NEWS FROM THE PRESIDENT

Staying Charged

“How do you have so much energy? I could never do it.” Energy, as defined by Webster is the actual or potential ability to do work. Energize is to give strength or force, and energetic described as acting with or exhibiting energy. The Affordable Care Act (ACA) 2010 brought about changes that would not only address affordable care, but challenged the health industry in changing the long-standing premise of treating the sick to that of promoting health (Kovner & Knickman, 2011). My energy derived from the goals I set out to accomplish in my youth, to become a nurse, to live with no regrets, and to continually challenge myself. In the words of Joyce Meyer’s “Do it afraid” (Meyer, n.d.). You cannot turn on the television, access social media, or pass a billboard without seeing promotions for healthy lifestyles. Health care professionals work together with legislators to seek out and promote behavioral lifestyle changes that will improve population health. Acute and infectious diseases are no longer the leading cause of death in the United States. Behavioral risks as tobacco and alcohol use, sedentary lifestyle and unhealthy diet accounted for more than 900,000 deaths in the year 2000 (Kovner & Knickman, 2011). The paradigm of healthcare will be changing from that of Physician based hospital care to that of Patient Centered Care team approach with the Nurse as a full team member verses the subordinate role of the past (Milstead, 2013). Healthcare is in the early phases of disease prevention and promotion of lifestyle changes in the United States.

Nursing must continually evolve to remain in alignment with the ever changing climate of healthcare and become involved in conversations regarding healthcare reform and patient advocacy. Licensed nurses, including licensed practical, vocational and advanced practice registered nurses, represent the largest numbers in the healthcare industry (Kovner & Knickman, 2011). These numbers reflect the strength behind our profession. Knowing that so many travel the same path as I do in healthcare allows me to know I have support, and leadership gives me a voice.

As for my personal energy, I work to maintain a balance in my life between family, work, religion, and that if I

take care of me first, I have more energy for the rest. Things I have learned to remain charged:

4:30 am works best to fit in a run or a workout tape E-mails checked twice a day and responded to daily detours procrastination It’s OK to make a goal with a variable date Conferences, volunteering and furthering education renew excitement in nursing Running a marathon is accomplishable Only expect of others that which you expect of yourself You only have to be willing, and the rest will fall into place

Conferences: Fall 2014 and “Spring” 2015

Please join us for the fall conference “Preserving the Knowledge” on October 4, 2014 in St. Cloud, Minnesota. The

planning committee, led by Karen Reinholz, has put together a wide variety of topics current to our perianesthesia

practice. The brochure with all the conference details, can be found on our website http://www.mndakspan.org. As

always, we look forward to networking and hearing about current issues in our region.

Wanda Osland is leading the planning committee for the 2015 Spring Conference. “Ever Changing Times” will be

held on February 28th at the same location as last year, the Ramada Mall of America in Bloomington. We are planning to

add something new this upcoming year. Friday the 27th we will offer a FREE additional half day conference. The half day

will focus on a general perianesthesia review. It will be a great opportunity for anyone new to PACU or for someone

preparing to take the certification exam. The traditional Saturday conference will be a full day with a variety of local

speakers.

The location for the fall 2015 conference has been secured. This conference will be held on September 26, 2015

at The Inn on Lake Superior in Duluth, Minnesota. If you are interested in serving on the planning committee please

notify any of the board members.

Conference Cancellation Policy

Full Refund 2 weeks preceding conference dates. Fourteen to two days preceding conference less a 20% administrative fee. No refund 24 hours

prior to conference. Due to the expenses that are incurred during the planning of this event, there will be no refunds issued if the event is

cancelled for any reason including but not limited to, bad weather, natural disaster, or acts of terrorism.

I recently sat down with one of my colleagues, Kelly Zschoche, who recently passed her CPAN examination. I wanted to

congratulate her and discuss with her things she had learned and found helpful in studying for the exam. We first talked

about why become certified? Kelly felt that certification is important for professional growth and that she learned a lot

of new things.

Some advantages to being certified might be helping your hospital to achieve magnet status. In our institution we

receive an annual bonus for being certified. Being certified can also be a part of our professional nurse advancement

program which does bring a salary increase.

Kelly feels that working her hours in periop also helped her to be ready to take the certification test. Eighteen hundred

hours of current bedside PACU/SDS experience in the last two years is required for eligibility to take the exam. She also

used a review DVD and took a certification review course which helped give her an idea of what to study. She took a

practice test, which is available for a fee on the ABPANC web site. The tests are frequently buy one/get one free. She

used review books that had questions and answers with rationale. So Kelly, thank you for an enjoyable evening, and

congratulations on your certification!

® Certification is Growing!

The role of nursing certification is becoming increasingly important throughout all healthcare institutions and our numbers are growing. Nationally, we now have a total of 11,089 CPAN and CAPA certified nurses representing a 6% growth over last year.

Ruth Novack and Cindy Swenson ASPAN Leadership Development Conference leaders and attendees

Nashville, TN was the site of this year’s ASPAN Leadership Development Institute (LDI) Conference, at the Millennium Maxwell House Hotel, September 5th-7th, 2014. MNDAKSPAN participates in this conference annually by sending two elected officials to participate and bring back these strategies to incorporate into the MNDAKSPAN structure. Our biggest challenge was battling the heat and humidity of Tennesee, but that was offset by the abundance of educational opportunities that we brought back to strengthen our component. Components from across the United States send officials and representatives to attend this conference, which then offers opportunities for breakout sessions and networking. MNDAKSPAN is part of Region 3 which consists of components from several states including:

Illinois Society of PeriAnesthesia Nurses (ILSPAN) Indiana Society of PeriAnesthesia Nurses (INSPAN) Kentucky Society of PeriAnesthesia Nurses (KSPAN) Michigan Association of PeriAnesthesia Nurses (MAPAN) Minnesota-Dakotas Society of PeriAnesthesia Nurses (MNDAKSPAN) Ohio PeriAnesthesia Nurses Association (OPANA) Wisconsin Society of PeriAnesthesia Nurses (WISPAN) West Virginia Society of PeriAnesthesia Nurses (WVSPAN)

Presidents from these groups and their representative gathered together to discuss current trends and share positive outcomes on conference planning. For example, WISPAN has developed a family weekend conference that is held at the Wisconsin Dells. The conference offers two half-day seminars for a total of 7.0 contact hours. Each day is completed by noon to allow families and participants some downtime to enjoy the sights and play. This is one of the many creative ideas to be shared that MNDAKSPAN will survey their membership and develop determined on interest. An interesting topic was that of Social Media. Facebook remains the largest base of social use with Google +, Pinterest, LinkedIn, and Twitter also increasing in their popularity. Social media provides MNDAKSPAN the opportunity to keep our members updated and provides a platform for interaction between nurses. Although social media provides an area of fast access, it comes with professional responsibilities, and several points were mentioned in working social media into your practice. As professionals it is imperative to remember, you can and will be held accountable by your organization for any misuse of social media that violates HIPPA. Make it your practice to know the American Nurses Association (ANA) has developed 6 principles related to the nurse and social media use. The American Society of PeriAnesthesia Nurses (ASPAN) has Position Statements on both pros and cons of social media (http://www.aspan.org) The promotion of Evidence Based Practice (EBP) through ASPAN continues. We were given tools to stimulate our inquisitive minds to delve into the unanswered questions of our practice. Those things we continue to do because we

have always done it that way are now becoming supported by evidence. Through your membership to ASPAN, you have access to the Johanna Briggs Institute. “JBI” offers a plethora of systematically reviewed articles and use of the OVID search engine when looking for those answers to support your work. MNDAKSPAN offer a Research Scholarship--see our website for further application information and share your evidence based research (http://mndakspan.org). As the conference title suggests, leadership was infused throughout the conference material. Sharing and networking with multiple colleagues gave opportunities to receive and share information and visual improvements that will inspire growth of the MNDAKSPAN component. We thank our component members for entrusting us to bring back to you what we have gleaned from this conference and disseminate it to you, the component member.

The recipient of this award will be announced at the MNDAKSPAN 2015 Spring Conference and every Spring Conference

biennially thereafter. MNDAKSPAN will evaluate the expenses (conference registration and hotel fees, coach airfare)

involved on a biennial basis to attend the ICPAN conference and reimbursement for conference registration fees, hotel

costs, and coach airfare will be granted based upon MNDAKSPAN’s current financial status and budget for that year. The

Component Excellence Award application and information can be found on our website at

http://www.mndakspan.org/membership-awards.php.

Applications can be submitted anytime between now and January 31st 2015 by following the instructions on the

Discussion regarding board members visibility during the

NSF / spring conference

Karissa spoke regarding our last conference and that we had 2 NSF

checks that were unable to be collected on from walk in

registrations. Discussion followed on how to avoid these. Cindy

will investigate the possibility of software used by an iPhone to

accept debit and or credit cards.

Pay Pal

Cindy will be getting training for the pay pal system.

Nonprofit vs charitable status

MNDAKSPAN is not a nonprofit but a charitable organization.

There needs to be close attention of how many funds we have in

our accounts due to this. It is recommended that the net worth be

under $60,000 especially at the end of the year. Cindy will have

the new accountant complete the official audit and seek his advice

regarding how things are set up.

Financial report

Wells Fargo account advisor recommended we move some of the

funds out of the checking (non interest account) to a saving

account.

Motion made by Mary and second by Paula to give the treasurer

the permission to transfer funds to a savings account keeping

$4000-$6000 in the checking account.

bills that were at the Senate. Learned how to ask and have a voice to get

additional funding for nurses. Encouraged all to give as those organizations

Fall conference is going well for St. Cloud. There are topics and

speakers lined up. A save the date was sent out. Update to the

website needs to be done.

Cancellation wording for the brochure was discussed and agreed

Spring 2015

Spring 2015 Plans to be back in Bloomington with a variety of

speakers instead of a national speaker. Julie felt we had plenty of

options to choose from. Discussion regarding offering a half day

review course for the Friday which would be presented by our

membership.

Motion was made by Cindy and seconded by Paula to trial this new

model of a half day review course put on by our own speakers.

Fall 2015

Fall 2015; Ruth has investigated having this conference in Duluth

September 25 & 26th

2015. She had contacted the Inn of Lake

Superior. This center can accommodate 180 people and the room

rate would be $159 a cost savings from $259.

The consensus was to reserve this and make this the plan.

Discussion followed regarding having a half day conference

instead of a full day.

Considering Exparel

Julie Oppold, MNDAKSPAN Director of Education, sent the following anecdote to the board, and at the request of the board, agreed to have it reprinted here: “Sometimes we so overwhelmed in our work trying to offer comfort to our patients. This week I had a great experience I wanted to share. We have done amazing things with pain control for our total knee patients. I was caring for the patient next to a total knee patient who was quite a talker and a singer. The ‘singer’ had general anesthesia, not a spinal, and he was not confused, just happy. (Wow, what progress we have made in post op care!)

They provide preop classes in a total joint class where they meet with others planning joint replacements. They have an excellent preop medication plan and they use Exparel in the OR. Not all of our total joints come in to us singing, but overall we use much less pain medicine.

My patient and the others around him didn't know what to think. My patient thanked me and said I was a good nurse. I told him "well not as good as the nurse beside us, because you aren't singing!" He laughed and just smiled. We all were laughing, because the singer was crooning, "all my ex’s live in Texas!"

To accompany this great patient care tale, here’s a word about Exparel from an anesthesiologist concerned about his colleagues understanding its use:

“It came to my attention that during a case today a surgeon asked the anesthesiologist if they could use more local after

a block with liposomal bupivacaine (Exparel) was done several hours earlier. The anesthesiologist stated they shouldn't

give any more local. However the surgeon decided to give some anyway, partially influenced by others in the room

stating that other surgeons do it all the time without any problems! This is a big issue in that it could have caused the

patient to develop local anesthetic toxicity which could lead to intractable seizures, heart block, and even death.

Please send the attached alert to staff so they are educated re the administration precautions and risks with liposomal

bupvacaine and inform them if ever any questions to call the anesthesiologist.”

MEDICATION ALERT:

Liposomal Bupivacaine (Exparel)

This is a formulation of local anesthetic that may have plasma levels last up to 96 hours post-injection with analgesia up

to 72 hours post-injection.

Some of the key safety information regarding Exparel:

- Liposomal bupivacaine(Exparel) is not approved for on-label use under age 18 - If liposomal bupivacaine is admixed with non-bupivacaine local anesthetics such as lidocaine, it may result in

release of all liposomal bupivacaine and possible local anesthetic toxicity - Once liposomal bupivacaine has been administered, NO other local anesthetics are to be administered into

the patient for 96 hours post injection, as there is a risk of local anesthetic toxicity which can lead to intractable seizures, heart block, and possible death.

- Liposomal bupivacaine cannot be used in conjunction with peripheral nerve catheter, epidural, or wound infiltration catheters.
