SUBJECT: Mismatched Hammer Union

SCOPE: All Halliburton Locations

PURPOSE: To notify all Halliburton locations of the hazards of mismatching hammer unions

This Flash Report is an update to Report Number 09, issued August 13, 2001. The 2001 Flash Report reportedthe hazards associated with mismatching 2 inch 1502 wing nuts and 602 union crossovers. The information in thefollowing FMC Technologies Safety & Technical Alert discusses this mismatch hazard, as well as additionalmismatch hazards not covered in Report Number 09.
