Innovative Clinic will build on south end success

Seven Oaks Hospital’s Access to Care Project will create a bright beautiful atrium for patients and visitors, a small pharmacy and gift

shop and an innovative new medical clinic. The clinic will be operated by Prairie Trail Medical Centre which has a successful group medical practice on Taylor Avenue near Kenaston in south Winnipeg.

Seven Oaks General Hospital Foundation, which is raising funds for the project, sat down recently with one of the clinic partners to

find out a bit more about the model. Dr. Kanwal Saran grew up in the Maples and still has family in the area.

The Foundation: Prairie Trail has created a successful medical practice in south Winnipeg that is seen as a model for other new clinics including Seven Oaks. What is it about your model that serves patients well and attracts new physicians?

Dr. Saran: Well, on one level it’s quite simple. We’ve been able to attract physicians and other healthcare professionals to our current clinic by supporting what they love to do... take care of patients. We've offered flexibility in schedules and a supportive work environment. We allowed within the concept the ability to expand their practice of medicine. Some of the physicians divide their time between our clinic, teaching medicine, working at other facilities, or working in acute care clinics or hospitals.

As a result we’ve been able to build a cohesive group that is able to work well together and supported to do what they do best which is seeing patients. We’ve found that if the doctors work well in a supportive environment it translates into good care for patients.

Using this concept, the clinic has grown to 13 physicians. The practice also includes a Shared Care counselor, a private psychologist, a psychiatrist, an in-house pharmacy, physiotherapy, an office manager and five other support staff.

The Foundation: There often seems to be a separation between the care at hospitals and the care physicians provide in primary care clinics, but your clinic seems open to more integration, more connections with the other parts of the healthcare system. Can you describe some of the collaborative and integrated activities at your clinic and how that benefits patients?

Dr. Saran: I mentioned the Shared Care counselor and psychiatrist who are part of the WRHA Mental Health program. We also participate in an Oncology Primary Care Network to foster better communication about patients between family physicians and Cancer Care Manitoba.

We participate in a Manitoba Health program called the Physicians Integrated Network in which we monitor and attempt to meet good practice indicators

We do have electronic medical charts and we will be participating in the E-Chart program which links patient charts with other healthcare facilities.

We’re affiliated with the University of Manitoba Faculty of Medicine as a teaching site such that physicians and staff are involved in teaching resident physicians and medical students.

Being more integrated makes it easier to access services for patients, connecting with specialists,

diagnostic services and other parts of healthcare. It brings back the concept of the doctor’s lounge in hospitals where physicians practicing from various backgrounds and specialties would interact.

The Foundation: Is the clinic at Seven Oaks going to operate in a similar way to the current clinic? Will there be differences? What will that mean for patients?

Dr. Saran: It’s a similar model but with the further advantage of being in the hospital. Clinic physicians will have the chance to have interactions with other colleagues in the healthcare world. It’s an exciting opportunity to build a healthcare team for patients.

We see primary care physicians as being in the forefront of healthcare... as the quarterback on the healthcare team. We are excited at Seven Oaks the family physicians will have the ability to walk into the hospital, see their patients, and be active members of the team throughout what are stressful times for patients in hospital.

The Foundation: Some clinics schedule in a way that allows existing patients to be seen sooner, when they need to, not weeks after they call to make an appointment? Will that be the case at the new clinic? How does that work?

Dr. Saran: We offer a same day clinic at our current site, and we’ll be looking to implement that at the new clinic so that patients who have more acute illness or injuries can call in the morning and be seen later that day. It's for patients of the physician group to be seen by a doctor of the clinic with access to the patient’s medical history and chart.

Generally Advanced Access means you can get an appointment when you need it. If you are looking for a

routine check-up or follow up for some ongoing condition, you might wait a bit more but it is our goal to provide access to timely care.

The Foundation: What else can we expect to see at the new clinic in Seven Oaks? Will there be some back and forth from the current clinic?

Dr. Saran: We’d like to have fresh new doctors with a desire to practice in the area bring new ideas and interactions to the group and let the group develop within the framework we’ve outlined. The concept will be same as the Prairie Trail clinic in the South but with we hope exciting new ideas and energy. The new group would be a welcome addition to the Prairie Trail family.

The Foundation: How does someone get to be a patient of the new clinic? Is there a waiting list?

Dr. Saran: We are in the recruitment phase of the project. Once we have the new team together, we will start taking patients to what we hope will be a exciting new clinic at Seven Oaks.

Contributing to Access to Care: The entire project cost is $7.6 million. Seven Oaks General Hospital Foundation has committed to raise $1.2 million from the community. Funds raised will support the atrium and construction, not operating costs for the new clinic.

Getting around

Seven Oaks during Construction

In order to complete construction of the new clinic and the new entrance/atrium, the current front entrance and part of one hallway on the main level will be closed from approximately mid-August to April 2012. A Temporary Front Entrance will be maintained throughout construction, as shown on the map.

To minimize inconvenience for patients and visitors Seven Oaks will:• Continue Information Desk service by the public elevators • Have Volunteer Ambassadors on hand to give directions• Designate McPhillips Street Entrance for Day Surgery + Elective Surgery drop-off and pick-up• Move Parking Payment and ATM near the public elevators • Temporarily relocate Gift Shop, Tim Horton’s and Vendors to the Dining Room (lower level)

Visitors are reminded that the driveway in front of the hospital is a one way road. Visitors should drive carefully during the construction period.

Seven Oaks General Hospital has appointed a prominent researcher

and Nephrologist as Director of Research.

Dr. Paul Komenda has been based at Seven Oaks for two years where he has treated patients in out patient kidney health clinics, provided inpatient consultation for nephrology and provided care in the Hemodialysis unit. He has served as medical director overseeing the expansion of Home Hemodialysis, as well as health services and clinical nephrology research.

Seven Oaks is hoping to attract research partnerships to study and invent new processes and programs to advance hospital and healthcare efficiency and improve the health of the community. As the youngest of Winnipeg’s hospitals, Seven Oaks has established itself as an innovator and change agent for healthcare.

Dr. Komenda received his medical degree from the University of Manitoba. He completed Internal Medicine Training at the University of Western Ontario and Nephrology Fellowship at the University of British Columbia. He went on to complete a Research Fellowship and a Masters of Health Administration at University of British Columbia specializing in epidemiology, health economics and health policy research.

While a student of health administration he was nationally recognized with the Robert Wood Johnson Award as the health administration student most likely to make a significant contribution to the

future of health care administration in Canada.

Dr. Komenda is a prolific researcher who has published over 30 peer reviewed articles in health services research and clinical nephrology since 2008 and he holds several provincial and national level grants. His current areas of research include establishing a provincial surveillance system for chronic kidney disease, and process engineering and health system improvement in the Manitoba Renal Program.

“There is quite a bit of synergy between my research work and the process engineering and innovation already occurring in the Seven Oaks Emergency Department,” Dr. Komenda said. “I see potential for Seven Oaks to also become a hub for excellence in clinical process engineering and epidemiology.”

As Research Director Dr. Komenda will continue his own, and collaborative research, as well increase the research presence at Seven Oaks and the Wellness Institute by inviting research proposals from other researchers.

His work in kidney health is a good fit with the direction of Seven Oaks which is becoming a centre for excellence in kidney health, and as a leader in supporting chronic disease management and healthy lifestyle approaches to preventing illness.

“I see research potential connected with the Wellness Institute programming to reduce risk factors for vascular disease and diabetes

because the consequence of uncontrolled vascular disease and diabetes is often chronic kidney disease,” Dr. Komenda said.

“There are opportunities for combining epidemiological surveillance with targeted interventions to prevent or delay chronic kidney disease from progressing.”

Dr. Komenda has assumed the role effective immediately.

Seven Oaks Woman of Distinction

Seven Oaks General Hospital’s President and Chief Operating Officer Carrie Solmundson has been named to the 2011 YM-YMCA Women of Distinction Awards in the Wellness, Healthy Living and Recreation category.

The 35th Annual Awards celebrate talent, achievement and innovation, and honour Manitoba women who have made a unique and exemplary contribution to the development of others. Carrie was nominated by Seven Oaks General Hospital Board of Directors Past Chair Stuart Greenfield and Board Member Stan Kruse. The nomination was supported by former Winnipeg Regional Health Authority CEO and current University of Manitoba Dean of Medicine Brian Postl as well as UNICEF Canada where she has served as a board member for six years.

The award recognized Carrie’s contributions to wellness and healthy living throughout her career, including the development of the Wellness Institute, the integration of health promotion with hospital and health care, and her commitment to health promotion for employees at Seven Oaks.

She has always understood that hospitals and healthcare cannot be judged only by the care delivered inside the hospital walls, but by the health status of the community they serve. Under her leadership the Wellness Institute has focused on serving people with chronic disease and barriers to health, helping them to make healthy lifestyle changes and prevent further illness and challenges.

“The challenge for healthcare is to find more ways to effectively support patients to become well and stay well,” she said about her award. “It isn’t about building fitness centres, it’s about building connections for patients between hospitals, doctors and health services in the community so patients can learn to manage their own health and prevent being re-hospitalized.”

Postl’s letter of support lauded her redevelopment of Seven Oaks’ role as an innovative community hospital with creativity and out of the box thinking. “She is a remarkable visionary – always looking for evidence to support change and always looking to change for the betterment of patient care. She is a dynamic leader.”

No one Dies AloneCBC radio program Tapestry aired

a documentary in April about No One Dies Alone (NODA) a program started by a nurse in Oregon to match compassionate volunteers with patients who are alone at the end of their life. Seven Oaks hospital has an active NODA chapter, mostly non-medical staff who volunteer to simply sit with patients in shifts and be there to comfort them at the end.

One of those volunteers, Joe Lekso, a maintenance worker, was interviewed for the program, and talked about why he volunteers, and the importance of the program. You can hear the program at the CBC website for Tapestry episodes at http://www.cbc.ca/tapestry/episode/2011/04/17/death-be-not-scary/

Seven Oaks in the News

One Sweet Affair pits Winnipeg’s most creative pastry chefs against each other to win the Grand Prix for best dessert. At the end of the night, these one-of-a-kind cakes are auctioned off to the highest bidder. All auction proceeds go towards raising funds for the Seven Oaks General Hospital’s Foundation.

The success of last year’s sold out Gala raised $51,000.00 and received great feedback.

The Foundation supports the Seven Oaks General Hospital efforts to improve its facility and programs for patients and the community. Proceeds of the 2011 Gala will help fund our Access to Care Project and other facility improvements and programs for better healthcare.

Changing the Way We Age?

“Changing the Way We Age®” is not just the theme of this year’s annual Active Aging Day open house at the Wellness Institute, or a

new way to head off wrinkles. It is a campaign initiated by the International Council on Activity and Aging (ICAA) that hopes to challenge stereotypes surrounding aging while encouraging older adults to be engaged in an active life.

As a member of the ICAA, the Wellness Institute was excited to hear about the campaign. Working alongside the Active Living Coalition for Older Adults in Manitoba (ALCOA-MB), we are proud to be champions of the message and to partner in promotion of the Active Aging Day open house on September 27.

The Wellness Institute has long recognized the varied needs and abilities of older adults in the community, promoting opportunities for us all to age actively. Programs, services, membership options and even community partnerships have been developed to help manage existing health conditions, prevent declining abilities and promote positive lifestyle changes for wellness at any age.

Our ongoing membership of ALCOA-MB allows us to partner to offer free seminars for older adults at our facility while housing the ALCOA-MB office aids our continuing collaboration to support active aging for older adults.

With “older adult” widely defined as 55+, the term covers a large and growing group. Among their numbers are many who are striving to stay active and who are not content to be defined by the stereotypical grandma and grandpa roles.

“Some people think that when you get older you should just sit in a rocking chair and wait to die… but it is best to stay on your feet,” says Eulah Matheson, an active Wellness Institute member at age 72.

“It is very important for me at this stage in my life because I have arthritis in both knees,” she continues, crediting continued activities such as aqua classes, walking and lifting weights for making housework and other daily activities easier. For her, an active lifestyle goes beyond exercising to include eating healthy, spending time with friends, cooking and community involvement through her local Guyanese cultural group. And she is just one example of how aging actively incorporates all the dimensions of wellness and engaged living.

To better understand the benefits of this approach, the Wellness Institute has partnered with the Canadian Centre of Activity and Aging (CCAA) for research into active aging and are the demonstration site in Manitoba for a National Embedded Learning – Health Literacy Project for Older Adults funded by the Public Health Agency of Canada (PHAC).
