Register residents in this neighborhood who do not have family doctors from September to January. Develop a Consent Form within this clinical rotation before December 1, 2015. Develop a Patient Health Action Plan within this clinical rotation before December 1, 2015. Initiate a focus group within the community to receive feedback on the Patient Health Action Plan and Consent Form by December 1, 2015. Involve literacy support workers to aid in audience comprehension of the forms: Improve and/or maintain functioning, resilience and health for individuals including: Wellness Longevity It is hoped prior to July 2016 the North End population will have increased awareness about the local health center: increased accessibility & availability &services offered. It is expected that public/patients will understand how the Wellness Center integrates all health care disciplines within 1 year of opening: January 2017. Modify the health literacy on the consent form and patient health action plan so this population can understand how to appropriately self- manage prior to January 2016. To offer a range of primary health care services based on health needs will reduce 30% episodic health conditions. This new centre will enhance timely & appropriate access to health center services thus the public will positively report receiving timely health care services within 1 year of opening this centre. In two year, emergency admissions from this neighborhood will reduce by 30%. Aiming towards an ISD approach for the next 1-2 years. The centre will enhance community capacity for health promotion and improve the health of North End residents by 5 years. Usage of the Patient Health Action Plan will lead to patients independently being able to understand and develop goals for their health in the

Assumptions External Factors After educating and raising awareness the population will agree on the need for a Wellness

Center in the neighborhood and utilize the center. Sharing knowledge about the center will to lead to reduced risk, duration, and effects of acute

and episodic health conditions. Wellness Center is at a convenient location/distance to the individuals living in the North End

thus reducing transportation costs.
