Welcome to the Comprehensive Care for Joint Replacement Model, also referred to as CJR. Instructions for Accessing the CJR Portal. This slide deck walks through the steps to gain access to the CJR Portal. This is the application through which participating hospital users will access CJR data files, including claims data, target prices, and reconciliation results.

To request access to the CJR Portal you will need: 1) A CMS ID (Enterprise Identity Management ID or EIDM

ID) 2) Approved Access to the Innovation Center (IC)

Application 3) Your hospital’s Medicare number or CMS Certification

Number (CCN)

Step 1: Requesting Access to the CJR Portal

To request access to the CJR Portal you will need: 1. A CMS ID, otherwise known as the Enterprise Identity Management ID or EIDM ID 2. Approved Access to the Innovation Center (IC) Application 3. Your Hospital’s Medicare Number/CMS Certification Number (CCN) If you do not already have an EIDM ID or Access to the Innovation Center (IC) Application, please refer to the instructions in the slide decks titled “Requesting a New CMS Enterprise Portal Account” or “Instructions for Requesting Access to the IC Web Application”.

Go to https://portal.cms.gov through a web browser. Fully supported web browsers are: Microsoft Internet Explorer version 9.0 or higher (for Windows) Mozilla Firefox (latest release) (for Windows and Mac) Click the “Login to CMS Secure Portal” button on the right-hand side to log into the CMS Portal.

Review the Terms and Conditions. After you have reviewed, click the “I accept” button at the bottom of the screen to accept.

In the Welcome to CMS Enterprise Portal screen, enter your CMS User ID and password. Click the “Login” button.

In the Welcome to CMS Enterprise Portal screen, the Innovation Center dropdown menu displays in the top left-hand corner. Select “Application Console” in the dropdown menu.

The CMMI Application Selector portlet displays a welcome notation for the user. In the CMMI Request Access section, click the “Request New Access” button.

In the CMMI Request Access section, select “Comprehensive Care for Joint Replacement” from the Application Name dropdown menu.

Types of Roles to Request

If you are a CJR Participating Hospital User, select one of the following: • CJR Primary Point of Contact (PPOC) - if you are

one of the two hospital representatives that filled out and signed the Comprehensive Care for Joint Replacement (CJR) Model Data Request Form that was included in your hospital’s orientation materials.

• CJR Standard User - if you did not fill out that form

and you are requesting data for a CJR participant hospital.

CJR Participating Hospital Users

If you are a CJR Participating Hospital User, select one of the following: CJR Primary Point of Contact (PPOC) - if you are one of the two hospital representatives or Data Points of Contact that were listed on the Comprehensive Care for Joint Replacement (CJR) Model Data Request & Attestation Form that was included in your hospital’s orientation materials, or CJR Standard User - if you did not fill out that form and you are requesting data for a CJR participant hospital.

In the CMMI Request Access section, select your role in the User Role drop down menu. For this example, select “CJR Standard User”.

In the CMMI Request Access section, the CCN field displays after you select a user role. Type the CCN (or Medicare Number) of the participating hospital for which you are requesting CJR Portal access. Click in the blank area of the screen. The associated participating hospital’s name and state location display.

In the Comments field of the CMMI Request Access section, type “Requesting access to the CJR Application”. Click the “Submit Request” button.

A notification of your successful submittal displays, along with a request ID. Your request was e-mailed to the IC Center for processing. Save the request ID for your reference. Click the “OK” button. You will return to the CMMI Request Access portlet, where a record of your request displays. You will receive an email notification once your request is processed.

to access the CJR Application. 2) Your CMS ID or EIDM ID 3) Multi-Factor Authentication (MFA) Security Code

Step 2: Accessing the CJR Portal

To access the CJR Portal you will need: 1) An email notification confirming approval of your request to access the CJR Application 2) Your CMS ID or EIDM ID 3) A Multi-Factor Authentication (MFA) Security Code. Multi-factor authentication requires an additional piece of data to verify your identity. That additional piece of data will be a security code that is obtained through a downloadable software program, or sent to you via SMS text message, voice response, or e-mail. Repeat this process each time you log in and access your data. Refer to the slide deck titled “Instructions for Requesting Access to the IC Web Application” for more information about MFA.

The Enter Security Page displays in the CMS Portal. Obtain a security code from your Multi-factor Authentication configured device. Enter that security code in the Security Code field and click the “Log In” button.

The CJR Application displays and defaults to the Download tab. You have successfully launched the Comprehensive Care for Joint Replacement application. Select your participating hospital’s name and CCN in the Select Your Organization dropdown menu. The hospital’s available data displays in a table in the lower portion of the screen.
