Introduction Since the founding of Ellis Funeral Home in 1954, the policy to provide full information to the public

about funeral arrangements has always been our desire. Today, when individual attitudes require a

funeral charges to be informative and helpful when considering service details. Greater satisfaction

will come to those who carefully consider the value of all details offered to fulfill their personal

needs. The following price quotations are provided at this time to assist you during these considerations.

Our Complete Traditional Service Package Includes: Basic services of funeral director

and staff, embalming, other preparation of the deceased, dressing and casketing, use of fa-

cilities, removal to funeral home, one limousine, hearse, lead service car, register book and

thank-you cards (plus cost of casket of your choice selected from our facility offerings via

Outer Burial Container Price List Prices are effective July 20, 2015 and are subject to change without notice.

In most areas of the country, state or local law does not require that you buy a container to surround the casket in

the grave. However, many cemeteries require that you have such a container so that the grave will not sink in.

Our funeral home makes no representations or warranties about the protective value of certain caskets or outer

burial containers other than those made by the manufacturer. The only warranties expressed or implied, granted in

connection with the goods sold with this funeral service are the expressed written warranties if any, extended by

the manufacturer thereof. No other warranties of merchandising fitness for a particular product are extended by the

seller.

**This list does not include prices for certain items that you may ask us to buy for you, such as:

Cemetery services, flowers, programs and newspaper notices, etc. The prices for those items will

be shown on your statement describing the funeral goods and services you selected. We charge you

for our services in buying these items.

***Regarding Assistance from the Township Trustee or Division of Welfare*** For those families who qualify for aid from the township trustee burial fund or welfare (Medicaid), we will

provide a minimal chapel service with 30 minutes of viewing prior to the chapel service to total 1 hour of

total service for whatever funds the agency provides, including our choice of casket. Services and burial

must be held Tuesday through Thursday. Funding from both agencies cannot be combined.

FINANCIAL PLANNING AND CREDIT POLICY We know that many times a death comes at a time when you, the survivors, are the least pre-pared financially. A funeral is not meant to become a financial burden for anyone. We encourage the frank discussion of these matters and assure you that it is our goal to assist you in any way possible.

Before you leave today, your funeral director will discuss the following terms for payment with you and provide you with an itemized list of the goods and services you selected.

We offer these convenient options for the payment of the services you selected:

Option 1. Cash/Check payment in full four (4) days prior to services. Please make checks payable to: Ellis Funeral Home L.L.C.

(A $25.00 Service Charge Applies on Returned Checks) Option 2. You may request to pay using DISCOVER, VISA or MASTERCARD.

All cash advances applied to a credit card are subject to an additional surcharge

noted on the funeral bill as a convenience charge.

Option 3. As a service to all our families, we will honor an insurance assignment to be

completed at the time of the arrangement conference and the policy provided to us. There will be

no additional charge for our assistance in filing for the insurance. The insurance policy (ies) must

be in force for two (2) years and not contestable. All insurance paperwork must be completed and

signed 48 hours before funeral services. Beneficiary (ies) must be verifiable. *If you choose to file your own insurance, payment is expected upon completing funeral arrangements.

We do not invoice estates. Victim assistance: Must be paid in full. The agency will follow up with you if you apply.

Cash Advances: If we have assisted you wit h a cash advance f or it ems such as cemetery

charges, newspaper not ices, honorariums, or certified copies of death, payment is t o be made

for these charges four days before the service, unless other arrangements have been made.
