Setup
FormattingTo add you organisations logo to the file right click on the form, and select “Design View”
Then under “Design” on the top ribbon select “Insert Image”, find the relevant image and position it on the page
TablesIn order to fully utilise the CPD Database you will need to ensure that the relevant tables are all set up correctly, there are a number of ways to do this. You may need to save and close the database for some changes to take effect.
CPD LogThis table will be feed by the CPD form and will require minimal intervention; you may need to look at it from time to time in order to check data quality.
If you run group sessions, with group feedback it may also be useful to add these records through the CPD Log Table. If the first person fills out the CPD form as normal then goes into the CPD Log, they need to select the relevant record, right click and ‘Copy’.
Then go to the bottom of the file, Highlight the Row with (New) in the first box then paste in the record. You will need to delete the original name and add in the relevant person. This saves multiple people inputting the same information a number of times. Individuals can then find the relevant

Financial YearOur CPD log is monitored over a financial year, but you can chose to change this. To amend or add a financial year simply open the table and either type into the bottom line to add a new period or overtime the existing period to make changes.
Personal DetailsTo add a new person simply open the Personal Details form, complete and save. Just like the CPD form. You may need to close for database and reopen in order to find the person in the CPD form.
This method can be used for the initial set up or if you already have the information available then you can import multiple lines at once. Ensure that the data is in the same format in an Excel spread sheet, right click the top left hand corner of the Table and ‘Paste’.
Prof Status, Provider, Qualification, TypeThese fields are pre-set however you can add or amend this list in the same way as the “Financial Year”, simplify type into the blank line or over type one of the existing.

You must fill in each of the 4 Finance Skills and Strengthens boxes in order to complete the form (even if this is just n/a)
Use the drop down options where applicable as this will feed the CPD Dashboard. If you need at add a new option then follow the steps above and add this to the source table (you will have to save and close the database for changes to take effect).
Press the Save button when you have finished entering your details
Adding EvidenceSave a folder in a Shared Drive under CPD with your name
In the Evidence box on the CPD form right click and select Hyperlink, then Edit Hyperlink
You can then include a link to the relevant file.

Personal Details Open the “Navigation Pane” with the Arrow buttons, and select the “Personal Details” form:
Search and create new records as per the notes above
Once you have created or updated a record save and close the form. You may need to close the database to update the “CPD Form”
Amending ListsOpen the “Navigation Pane” and select the relevant table
Overtype an existing entry or add a new entry at the bottom of the column

DashboardThe attached document has notes on how to link a data table in Access to an Excel spread sheet. This can be used to create pivot tables and dashboards to report on CPD. An example of a CPD Dashboard is included below.
SuggestionsWhen building the dashboard it may be beneficial to include criteria to ensure the 4 strengths have been considered. For example, only bringing back records when all 4 boxes have been completed, even if this is just N/A. To do this, during the linked table process select the relevant field and apply criteria, as per below. You will need to do this for each field, and they will be highlighted in bold when the criteria are applied.
When creating the dashboard link the date into a pivot table and grate the relevant graphs/reports from this. The pivot table can be copied and paste without having to be re-linked each time.
