Valley Baptist Medical Center-Harlingen and Valley Baptist-Brownsville have received state-wide awards for reducing the hospital re-admission rates for patients with various conditions, such as heart failure, end-stage renal disease, chronic obstructive pulmonary disease (COPD), heart attack and pneumonia.
The awards from the TMF Health Quality Institute recognize Valley Baptist’s achievements in reducing re-admissions, as well as participation in efforts to provide education and to promote follow-up care for these patients after they are discharged from the hospital – so that their condition can be managed in the broader community, without coming back into the hospital.
The Harlingen area was the only community in Texas -- and one of only 14 in the nation -- selected to participate in the U.S. Centers for Medicare & Medicaid Services (CMS) “Care Transitions Project”, which seeks to reduce and eliminate unnecessary re-admissions to hospitals. The effort in the Valley involves fi ve hospitals from Harlingen, Weslaco, and Brownsville, which are working to transition patients back into the community, by working with physicians, home health agencies, outpatient clinics, skilled nursing facilities, rehabilitation centers, and other health care providers.
“The goal is to keep these patients stabilized in the community, rather than having them come back to the Emergency Room or be re-admitted to the hospital,” said Pam Warner, RN, BSHA, CPHQ, CPMSM, Administrative Director for Medical Staff Services and Quality Improvement for Valley Baptist. Ms. Warner previously oversaw quality initiatives which resulted in Valley Baptist receiving several national awards for treating heart failure patients.
