Each year we have one or two registrars who are qualified doctors that spend a period of 12 months for further training and experience in general practice and foundation doctors who spend four months in general practice. The Practice is also involved with the training of Medical Students, Practice Nurses and Other staff. The reception staff will inform you when a student is present with a particular doctor or nurse. North Preston Medical Practice Park View Surgery Park Medical Practice Staff at the locality clinics will provide a range of appointments from Practice Nurses who will be available to provide chronic disease reviews, cervical screening and childhood immunisations along with pre-bookable GP and other Health Care Professional appointments. The majority of the Increased Access appointments are held at Broadway Surgery, Penwortham St Mary's and Briarwood Medical Centre. The appointments are with Locum clinicians including our very own Dr Joanne Murray, Dr Nidghtta Anjan and Dr Dawn Webster, Sister Janet Melling and our Assistant Practitioners Mrs Aimie Bell and Ms Alicia Grimshaw. You can contact the locality clinics on 01772 529200 at the following times: Monday - Friday (incl. Bank holidays) 18:30-20:00 Saturday 8:00 - 13:00 Sunday 9:00 - 12:00 Please note that once hub availability is full you may be directed to an alternative service.

Please also be aware that the administrative staff working at the locality surgeries do not have access to patient’s full medical records and therefore will not be able to answer queries on referrals, prescriptions, test results, private work and entries to medical records.

If you are looking for a weekend appointment, please contact our reception team who can help arrange this for you.

In a genuine emergency you should call 999. Chest pains and/or shortness of breath constitute and emergency. ROUTINE APPOINTMENTS: You will normally require an appointment to see your doctor or practice nurse. You will be registered with a named doctor; however you are free to choose any (available) doctor you wish for your appointment. To book an appointment, please contact the receptionist. Routine appointments are booked at 10 minutes intervals. If you feel that your problem(s) will take longer, please ask the receptionist to book a double appointment

URGENT AND EMERGENCY APPOINTMENTS

Telephone the surgery after 08:00 am. An urgent appointment will be made by one of our Reception Care Navigators who have been trained accordingly. In order to assist them in this, they have been asked to gather as much information as possible regarding the nature of the problem that the patient is calling about. We would sincerely appreciate the co-operation of our Patients in allowing them to do this. Your Urgent appointment will usually be with an Urgent Care Practitioner, however if they feel that the problem requires input from a GP, they are then able to bring them into your consultation.

TELEPHONE ADVICE If you wish advice from the Practice Nurse or Doctor, Please inform the receptionist who will take the necessary details to arrange this with the Practice Nurse or Doctor. You will be asked for a contact telephone number. Telephone Consultations with a GP are available on a daily basis in the morning and afternoon.

APPOINTMENTS & PRESCRIPTION REQUESTS ONLINE EMIS Access is a convenient way to book appointments with a GP over the Internet. It is quick and convenient to use, and can be particularly useful during those times when the Practice is closed or telephone lines are busy. Easy to follow screens and prompts ensure it’s simple for you to view, book or cancel appointments online – whatever time of day or night. Please ask at Reception for further details.

PATIENTS ON LONG-TERM MEDICATION AND APPROVED FOR REPEAT PRESCRIPTIONS CAN PLACE ORDERS IN A NUMBER OF WAYS:

In person - Drop your prescription in the post box in the surgery reception. By post - Send it in with a stamped addressed envelope if you want it posted back to you. Online - Once you have registered for online services, follow the link at the top of this page. Our telephone line - Our prescription line is available between 10am-1pm and 2pm-6:30pm Monday-Friday. It will also be available between 6:30pm-9pm on a Wednesday evening and 8a,-11am on a Saturday morning. HOME VISITS: We offer home visits to those who are house bound patients or the seriously ill who are unable to travel to the surgery. Please make requests for home visits early in the day, before 10:30 am on the day required. Doctors will attend home visits after the morning surgery. Please give the receptionist necessary details in order that the doctor may assess the urgency of the visit. Please leave any helpful directions for the doctor to get to your home.

OUT OF HOURS: If you require medical attention when the surgery is closed, please contact NHS Advice on 111. Please find below the times when you can contact NHS 111 Service:- Monday - Friday - From 6:30 p.m. to 8:00 a.m. Saturday & Sunday - 24hrs REPEAT PRESCRIPTIONS: Requests for repeat prescriptions may be arranged by leaving the repeat prescription request slips in the Post box at the surgery reception or via on line access (please ask at reception). Written requests can be returned by post if accompanied with a stamped addressed envelope. Local pharmacies operate a free collection and delivery service if required. Repeat prescriptions are monitored by the doctors, who may request that you periodically attend for a medication review. Two (2) working days (48 hours) notice is required to process requests for repeat prescriptions. Electronic Prescriptions: You can now have your prescriptions sent electronically to a Pharmacy of your choice who has the appropriate software installed. Please advise a member of staff or your GP if you wish to take up this option. Repeat Dispensing: If you have been on stable medication for 6 months or more, you could join the Repeat Dispensing Scheme. This will enable you to have 6 months of medication ordered at the same time. Each month you would just collect your prescription from your nominated pharmacy without the need to order from the surgery every month. Please ask at reception or discuss with the practice nurse at your next review. ACCESS: Bus Route near to both surgeries Car Parking is available on the Surgery grounds and nearby DISABLED ACCESS Wheelchair access is available at both sites. Reserved parking areas are available. Broadway patients should inform the receptionists if you are unable to climb stairs. There is a lift at the Broadway Surgery site for patients who require assistance to the upstairs consulting rooms. CANCELLING APPOINTMENTS Please let the surgery know if you are unable to attend for your appointment. Missed appointments result in other patients having to wait longer for appointments. Failure to attend multiple appointments without a reason will result in your removal from the Practice List COMMENTS OR COMPLAINTS The practice welcomes comments and suggestions to improve its service for you.

Complaints are dealt with according to the NHS Complaints Procedure and should be addressed to the Practice Manager. Please ask the receptionist for a leaflet, detailing “how to make a complaint”.

PATIENT PARTICIPATION GROUP We have now established a Patient Participation Group (PPG) and we are looking to ensure that the members we have are representative of our practice population. If you are interested in joining the group, please contact the surgery and we will provide you with an application form or visit our website at http://www.broadwaysurgery.nhs.uk/ppg.aspx . All patients are welcome and we would be particularly interested in hearing from younger, disabled and ethnic minority patients as we have no representation from these groups at the moment. NEW PATIENTS You must provide documentary proof of identity and proof you live in our Practice Area to register.

The practice recommends that all patients should attend for a New Patient Medical. However, if you are not on any medication or over 16

years of age and do not wish to have a New Patient Medical, please tick the box on the New Patient Questionnaire that you are given on

registration to opt out. If you are on medication, the reception team will book you an appointment to see one of our Assistant

Practitioners. Please bring a list of your medication and a urine sample to your appointment. If you do not attend your new patient check

appointment, you will be advised to re-book the appointment and your prescription will be limited to 7 days until you attend. NON-NHS SERVICES The doctors offer certain services not covered by the NHS, these services include the completion of Insurance Forms, Driving License Medical Examinations, letters and Passport applications and so on. The current fees for these Non NHS services are displayed in the reception area. If you wish further details or require an appointment, please contact our reception staff. FOREIGN TRAVEL Advice and vaccinations for foreign travel is available from our Practice Nurses. We encourage you to make telephone appointments to discuss travel advice and vaccinations at least six weeks or earlier before travelling. Please ask at reception for a travel vaccination leaflet which will give you more information on what vaccinations can and cannot be given under the NHS. Note: Fees are payable for many travel vaccinations. MOBILE PHONES We operate a low tolerance policy for mobile phone use. Please do not use them in the consulting rooms. Please turn your phone on silent before entering the building. REFERRALS Your GP will discuss this with you when appropriate. FAMILY PLANNING SERVICES Family planning and a range of contraceptive services are available from all the doctors and Practice Nurses. A Women’s Health clinic will be help on the last Tuesday morning of every month with Dr Rangaswamy. You can also attend local Family Planning Clinics for IUCD and Implant advice. INFORMATION ABOUT YOURSELF The main reasons for which your information may be needed are:

Helping us to review the care we provide to ensure it is of the highest standard. The Training and education of staff; which you can choose whether or not to be involved personally. Research approved by the local Research Ethics Committee. You will be contacted to see if you are willing to take part, you would not be identified in any published results without your prior agreement. If at any time you would like to know more about how we use your information, you can speak to the person in charge of your care. You have a right of access to your medical records. (See separate paragraph) Everyone working for the NHS and the Practice has a legal duty to keep information about you strictly confidential. CHANGE OF PERSONAL DETAILS Any changes to your name, address or telephone/mobile numbers etc should be notified to the surgery as soon as possible, so that we may update your records accordingly. If you change address and wish to change surgery site, please inform the receptionist who will arrange for the transfer of your medical records. If you move out of the Practice area, you may, in certain circumstances, be removed without notice. Note: you must remain in our Practice Area to remain registered.

PROMOTING GOOD HEALTH & DISEASE PREVENTION The Practice is committed to a policy of promoting good health and preventing disease. We encourage a no-smoking policy. There is no smoking permitted on the Practice premises or surrounding area. Health promotion advice is available to all patients if required

ADVICE AND INFORMATION If you are unclear about the advice or treatment given to you, please do not hesitate to ask us any questions. Please remember that the staff are not authorised to disclose confidential information especially over the telephone to a friend or a family member without your consent. ACCESS TO MEDICAL RECORDS Patients may apply for access to their own medical records, or may authorise a third party, such as a solicitor, to do so on their behalf. Parents may have access to their child’s records if this is in the child’s best interest and not contrary to a competent child’s wishes. Medical information may be withheld if it is likely to cause serious physical or mental harm to the patient or a third party who has not given consent for disclosure. Formal applications for access to records must be in writing accompanied by written consent. PATIENT RIGHTS AND RESPONSIBILITIES The Practice staff will make every effort to ensure you have the best care and service available. In return we would hope from you will show courtesy towards all members of the Practice Staff. Inform us if you are unable to keep an appointment. Missed appointment results in others having to wait longer for an appointment.

ZERO TOLERANCE The Practice is committed to the Government’s policy of Zero Tolerance with regards to threatening, abusive or discriminatory behaviour or comments towards members of staff. Violent or abusive behaviour towards any member of staff will not be tolerated, which will result in your immediate removal from the Practice List

SELF TREATMENT OF COMMON ILLNESSES

Colds & Flu These usually start with a runny nose, temperature, aches and pains. Antibiotics will not help as these illnesses are caused by viruses. Keep warm, rest and take plenty of fluids. Paracetamol taken every 6 hours will relieve the worst of the symptoms. Sore Throats Again viruses mainly cause these. Unless the patient is really unwell or has spots at the back of the throat antibiotics are unlikely to help. Keep drinking fluids and take Paracetamol every 6 hours. Diarrhoea & Vomiting These illnesses are usually self-limiting. Avoid food for at least 24 hours or until the symptoms ease. Drink plenty of water or electrolyte solutions (e.g. Dioralyte or Electrolade available from the Chemist and avoid any milk and dairy produce for several days after you are better. In small children and babies you may need to telephone for advice if the symptoms persist. Head lice & Worms Both of these problems are easily dealt with by treatment available from the Pharmacy. Head lice prefer clean hair and are not always a sign of poor hygiene! Burns Apply cold water to the affected area immediately and apply a clean dry dressing. For severe or widespread burns consult the doctor or practice nurse as soon as possible. Sunburn Avoid exposure to strong sun and use a good sunscreen with a high protection factor especially for children. Otherwise treat as burns and drink plenty of fluids. Back Pain The best answer is prevention – learn to sit, bend and lift correctly and generally keep fit. If you do have pain bed rest is advisable with regular analgesics e.g. Paracetamol or Ibuprofen. Consult the doctor if the pain is severe, fails to ease or causes problems in the legs, bowel or bladder disturbances. Nose Bleeds Sit in a chair, bend head slightly forward and squeeze the nose firmly below the bone for 10 minutes. Avoid the temptation to blow the nose or sneeze for 24 hours. If symptoms recur consult the doctor. Insect Bites & Stings WASPS - Apply vinegar to neutralise the sting as soon as possible. BEES - Scrape away the sting and apply a little bicarbonate of soda solution as soon as

possible. Generally keep antihistamine cream or tablets in the house to use when necessary (and particularly when travelling abroad) to soothe the itch and soreness. Fever in Children These are most commonly caused by viruses, especially in toddlers. Keep the child cool and give regular Paracetamol syrup (eg. Calpol); according to the dosage instructions on the bottle and plenty of drinks. Children often lose their appetite so do not worry about meals if they are not interested. Consult the doctor if the child seems particularly unwell or, is not improving after 48 hours. Contact the Health Visitor on 0300 247 0040 for advice on the under 5’s Chicken Pox On the first day a rash appears as small red patches. Within a few hours these developing small blisters appear in the centre of the patches. During the next 3-4 days further patches appear and the earlier ones will turn crusty and fall off. Calamine Lotion may be applied

to soothe the often severe itching. Cool baths may also help. The most infectious period is 2 to 3 days before the rash appears and up to 5 days after this date. Children may return to school 7 days after the outbreak of the most recent crop of spots.

WHAT TO DO IN A BEREAVEMENT If death occurs at home:-

1. Telephone the doctor. He/she will visit to confirm that death has taken place.

2. Contact the funeral director-

When death occurs in hospital: -

1. Contact the Funeral Directors to inform them that their services will be required.

2. Collect Doctor’s death Certificate from hospital.

3. Take this to the Registrar for the area in which the death took place. Also take deceased medical card if available and birth certificate.

4. Take green form to Funeral Director who will take over complete responsibility for arranging the funeral.
