, or treat their own patients in hospital to improve continuity of care and follow-up treatment in the community after discharge.
Seven Oaks is working with an existing private medical practice in south Winnipeg to recruit up to 10
participating, the time might be right for Seven Oaks to return to what was good about the original model.
“We need family doctors to return to hospitals because the care is often better when provided by the physician who knows the patient as a well person. There is much less need for expensive diagnostic testing and the delay in treatment from waiting for results. Patients can also return home sooner
and more safely with their own physician overseeing care in hospital and in the community,” Dr. Lobato de Faria said.
A major goal of the development is to improve and enhance the main level and entrance of the hospital to make a bright, open and comfortable space for patients, their visitors and staff. The clinic and other amenities will be developed in or adjacent to a two-storey atrium extending from the current entrance out to the sidewalk in front of the hospital.

The atrium, along with renovations to the current interior courtyard, will bring natural light into much of the main level.
“We’re an icon in the community, but right now we’re not very welcoming,” said Seven Oaks Chief Ancillary Business Development Officer Ron Van Denakker, who is overseeing the planning for the project.
“Access to Care will improve healthcare services but it will also create a much more pleasant environment and a better experience for patients and families and staff,” he said.
LM Architects, the same architectural group that designed Seven Oaks’ acclaimed Emergency Department redevelopment has been retained by the hospital and is proceeding to detailed design over the summer. Seven Oaks is going ahead with a new staff parking lot this summer, so that the existing parking lot in front of the hospital can be reserved for visitors and eventually the patients attending the clinic.

Seven Oaks is developing several accompanying services along with the family medicine clinic including a new, expanded gift shop; a small retail pharmacy so that patients of the clinic can fill their prescriptions without leaving the hospital; and new food services in a kind of food court development on the main level.
A Request for Proposals is being distributed to prospective food services providers over the summer with a goal of balancing profitability and healthy food choices in keeping with Seven Oaks’ longstanding reputation as a health promoting hospital.
Van Denakker says that the new retail developments, including the clinic, are part of a strategy to expand ongoing
sources of revenue to fund future hospital improvements.
Funding for the Access to Care project will come from three sources:
• $2 million is being provided by the Winnipeg Regional Health Authority in recognition of space Seven Oaks provided for the creation of a desperately needed second dialysis unit
• $4 million from a combination of current reserves and future revenues
• $1 million from community benefactors
“We think there are progressive philanthropists who will want to invest in innovation, who will recognize that success at Seven Oaks will be a success
for the whole community now and in the future,” Van Denakker said.
Seven Oaks hopes to be able to tender the project this fall so that construction can begin early in the New Year for completion by the end of 2011.

ACCESS TO CARE
You might think medical training is something that only happens in
large “teaching hospitals” but a family medicine teaching unit has been in place at Seven Oaks General Hospital since it opened in 1981.
Integrating Kildonan Medical Centre with a community hospital was fairly cutting edge in those days, the forefront of a trend according to the director in charge of post-graduate training for University of Manitoba’s Department of Family Medicine.
“Distributed education is happening more and more. There’s a real drive to do more teaching in context where (the doctors) are going to be practicing,” says Dr. Tamara Buchel.
She splits her time between her Faculty of Medicine offices at Health Sciences Centre and Kildonan Medical Clinic (KMC) which along with Family Medicine Clinic at St. Boniface Hospital oversees half the residents in the University of Manitoba’s “urban stream” of 18-20 doctors accepted each year for the two-year program.
For every patient that is severe enough to require care in a tertiary care hospital, there are thousands more that have minor surgery or just need a check-up or have an ailment that can be treated by a doctor in his or her office. That’s why residents at KMC
learn to provide care to in-patients of Seven Oaks Hospital as well as
out-patients who just come to the clinic for a visit.
Residency based on attending very sick patients in a trauma hospital would mean “missing most of what medicine is,” according to Buchel.

That’s why U of M is distributing education even further with a new Northern Connection Medical Centre, a rural stream and a bilingual stream.
There is also a trend towards getting medical students out into practice situations and teaching units to expose them to actual medicine sooner.
KMC residents spend six months consecutively at Seven Oaks in the first year, three months consecutively in the second year, and then half days weekly for the latter half of their second year of residency. That continuity means the residents tend to
“take intangible ownership and grow into the skin of family doctor when they are here,” Buchel says.
Residents and faculty at KMC also participate in Seven Oaks’ unassigned patient model, and cover for each other to ensure continuity of care.
A native of Pinawa, Dr. Buchel is a University of Manitoba graduate who went on to a distinguished career in the United States at Beth Israel Hospital in Manhattan, and then as faculty for Family Medicine residents at the Mayo Clinic in both Rochester, Minnesota and Scottsdale, Arizona.
Buchel returned to her roots in 2004, and says it felt like coming home in more ways than one. She still sees patients that seem familiar and then realizes they were patients she treated as a resident in the 1990’s.
“I did my own training here (at Seven Oaks) and when I came back for my job interview here, even walking into Seven Oaks Hospital and particularly KMC, it was a homecoming.”

What is Wellness Coaching

Wellness Coaching is a service focused on you, your lifestyle and making changes for your well-being. Working with a Wellness Coach does not mean having someone tell you how to live better, rather your coach helps build confidence and develop strategies to change the things you are ready and willing to change.
Unlike life coaching, wellness coaching includes lifestyle factors to help you address roadblocks that have kept you from anything from losing weight to working towards a more energetic, balanced life. The goals and actions are personalized.
A Wellness Coach can also lead supportive group sessions as a way to share ideas with others as you work through your personal obstacles to a shared goal.
The Wellness Coach certification used by the Wellness Institute combines evidence-based behavioural psychology and proven coaching skills from the International Coach Federation™ with existing training in a health or wellness profession.
According to Dawn Ziemanski, a certified Wellness Institute Wellness Coach with a degree in Kinesiology, clients benefit from this broad expertise because they are often dealing with changes that affect multiple aspects of their personal wellness.

The 12th Annual CP Bike Race on March 13 raised funds for Cerebral Palsy with over 1000 volunteers plus riders.

A Seven Oaks project to pilot ways of retaining retirement-eligible
healthcare professionals for longer has identified several phenomena that apply to other workplaces as well.
The Retention of Older Worker project led by Wellness Institute tested several measures designed to support older healthcare workers to continue working beyond their date of eligibility including 12 or 24 week refresher leaves offered in several Winnipeg healthcare facilities. The Older Worker leaves offered 55% of a worker’s salary while on leave, similar to what is provided for maternity leave by Employment Insurance. The two year project was supported with a Workplaces Skills Initiative grant from Human Resources and Social Development Canada.
According to Project Manager, Rose Dziadekwich, prior to taking the leaves 10 of the 15 participants were planning to retire imminently, but now more than half (eight) plan to stay longer and two plan to retire and return to work part-time. All of the participants agreed to a return of service, and so even the employees who have decided to retire will have extended their time in the workplace.
Healthcare can be a physically demanding and emotionally
workforces that have more and more of what is sometimes called the squeeze generation – adults who are caring for youths still in school at the same time they are also caring for elderly parents. “Elder care is as important to older workers as childcare is to younger employees.”
Other initiatives implemented during the project at Seven Oaks Hospital included:
• Retirement planning education
• Compressed work week experiments
• 2333 Mentorship initiative
The retirement planning education sessions have helped employees get the facts on retirement so they can make an informed decision, and in so doing have proven to be a valuable way to connect with retiring employees who might consider returning to work. Retired nurses make up about 30% of Seven Oaks current pool of casual nurses.
While the refresher leaves are something that can’t be maintained without a source of funding such as the Employment Insurance system, the other measures will continue as part of Seven Oaks ongoing efforts to attract and retain top performing healthcare professionals.
challenging occupation, but like other workplaces, the average age of healthcare workers is still climbing. Taking a few months out of the workplace allowed the participants “the ability to get a break, to rest and rejuvenate,” Dziadekwich said.
“Many said that this was the first time in their careers that they had a chance to reflect on their career and think about what it would be like to retire, a chance to evaluate retirement.” Even participants who came back to work determined to retire, came back “with a different mindset and even if they were only going to be back with us for a year, it will be a very productive year,” she said.
One of most surprising findings of the project was that a majority of participants used their leave to care for ill, elderly family members. Dziadekwich says that healthcare employees tend to be the ones in their families who take on the role of caring for others, and can be difficult to convince they should look after themselves for a change.
She notes, however, that without the leave, most would have been in the position of caring for their family members while still coming to work, and that has huge implications for

or butter crunch lettuce) 1/3 cup slivered almonds
2 cups summer fruit (sliced strawberries, peaches, or blueberries
1 tbsp sesame seeds
¼ cup vinegar
3 tbsp canola or olive oil
3 tbsp water
1 tbsp poppy seeds1/8 tsp Tabasco sauce
¼ tsp paprika
¼ tsp Worcestershire sauce
1 green onion, minced (or 3 tbsp chopped summer chives)
Directions – makes 10 servings
1. Heat oven to 350 F. Toast almonds on baking sheet until golden brown (about 5 min). Transfer toasted almonds to a small plate.
2. Wash greens. Tear into small pieces. Place in salad serving bowl.
3. Stir sesame seeds over medium heat in an ungreased skillet a until lightly browned.
4. In small bowl, mix all tangy dressing ingredients. Pour over greens. Toss.
6. Add fruit and almonds. Serve immediately.
