Be Responsible

Handling equipment is easy to do as long as you are aware of your responsibilities. Use the following

list as a guide for proper equipment care:

DON’T:

• let anyone else use the equipment while it is in your care;

• throw, toss or in any way mishandle the loaned equipment;

• leave the equipment unattended at any time;

• videotape material that is does not meet school guidelines (I.e. no swearing, no illegal acts, etc.);

• neglect to return all equipment at the assigned time.

DO:

• check equipment out only with the designated instructor or peer mentor;

• complete the appropriate check-out forms thoroughly and accurately;

• Protect the loaned equipment by keeping it with you at all times;

• Charge all batteries and return equipment in the same condition in which it was loaned;

The Permission Form

• The Permission Form lets the instructor know that you and your family understand what your responsibilities are towards the care and protection of equipment you are loaned.

• It is important to always remember that YOU are ultimately responsible for the care of the equipment you borrow. For each item damaged or lost, numerous other students must do without.

The Permission Form

• Step One: Read the permission form from beginning to end. The top paragraph summarizes your responsibilities. The items requiring your initials indicates the rules you must follow when the equipment is in your care.

• Step Two: After reading the policy from beginning to end, ask questions. Do not sign off on anything you do not understand thoroughly.

• Step Three: Once you understand the full content of the form, then write your name clearly at the top “I John Smith promise to take the best possible care of any equipment that is checked out to me for use through my school. By signing my initials next to each of the following lists I am showing that I understand the rules regarding checkout and that I promise to take the following precautions every time a piece of media equipment is placed in my care.

• Step Four: Now re-read the specific line items and place your initials next to every item. If you cannot comply to each item, consult your instructor.

• Step Five: Review the entire document with a parent or guardian. They must agree to share in the responsibility of the equipment that has been loaned to you.

• Step Six: Return the completed form to your instructor.

The Overnight Check-out Form

When you are ready to check out equipment,

you will be asked to complete the Overnight

Check-Out Form

This form will list the specific items that

are being put in your charge.

• Read the form carefully.

• Make sure that all of the equipment you are planning to use is listed in detail on the form.

• If you agree to the terms, then sign the form.

Note: An instructor or designated peer mentor must co-sign the form ensuring that before you sign. Do not plan to borrow equipment unless you are ready to act responsibly.

The Overnight Check-out FormThe Overnight Checkout Form requires you to complete the following information:

• Your name and telephone #

• A specific list of the equipment you are requesting. Make sure the equipment identification (serial or code #s) is recorded accurately.

• The reason you are borrowing equipment (i.e. special project, assignment, etc.)?

• The specific dates you will have the equipment in your care (typically one or two days).

• Sign the form and date it.

• Present it to an instructor or designated peer mentor for review and co-signing.

Returning Equipment

Step One: Return the loaned equipment at the agreed to time.

Step Two: Return the equipment only to the instructor or designated peer mentor.

Leaving the equipment in a classroom or in the hands of any other student or staff

member is not acceptable and will be consider a violation of the check out agreement.

Step Three: Return equipment in the same condition you checked it out in. Make sure that batteries

are charged, that the tripod clip is in place on the camera and that you have removed your video

tapes and any other personal items.

Step Four: Report any loss, theft or damage to the equipment immediately. If theequipment did not function properly report that as well. The designated staff person will record theincident in writing and will notify you later if they require additional information from you.

The Delinquency Notice:

The Delinquency Notice is issued whenever a student is found to be in violation of the

checkout agreement.

The Delinquency Notice will inform you as to how you violated the agreement. Some

Examples that merit this action include:

• returning the equipment late

• forgetting to return part of the items originally borrowed to you (I.e. a battery, charger, etc)

• neglecting to report malfunctioning equipment

• neglecting to report damage to the equipment.

Once you receive three notices you will be banned from checking out equipment for the rest of the

school year.

If there is an emergency or for some reason you cannot attend school for the day, you can avoidgetting a delinquency notice by calling your instructor and requesting a short extension of the loan.

Note: If you receive three delinquency notices, you will lose your check-out privileges for the school year.
