The Form is a Department of Defense form and is not easy to follow?

The school is responsible for completing only page 3 of the DD FORM 2792-1. The form is currently under revision and a more user friendly version is set for release in Nov 2009.

The child is home schooled or between the ages of 3-5. “Not enrolled in the school”

The Individuals with Disability Education Act (IDEA) entitles that these children are eligible for special education services through their local school. If the child has never been evaluated , is not currently receiving services, and/or has no identified needs that the school has been made aware of…there is no liability attached. The form can be completed by the school.

If the child does not receive services and has no special needs identified, the school can elect to leave the form blank and simply write a letter (on official letterhead) stating that : “named child has no identified special needs and is not receiving services at this time”. Air Force preference would be that the school official completes the DD FORM 2792-1 and marks no to all responses on item 3, skips to item 6, and returns form to parent.
