National Nutrition Council (NNC) and Nutrition Cluster partners conduct

Nutrition Summit in Tacloban City The National Nutrition Council (NNC) in Region 8 headed by Dr. Catalino Dotollo, Jr. in coordination with ACF and UNI-CEF conducted a Nutrition Summit for nutrition stakeholders from the provinces of Leyte, Southern Leyte and Biliran. The Summit, held on July 31, 2014 at the Leyte Park Resort Hotel, was part of the culminating program of the Nutrition Month celebration and advocated for Nutrition resiliency and disaster preparedness, consistent with the theme of the 2014 Nutrition Month, ―Kalamidad Paghandaan; Gutom at Malnutrisyon Agapan‖. Over 300 registrants participated in the Nutrition Summit with representatives from municipalities in Leyte, Southern Leyte, and Biliran, including the MNAOs and BNS. Aside from the participants from the government sector, the humanitarian com-munity joined the summit. Different booths showcased nutrition advocacy in the participating municipalities. The topics discussed during the event ranged from Nutrition and IYCF in Emergencies, Nutrition in the First 1000 Days of a Child, Health System Strengthening, to Generation Nutrition, a global nutrition advocacy effort. The MNAO of Limasawa and the CHO of Davao City shared their innovative strategies in making their Local Government Units nutrition role models. The LGU of Limasawa, an island municipality prone to various disaster hazards due to its location, received the Nutrition Honor Award - the highest award achievable - for its highly successful nutrition program. The City of Davao has recently implemented the PIMAM program providing treatment to malnourished children and funded by city funds. The Governor of Leyte, Hon. Leopoldo Dominico L. Petilla, graced the Summit with his presence and shared the plans of the province to strengthen its nutrition programs. The vice-president of the League of Municipalities of the Philippines, Hon. Remedios L. Petilla, led the ceremonial signing of the 10-point agenda of Generation Nutrition and Pledge of Commitment.

Community Based Management of Acute Malnutrition (CMAM) program in Region VIII—ITP and OTP

Number of ITP sites and number of children admitted (June 25—July 25)

Severe Acutely Malnourished (SAM) children without complications are referred to Out-Patient Treatment (OTPs). OTPs are either static (i.e. at the Rural Health Unit) or mobile and operated by Nutrition Cluster partner organizations. Children identified as SAM in municipalities without an operational OTP are referred to the closest OTP in a neighboring municipality. Severe Acutely Malnourished (SAM) children with complications are referred to In-Patient Treatment (ITPs).

Co-Creating a Nutritionally-well Society By Selahuddin Yu Hashim Executive Director of Health Organization for Mindanao (HOM)

Eastern Samar. In February 2014, 23-month-old Jana was brought to the rural health center of Quinapondan. Jana is one of more than 21,000 under five children in Eastern Samar who were screened using the mid-upper arm circumfer-ence (MUAC), which determines the nutritional status of 6-59 months children. She was identified with Severe Acute Malnutrition (SAM) as eighty-two (82) other children in the province. Jana is the youngest of the three children of Lisa Rosaldo (21 years old and pregnant for 8 months). During the screen-ing, the Barangay Nutrition Scholar who measured Jana was very concerned about her physical state: ―She was crying the whole duration and you can feel her discomfort. She was very thin and small with a height of 73cm and a weight of 6.9kg. Her MUAC is 119 mm and a Z-score of below -3‖. Jana’s late referral to the RHU is an indication that many parents do not really understand malnutrition, its causes, treat-ment and how to prevent it. Additionally, many RHU staffs also have limited knowledge about the underlying and immediate cause of malnutrition. Most of them have only heard of the Community-based Management of Acute Malnutrition (CMAM) after Yolanda dev-astated their province. Ms. Sallie Javinal, the Provincial Nutrition Action officer (PNAO) of Eastern Samar, related: ―The province has a nutrition program but is very weak.The budget allocated to nutrition is very meager and is not able to cover the implementation of nutrition-promoting activities. It gained less support from the local government unit. When Typhoon Haiyan (Yolanda) devastated most of the areas of the province, several international organizations such as UNICEF, ACF and HOM arrived in our place and introduced the concept of CMAM. At first I was skeptic, but with thor-ough discussion and evidence-based presentations, it changed my heart and mind. I was really hoping that we could implement the program in our province immediately.‖ Jana’s case tells a story of the need for a comprehensive and innovative approach in addressing malnutrition. During the CMAM verification and admission, the Public health Nurse of Quinapondan RHU related that ―Jana’s parents are shy and they didn’t bring her during the screening. It was only during one of the counseling sessions on Infant and Young Child Feeding (IYCF) that Jana’s mother brought her. The BNS saw her condition and immediately referred Jana to the RHU after doing initial screening.‖

After the RHU staff explained to Jana’s mother the benefits of CMAM, her mother decided to enroll Jana in the OTP pro-gram and pledged to strictly follow the procedures and treatment requirements. Quinapondan’s Municipal Nutrition Ac-tion Officer (MNAO), Ms. Pacita, requested HOM’s IYCF Counselor and Mental Health Specialist to provide series of counseling to Jana’s parents, because it seemed that the parents are having other domestic problems, which may affect the quality of care practices and treatment. HOM staff responded with a series of cognitive behavioral therapy and IYCF counseling. A series of home visits were also conducted in order to ensure faster recovery of Jana. ―After 2 weeks of follow-up, the mother was really complying with the treatment of her child which made a very good re-sult. Jana gained 1kg,‖ said by Jow Bangen, HOM Nutrition Supervisor. Jana’s mother, Lisa, also said: ―We are very happy with the results we are seeing. According to our neighbors, Jana is really gaining weight and becoming round.‖ As she was talking about the improvement of her child, the smile on her face was visible, she was really happy. After the 4th week of follow-up at RHU, Jana was already in a normal range, but she has to stay in the program for an-other week to make sure that she maintains her weight gains. Five weeks after, Jana was brought by her mother to the RHU and this time she was smiling all over. The RHU staff no-ticed that Jana was even playful and lively. ―When we took her measurements, her MUAC increased to 139mm, her weight also reached 8.8kg and height of 73.3 cm while her z-score is now -1.5. For two weeks, her MUAC and Z-score were consistently normal. Even after we discharged her, we still continue to monitor her status and provide counseling to her mother,‖ the BNS said. Even after discharge, HOM’s MHPSS Specialist continues the counseling. Up to now, Lisa still regularly attends the Care Practices Session and IYCF Counseling. She shares her and Jana’s story to encourage other parents to comply and change their behavior towards ensuring the health and nutrition of their family member. The CMAM program is now being adopted by a number of organizations in the Philippines focusing on health and nutri-tion. Moreover, various agencies and organizations are now working to develop a national policy that can make this treatment available to all children in the Philippines who need it. Infant and Young Child Feeding (IYCF) programs in Region VIII

Infants and young children are more vulnerable to infections and disease, especially in emergencies due to their lower immune function. Optimal breastfeeding signifi-cantly reduces mortality of children under-five years old. Nutrition cluster partners are working to promote and protect exclusive breastfeeding for infants 0 – 6 months and adequate complementary feeding for feeding for children 6 – 24 months, through community level counseling, establishment of mother/baby friendly spaces and monitoring violations of the milk code¹.

Nutrition Cluster Region VIII — IYCF Supportive Supervision

Improving Infant and Young Child Feeding in Yolanda’s Affected Ar-

eas: Supportive Supervision for improved efforts (please add pictures

of the visits)

The Nutrition Cluster is supporting the Department of Health and the National Nutrition Center in rolling out a community strategy aimed at establishing effective IYCF counseling services. A new Community based track-ing and monitoring tools for IYCF has been provided and rolling out of the same is underway.

Follow-up (supportive supervisory) visits are essential to ensure that local health and nutrition workers and volunteers are provided with sufficient guidance and on the job updates/inputs so that they can improve/offer better quality services.

Between June 25 to 25, a total of three (3) provinces in Region 8 were visited, and one (1)city/municipality per province was selected. The visits were coordinated by UNICEF Field Office in partnership with the different NGOs partners operating in the three (3) municipalities.

Meetings with MHO/MNAos provided helpful insights on the progress made in improving IYCF practices in the locality.

Discussions were facilitated with more than 80 Barangay Nutrition Scholars and Barangay Health Workers from the three (3) municipalities.

Community visits in two (2) of the three municipalities were conducted where more than 10 mothers with chil-dren less than 2 years of age were interviewed.

Two (2) group discussions with mothers (5/6 per group) were facilitated, the mothers were asked to share their experiences in feeding their children and also their view of the IYCF issues in their respective communities.

Objectives

The supportive supervision visits aim at:

1. Data and information driven review of progress of IYCF implementation in selected municipalities affected by a disaster

2. Identify strengths, weaknesses and opportunities

3. Provide on the job support to partners, govern-ment and volunteers

4. Agree on next steps and plans to improve IYCF in the selected areas

5. Agree on follow up visits

Expected Outcomes

1. Identified strengths and areas for improvement

2. Agreements on next steps both at barangay and municipal level

3. Improved monitoring and tracking system using the IYCF/MUAC tools

4. Agreement for follow up visit (barangay, munici-pality)

Nutrition Cluster Region VIII — IYCF Supportive Supervision

Findings:

1. Information dissemination, not transformed yet in counseling efforts

2. No health system participation in the work of the BNS/BHWs

3. Supportive supervision should happen at the barangay level, wherein NGO partner should/could facili-tate development of skills/knowledge of supervisors to help the local health and nutrition volunteers

4. Degree of the use of the tools varies according to areas, the area supervisors/monitors needs to exert efforts towards the review, processing and ownership

5. Barangay captains call for more efforts towards the participation of midwives in the process

Recommendations:

1. To increase coverage it is strongly recommended that health facilities start providing IYCF counseling to all mothers of children 0-23 months visiting the facility during the immunization schedule; We can explore the possibility of having an IYCF desk/corner and BNS/BHWS can be asked to go on duty dur-ing immunization day; Those mothers found practicing sub-optimal IYCF practices will then be followed up at their residence

2. Ensure integration and coordination of existing efforts with health system (nurses/midwives)

3. Step up supportive supervision efforts focusing on improving quality of counseling services rendered, data collection, review and processing

4. Support MNAO in engaging the barangay leaders and creating a supportive environment for the health and nutrition volunteers;

5. Closely support and supervise the utilization of the community based nutrition recording and tracking system
