Dear Friends,

In 1946, a small group of engaged community members gathered to create an organization that would provide skilled nursing in the home with a mission to promote health and independence. From these humble beginnings to the strong, vibrant agency of today, the Visiting Nurse Association & Hospice of the Southwest Region continues to remain focused on our mission of providing compassionate, nationally recognized home health care, hospice care, and community wellness to thousands of individuals throughout Rutland and Bennington Counties.

Last year was a time of reflection and big dreams. It was a time to look back and honor our past. It was a time to celebrate being named to the Top 100 of the 2016 HomeCare Elite, a compilation of the top-performing home health agencies in the United States and the only performance recognition of its kind in the home health profession.

Above all, it was a time to look ahead to the exciting possibilities for home health care and hospice care.As the American health care system evolves to reward cost containment, outcomes, and efficiencies, all while managing a dramatic increase in the older adult population–we stand ready, as we have for more than 70 years, to redefine how health care is delivered at home.

We are energized by the opportunities to take a leading role in providing high-quality, cost-effective care. We continue to move forward with initiatives designed to keep patients healthy and independent at home, reducing avoidable hospitalizations and improving their quality of life. We continue to design new models of care that provide patients more seamless transitions along the continuum of care and help address patients struggling with chronic health conditions or behavioral issues.

We are proud of our 70 years of mission-focused care and remain fiercely loyal to that mission and the communities we serve. With a renewed spirit of innovation, we look ahead to the future and are grateful for the many that join us in our journey. Together with our staff, Board of Directors, friends, donors and volunteers, we can achieve great things, we can seize the opportunities ahead and position the Visiting Nurse Association & Hospice of the Southwest Region as a leader and innovator in home health and hospice care.

LOOKING AHEAD TO THE FUTURECelebrating the Past...

THANK YOU FOR JOINING US ON OUR JOURNEY!
