PROGRAM DESCRIPTIONOver the past decade there are very few topics in the delivery of health care that have attracted more attention than the continuing dilemma of complex case management involving pain which spans all continents, socioeconomic standings and ethnicities. It permeates into each facet and vocation of healthcare delivery with the greatest burden placed on the shoulders of primary care providers. A new emphasis on patient-centered and holistic team-based care which has proven to increase patient outcomes, requires preparation and skill development among providers and especially among learners in the health care system.

Hosted by PCOM Georgia located in Suwanee, Georgia, the “Interprofessional Approach to Complex Case Management” conference will explore complex case management from the team’s perspective. This program will emphasize the importance of communication in delivering patient-centered care. A panel discussion will incorporate interprofessional collaborative strategies in the assessment and treatment of complex cases involving pain. Case-based learning will be followed by implementation of these strategies as interprofessional teams collaborate in a live clinical standardized patient scenario that will culminate with a panel summary discussion about the management of these complex patients, thus emphasizing the critical importance of using an interprofessional approach, interpersonal communication skills, systems based practice and dedication to life-long learning.

STATEMENT OF PURPOSE: The goal of this CME program is to demonstrate and emphasize the importance of interprofessional collaboration in delivering patient-centered care in the assessment and treatment of complex cases involving pain. Physicians and other healthcare professionals involved in the treatment of these complex cases will understand the roles various disciplines play and the importance of implementing interprofessional strategies in clinical practice which has proven to increase patient outcomes.

AUDIENCE: This activity is designed to meet the unique educational needs of physicians, physician assistants, APRNs, physical therapists, pharmacists and other healthcare professionals that are involved in the management and treatment of complex cases involving pain.

UPON COMPLETION OF THIS PROGRAM, PARTICIPANTS WILL BE ABLE TO:

• Explain the history and theory of the interprofessional team practice as it relates to complex cases involving pain.

• Describe the interprofessional approach to the care of complex patients that focuses on patient-centered care.

• Explain the importance of team-based care and collaboration for the safe and effective treatment of pain.

• Discuss the roles and expectations of each member of the healthcare team.

• Develop a comprehensive patient history and assessment and apply the interprofessional approach.

• Develop an integrated, multimodal, interprofessional and evidence-based approach to complex cases involving pain that is tailored to individual patient needs and discuss strategies for collaboration across the healthcare team.

• Discuss and review strategies for integrating the current knowledge of non-opioid pharmacological and non-pharmacological approaches to managing complex cases involving pain in a clinical setting.

• Demonstrate an understanding of one’s own scope of practice and the scope of practice of other healthcare professional involved in complex case management and determine appropriate referral of patients in a timely manner for comprehensive care.

• Communicate effectively with patients on short and long term goals of the care management.

• Utilize case scenarios to enhance communication and collaboration skills as it relates to managing complex cases.

• Assess interactions with other healthcare professionals for patient-centered care.

• Use clinical data and shared decision making process to integrate and apply current knowledge in a clinical practice setting.

CONTINUING MEDICAL EDUCATION CREDITPCOM Georgia is accredited by the American Osteopathic Association to provide continuing medical education for osteopathic physicians. PCOM Georgia designates this live activity for a maximum of 7.50 AOA Category 1-A CME credit hours and will report CME and specialty credits commensurate with the extent of the physician’s participation in this activity. The Georgia Board of Pharmacy course approval number will appear on the participant program certificate of attendance.

Physician Assistants: The Georgia Composite Board Chapter 360-5 document .07 continuing medical education requirements states that the board accepts the American Osteopathic Association Category 1-A credit as meeting its requirements for board approval. PCOM Georgia is accredited by the American Osteopathic Association to provide continuing medical education for physicians. Physician Assistants can earn up to 7.50 CE credits for participation in this activity and should claim only the credit commensurate with the extent of their participation in the activity.
