Reference is invited to Request for Proposal (RFP) issued by Employees State Insurance Corporation (ESIC) on 27-12-2018, Corrigenda dated 07-01-2019, 21-01-2019 and Addendum dated 09-01-2019 for selection of Service Provider for Operation and Maintenance of Project Panchdeep at ESIC. The RFP stands further modified as under:

2.5.1 Resource Requirement for Application Change Requests

The bidder will separately provide 10 software resources of required profiles as mentioned in RFP (Annexure - 07) at ESIC premises to cater to change requirement/modifications. ESIC will provide their seating arrangement and connectivity to Data Centre Server.

2.4 Technology Improvements In addition to the O&M activities, it is expected that Service Provider will carry out the following technology improvements activities. For the items mentioned under para 2.4 specific work order will be issued as and

2.5.1 Resource Requirement for Technology Improvement and Application Change Requests

The bidder will be required to provide software resources of required profiles as mentioned in RFP (Annexure - 07), as per ESIC request, at ESIC premises to cater to Technology Improvement and/or Application Change Requests. ESIC will provide their seating arrangement and connectivity to Data Centre Server. ESIC may decide subsequently whether to seek services of manpower for Technology Improvement and/ or Application Change Request or not. ESIC will also decide on number of resources required. The manpower may by sought in manpower mode in which case payment will become due based on number of man months for which services are hired or in Project Mode. In case manpower is hired in Project Mode, Level of Manpower & Man month will be decided on mutual agreement basis & Financial Implications would be worked out based on Manpower Rate quoted & a Value will be assigned to a 'Change'. Amount of the 'Change' would be released on Completion of the 'Change' & successful implementation.

2.4 Technology Improvements In addition to the O&M activities, it is expected that Service Provider will provide manpower, on such terms, as detailed at para 2.5.1, for the following technology improvements activities. 1. The bidder must provide the Financial Proposal in e-tendering mode. 2. The price would be exclusive of all taxes, duties, charges, cess and levies as applicable

from time to time. Bidder must provide list of applicable taxes (to be mentioned in %). 3. The bid amount shall be inclusive of packing, forwarding, transportation, insurance,

delivery, installation charges and any other charges as applicable. 4. Prices shall be quoted entirely in Indian Rupees. 5. The above table is comprehensive cost for bid evaluation purpose. Actual payment terms

and conditions are as referred in Clause 55 Payment Schedules. 6. All the prices quoted, including the man-power costs, in the Financial Proposal shall be

valid for the entire period of the contract. 7. Correction of Errors: 8. The Bidder is expected to price all the items and services required for successful

implementation of the project and subsequent operations & maintenance in line with SLA.

9. All costs incurred due to delay of any sort, shall be borne by the Bidder. 10. The bidder is expected to provide all the assumptions related to the proposal in the

technical proposal itself and not in the Financial Proposal. A financial bid with any assumptions or conditions will be disqualified.

11. No adjustment of the price quoted in the Financial Proposal shall be made on account of any variations in costs of labour and materials, currency exchange fluctuations with international currency or any other cost component affecting the total cost in fulfilling the obligations under the contract.

12. A proposal submitted with an adjustable price quotation or conditional proposal may be rejected as non-responsive.

13. The sum total of price quoted in Table D will be used for bid evaluation purposes.
