START WITH THE AUTHOR’S NAME
Name formatting is: Last, First Middle.
Guide to Formatting Authors’ Names in Citations – What to do if there is more than one author, one or more editors, a corporate author, or other challenges.
Middle names and middle initials follow the first name.
Don’t turn middle names into initials or skip the middle name.

AND SUBTITLE
Use a colon after the title, if there is a subtitle.

The X on your bibliography worksheet is just a place holder. It shows that you looked for the edition and there either wasn’t one listed, or it was the 1st edition.
When you write up your citation, you will skip over the X if you have one, but include any other edition information. Ex: 6th ed.
