condition• Qualifying exigency

• Employee’s spouse• Parent• Child under age 18

unless unable to care for self due to mental or physical disability at time of leave

• Employee’s spouse• Domestic Partner• Parent• Parent-in-law• Child at any age• Grandparent• Grandchild• Sibling

Eligibility:Employed & “disabled”

Medical Certificate Required

Job Protected?Requires modifications to existing leave policies and providing leave when needed for a disability, even if the employee has exhausted leave provided by employer (including workers' compensation, CFRA, or FMLA)

Employee notifies they are ill or needs time off (pregnancy, ill family member,

other).Location Admin provides Employee

with Leave Request form.Employee provides request form (and

medical certificate, if applicable) to Location Admin.

Location Admin will place employee on “leave” in Paylocity and send

completed Request form to Matt Habana.

Matt will track FMLA/CFRA leave & will communicate with Location Admin any

issues. He will also inform Location of 12

weeks of benefits ending.

Matt Habana processes the legally required leave letters and notices –

sends to employee and copies Location Admin in the email.

Location Admin is responsible for paying employee sick/vacation &

coordinating pay with SDI/PFL

Location Admin will coordinate with employee benefit payments (if

applicable). They will also keep in contact with employee.

Before Employee returns, they will need to submit a release from doctor (if out for medical reasons). Location Admin

will provide release to Matt Habana.If accommodations are required, Matt

will work with Location Admin.

If Employee requires more time off beyond 12 weeks of FMLA/CFRA, Matt

will guide Location on next steps.

If Employee returns to work, Location Admin will notify Matt & save all leave documents in employee’s medical file.

Matt will close employee’s file.Location Admin will place employee on

“active” in Paylocity.
