Item 1, 2 and 4 will also be reported in accordance with the instructions of the. subject bulletin. Attachment A identifies those bolted closures of the Reactor Coolant Pressure Boundary (RCPB) tiwt have experienced leakage with the resultant inspections made and corrective measures taken to eliminate the leakage. This attachment responds to Item 3.a of the subject bulletin with the exception of DH-V1. Attachment B identifies the RCPB closures and connections where fastener lubricants have been or are being used and the lubricant's composition. Our experience with these lubricants has been satisfactory. This attach- ment responds to part of item 3.b of the subject bulletin. Attachment C describes the only RCPB closure where injection sealant material has been used (DH-V1) and the sealant's composition. ATTACHMENT C

DH-V1 letdown (first isolation valve from the hot leg of the Reactor, Coolant"B". Loop) has experienced pressure seal ring leakage. DH-V1 is a 12 inch,type 316 stainless steel pressure-seated gate valve manufactured by theWalworth Company. Following stem replacement during Spring 1976 Refueling :Outage, DH-V1 developed a leak (0.2-0.5 gpm) past the pressure seal ring

gasket as evidenced:by-leakage up through the threads between the retainingring and the valve. body. _During' November 1976 nuclear grade Furmanitewas injected and successfully stopped the leakage. The Furmanite compound hasa Nitrile rubber base. Testing of the nuclear grade _ compound was performed andthe following levels were-determined:
