ADAPTABILITY
ADAPTABILITY: EXCEEDS EXPECTATIONS
“ Though many people find change difficult to handle, I understand how to handle it
with ease. We have a very fast paced environment and I understand how to navigate
the changes and provide high quality work, despite not knowing what may be
coming at me next. Over the next year, I will continue to remain nimble and adapt to
changing circumstances.
“ The past year has seen a number of changes happen in our department. I have
handled the changes well and supported other employees who were troubled by
them. This has not only demonstrated my ability to cope with change, but resulted
in improved team dynamics. Over the next year, I will continue to handle changes
that may arise and provide support to team members.
“ I see change as an opportunity. I encourage others to take initiative when new
projects or assignments come available and quickly adapt when times require it. I
believe I am a very flexible employee.
“ In an environment that is constantly changing, being able to rapidly assess a
situation and adapt is crucial. I am one of the most proficient employees at
maintaining composure and change as it is demanded.
“ I consistently show composure when put into a stressful situation. I face challenges
head on and with a motivated spirit. I try to calm those working around me by
keeping them focused on a specific goal. I believe I have been flexible and a
productive member of the team.
Simple Evaluation Page 13
ADAPTABILITY: MEETS EXPECTATIONS
“ I frequently adjust my work schedule to meet the needs of the team. I am very
willing to make the necessary changes to help the team succeed. I believe I am a
versatile employee who does what is necessary to keep business moving forward
and I will continue this over the next year.
“ I have been willing to work odd hours when a project requires it, demonstrating I
can adapt to changing work schedules. I face challenges with composure, identifying
solutions and efficient alternatives instead of cracking under the pressure.
“ I have greatly improved in adaptability this year. While I once shied away from
change with discomfort, I’ve learned to embrace it with renewed energy. I adapt to
changing schedules and assignments, and even help to calm those around me in
times of stress.
“ I have satisfactory standards of adaptability, and have proven that I can adjust to
sudden changes within the work place. I handle changes of schedule and office
protocol with ease, and have made suggestions on how to improve them further.
“ I consistently maintain composure during times of stress, and show that I can adapt
to new challenges that arise during an assignment. Sometimes, I know I struggle
with some tasks that are not part of my job. I could improve by adapting to the
addition of new responsibilities.
ADAPTABILITY: BELOW EXPECTATIONS
“ Occasionally, I become frustrated when faced with sudden, unexpected changes. At
these times, I know I lose focus of the end goal. My attitude toward unforeseen
challenges could use improvement.
Simple Evaluation Page 14
“ I have a tendency to micromanage assignments and allow for little deviation in plan
or schedule. I understand not everyone appreciates this style, and I will work to
improve in this area. I will work on improving my acceptance of change.
“ I do well adapting to small changes at work, but struggle when we have larger issues
that affect the whole office or department. Some of these changes are just too large
to adapt to quickly. Over the next year, I will work on accepting all types of change.
“ There are times when I struggle with new work assignments and training. I
understand how important these are to the business, but I need some latitude to
achieve these in my own timeframe. I will continue to work on improving in this
area.
“ We are in a period of change and I have struggled to keep up. These fluctuations in
the environment have made it difficult to keep track of the changes in the
environment. I will work on documenting processes and keeping up with the
changes in the business.

ATTENDANCE AND PUNCTUALITY
ATTENDANCE AND PUNCTUALITY: EXCEEDS EXPECTATIONS
“ I believe this year I have demonstrated that I care greatly about attendance and
punctuality. I get to work early to be prepared to start a productive and efficient
work day. I’ve had perfect attendance this past year.
“ Not only does Jason exemplify our standards of attendance and punctuality, but he
takes initiative to improve the attendance and punctuality of those around him. He
creatively introduced new incentives to keep his subordinates motivated to be at
work, and their work is consistently efficient because of it.
“ I am quite reliable in terms of punctuality and attendance. I show up on time every
day, and do not leave early without prior approval and meeting his obligations.
Additionally, I started a carpool to improve the punctuality of others.
“ I start each day ready for the challenges ahead. I am dependable and maintain a
great schedule throughout the year. I will continue to try to excel in attendance and
punctuality.
“ I have a nearly perfect record of attendance and am on-time, ready to work, each
day. I always seek prior approval before taking a day off, and arrange for back up
support during any absences. I complete my assignments on-time and my meetings
end on-time.
Simple Evaluation Page 16
ATTENDANCE AND PUNCTUALITY: MEETS EXPECTATIONS
“ I have very good attendance. I start and end meetings on time which helps keep my
team productive. I continue to work on improving my time management skills to
continue remaining on-time.
“ I complete my assignments accurately and on time and am prepared for meetings. I
rarely have a sick day and have improved my attendance this year. I meet all
company standards for attendance and punctuality.
“ A number of times I’ve had to work late to complete a project on-time. I do so
without complaint and want all of our projects to be successful. I believe I meet all
expectations for attendance and punctuality, showing up on time and eager to work.
“ I schedule vacations well in advance, ensuring that necessary preparations are made
for my absence. I show up on time and only leave early when my work is completed,
and I have received the necessary permission from my manager. I support my peers
when they are struggling with time management, and am willing to take on extra
work for the benefit of the team.
“ I have satisfactory attendance. I believe I have been on-time to work and work my
full schedule most work days. There have been, on occasion, days where I have been
late to work, but I stay late to make up the time. I will continue to work to improve
my attendance.
ATTENDANCE AND PUNCTUALITY: BELOW EXPECTATIONS
“ For most of the year, I have no attendance problems. During inclement weather,
traffic can cause me to be late on occasion. I try to adjust the time I leave to make
up for this, but on occasion I am still late. I will continue to work on improving my
punctuality.
Simple Evaluation Page 17
“ At times, I have a bad habit of arriving late to work. I understand how this affects
my team and am working on a plan to improve my attendance. I will show greater
effort to arrive on time and begin work along with everyone else.
“ Though my attendance is good, I know some of my breaks have run a little long this
year. I will continue to work on keeping my breaks to their allotted time and improve
in attendance and punctuality.
“ I consistently arrive to work on time, and schedules her absences according to
regulations. However, I understand the perception that I spend a lot of time on
personal matters at work. I will work on improving this and ensuring that my work
time is devoted to business matters.
“ My attendance has suffered at times due to unforeseen personal circumstances. I
believe that I am a reliable team member. I do understand the negative effect on my
colleagues and will work to improve in this area.

Chapter 4
ATTITUDE
ATTITUDE: EXCEEDS EXPECTATIONS
“ I think I display a uniquely positive attitude toward my work, always showing
enthusiasm and good humor when taking on a project. I accept criticism well,
always using it to make improvements, and remain actively engaged with my
colleagues.
“ One of my greatest strengths is showing an exemplary attitude at all times. I find it
easy to be energetic and ready to start work in the morning. I have strong
motivation in my responsibilities throughout the day. I work to keep a positive
attitude and help motivate others to do the same.
“ Offering encouragement and support is one of the things that sets me apart. I
believe this provides an excellent example for the ideal work attitude. This helps
keep my team working quickly and efficiently. I like to keep people focused on a
goal, and never shy away from a challenge.
“ I consistently portray an attitude communicating respect and patience. I treat
subordinates in a way that encourages growth and improvement and am always
willing to lend an ear when a team member faces a difficult challenge. I believe I use
humor effectively on my team to relieve tension.
“ I demonstrate a very good attitude toward work, always excited to take on a new
assignment or additional responsibilities. I remain actively engaged in my projects
throughout the entire work day, and it shows in the high quality of my final
products.
Simple Evaluation Page 19
ATTITUDE: MEETS EXPECTATIONS
“ I like to remain fully engaged in his work until completion. There is often much
office drama and I try to avoid it and not let personal issues affect the quality of his
work. I believe this is a strong way to provide an example to my.
“ I show a unique attitude toward work, always looking for something new to learn
about the industry. I ask insightful and well-formulated questions. I believe
“ I am willing to go the extra mile to put others at ease, fostering a comfortable and
open work atmosphere. My attitude toward positive change is one of enthusiasm,
and I consistently puts forth an effort to improve myself and my surroundings.
“ I have an encouraging spirit and a determined attitude to help to motivate those
around me, and frequently support team members in need. I believe I have an
attitude committed to progress and make frequent suggestions to improve
processes and the way we work.
“ I enjoy going the extra mile to complete an assignment, and also work hard to make
sure others feel at ease. I try to always remain a positive thinker and help to
motivate those around me during times of stress or difficulty. I will continue to work
to improve in this area over the next year.
ATTITUDE: BELOW EXPECTATIONS
“ There are times where people have an impression that I am uninterested and
unmotivated attitude toward my assignments. I have worked to improve this
perception, but have not been able to fully disperse it. This next year, I am going to
work to improve my attitude and my coworker’s perceptions of my attitude.
“ I attempt to maintain a positive attitude, but am consistently very critical of myself.
I have a tendency to take constructive criticism personally, and instead of using it as
Simple Evaluation Page 20
a method to improve, I let my feelings of fear and rejection stop me from
progressing. This is an area of focus for the next year.
“ I have a tendency to get so focused on a task that I ignore the people around me.
This has resulted in the impression that I don’t care, when in fact, it’s just extreme
focus on the work at hand. I will work to improve my communication skills and
dealing with people even when I am in the middle of a task.
“ I understand I have a very competitive attitude. While this can be good for certain
aspects of business, it creates issues within my team because people feel that I am
always competing with them. I do not want people to believe I have an air of
superiority and will work this year to tone down my competitiveness.
“ There are times where work can get very emotional and this has been off-putting to
some of my coworkers. They perceive me as being angry and frustrated, and
perhaps at time I am, but I know this is poor emotion control and I will work to
improve it.

Chapter 5
COMMUNICATION
COMMUNICATION: EXCEEDS EXPECTATIONS
“ My goal is to cultivate an office environment of openness. I have set up a system of
information sharing to ensure that everyone on the team is kept up-to-date on
industry advancements and relevant changes to office policy. Additionally, I do it all
in a friendly and non-aggressive manner.
“ I regularly request constructive feedback when turning in an assignment. I ask
questions when faced with doubt, and communicates problems in order to solve
them more efficiently. My communication with clients is always exceptional, as
well, and I am personally requested on many projects.
“ In terms of communication, I believe I stand out among my peers. I excel as a public
speaker and a company spokesperson, conducting myself with dignity and openness
in every business venture. I am often sought after for feedback on a project.
“ I work to create a welcoming and inclusive work environment, always making others
feel welcome to visit my office for a discussion. I remain composed under stress, and
have a calming influence on those around me. I foster open information sharing and
enhances the productivity of my team.
“ I am in constant communication with my team, holding weekly staff meetings to
make sure everyone is on the same page. I provide timely and constructive
feedback, and provide clear explanations when asked to clarify a difficult issue.
Simple Evaluation Page 22
COMMUNICATION: MEETS EXPECTATIONS
“ I believe I am a satisfactory communicator, always explaining ideas in a clear and
precise manner. I demonstrate an evolved sense of humor and candor during
business dealings, always keeping open lines of communication with clients and
colleagues. Additionally, my feedback on projects is always very productive, and
introduced in a non-intimidating way.
“ I regularly keep open lines of communication. I hold frequent meetings with my
team to keep them updated of changes in the field. I often provide feedback to my
subordinates, and requests feedback from my superiors.
“ While I have struggled with open communication in the past, I have greatly
improved in that department over the course of the year. Where I once was afraid
to request clarification, I now welcome the opportunity to ask well-prepared
questions. I will continue to make progress in this area.
“ I believe I communicate well. I am always making others feel welcome and act as a
calming influence when an upsetting situation arises. I create an inclusive work
environment, keeping my team comfortable and productive.
“ I have proven to be an effective communicator, keeping my team focused and up-to-
date on advancements in the field. I allow subordinates to express their successes,
and demonstrate a real willingness to hear all available information before making a
final decision.
COMMUNICATION: BELOW EXPECTATIONS
“ I find that I am an inconsistent communicator. At times, I believe I am insightful,
while at other times I struggle to explain concepts. I need to work more on being
consistently prepared in order to better communicate my ideas. I will work to
improve my preparation in the future.
Simple Evaluation Page 23
“ At times, I prefer to handle assignments on my own rather than communicate with
other team members. I believe this is the best path to getting things accomplished
right, but I understand how this can be viewed as isolation and not communicating
with others well. I will work on improving my teamwork.
“ I need to improve my communication by fostering open lines with my coworkers and
be more welcoming with my interactions. I have a strong personality which has a
tendency to alienate some around me and I will work to improve this.
“ I am a quiet person which comes across as shyness or possibly aloofness. I know I
need to find my voice and find the confidence to express my ideas. This is a difficult
proposition, but one which I am committed to improving.
“ I find it difficult at times to ask for help and when a task is late, this is evident as I am
often overwhelmed with work. I have been working to engage with my peers for
help, but still find it challenging to communicate my need for assistance.

Chapter 6
COMPANY COMPLIANCE
COMPANY COMPLIANCE: EXCEEDS EXPECTATIONS
“ I always try to adhere perfectly to our processes and procedures. I consistently keep
my staff up to date with company policy and ensure it is followed to the letter. This
has been an area of focus for me. I will continue to excel with company compliance.
“ I believe I have an excellent understanding of what the company expects and the
policies and procedures that must be followed. This is one of my strengths – I pause
long enough to consider how decisions would affect the company and then act
accordingly.
“ I am proud of my team’s efforts to uphold company policies and remain compliant
with all company directives. I have worked hard this year to ensure we have
properly trained and followed all company rules.
“ I believe I have an excellent understanding of how to comply with company
procedures in my current position. It is important to remain productive and efficient
by following all company regulations. I believe I am a model employee in this regard.
“ Managing and training others on department procedures is a joy of mine. It is
something I truly relish and believe sets us apart from our competitors. This is an
area I excel in and will continue to excel in this year.
Simple Evaluation Page 25
COMPANY COMPLIANCE: MEETS EXPECTATIONS
“ I understand the company processes and regulations and work hard to meet them. I
support the company’s vision and mission and work hard to achieve it. I can improve
in communicating changes in company direction this next year.
“ I work within the parameters of the company procedures to the best of my ability.
Sometimes my work requires minor adjustments to the stated policy, but I always
keep the company in mind when acting.
“ I understand and follow all rules and regulations set by our company. I am regularly
relied upon to instruct other employees in regards to these rules and how they
should act under them. I will continue to keep up to date in this area.
“ We are in a well regulated industry and I make sure that my team and I follow all of
the rules, regulations, and company policies to the letter. This protects our firm and
my team from legal exposure and also ensures we are being ethical in all we do.
“ I work well within the framework setup for me by the company and my manager. I
understand how to comply with everything the company has asked of me and I
believe I have done a good job with this.
COMPANY COMPLIANCE: BELOW EXPECTATIONS
“ There are a number of times where I know I have violated a procedure or company
policy in order to get the work done. I understand the effect this can have on my
team and will resolve to make sure all activities follow proper procedure.
“ I work well within the parameters set by my manager, but I understand there are
times when I am expected to think and act independently and I struggle to achieve
this. I will continue to work on this initiative and make sure I follow all company
policies and procedures.
Simple Evaluation Page 26
“ There have been some of the policies where I have struggled to adopt and practice
them. Many of these have seemed outdated, but I understand the company wants
me to follow them to the letter despite the deficiencies. I am working on
documenting these deficiencies so these policies can be revised and improved.
“ I understand there have been times where I have short cut the company policies to
save time and I understand this is not acceptable practice. In the future, I will follow
these policies to the letter.
“ There has been an impression that I am unreliable when it comes to following
procedures and policies. Though I believe I have done well in this regard, I
understand how this impression could have developed and will work hard this next
year to improve in this area.

Chapter 7
CONTINUOUS IMPROVEMENT
CONTINUOUS IMPROVEMENT: EXCEEDS EXPECTATIONS
“ Change is inevitable and I excel at using change as an opportunity. When we do have
changing conditions, I understand how to make the best of the situation and
improve myself. I have also learned how to affect positive change to the work
environment during these times. I will continue working hard in the area of
continuous improvement.
“ Continuous improvement is not just a business catchphrase, but something that we
should all be seeking. I believe this is an area of strength: identifying opportunities
where I can improve and see great returns. I will continue to improve in my job and
improve how I work with others.
“ I have made a number of breakthroughs in performance this year through
continually improving my productivity. I am constantly evaluating and improving the
way I work to continue making both incremental and breakthrough improvements. I
will continue to work on this progress.
“ I highly value feedback received which allows me to constantly improve myself. I find
implementing new ways of doing business both easy and effective. I am exciting
about the potential improvements I will make this next year.
“ To survive in business one must evolve and I believe I am very good at identifying
inefficiencies in my own work, the work of my team, and other coworkers, which if
improved, increase productivity and make us better as a company. I will continue
evolving and improving in my position.
Simple Evaluation Page 28
CONTINUOUS IMPROVEMENT: MEETS EXPECTATIONS
“ I have made a number of small, incremental improvements to my work and to
processes my department uses. These have result in increased productivity and
efficiency. I will continue to work to find ways to improve how we do our jobs.
“ I don’t believe I have a great talent in continuous improvement, instead, it just
comes naturally to me to identify potential improvements in my work processes. I
receive input from my team on potential improvements and then we implement and
test them. The best ones are retained and we restart the process. Overall, I think
continuous improvement is a good area of focus for continued investment.
“ I am good at identifying and eliminating suboptimal processes in our work
environment. I understand how to document a process, identify inefficiencies, and
improve it. This is an area which I will continue to improve in this next year.
“ Part of continuous improvement is identifying process improvement ideas, another
is identifying problems and breaking them down to their root cause. I am not very
good at the former, but very good at the latter. I can see a problem, identify the
symptoms and what is actually the root problem. I then identify a solution and
implement it. I will work to improve in process improvement as well.
“ I have attended kaizen training and implemented much of it in my day to day work.
There are a number of parts of kaizen that I initially struggled with in my job, but
since then have improved and see it working well. I have made a number of
incremental improvements to how I work and will continue to do so this year.
CONTINUOUS IMPROVEMENT: BELOW EXPECTATIONS
“ I struggle to identify new improvements in my work that I can actually improvement.
I do not feel empowered to make changes and find it difficult to implement new
processes at work. I know this is an area of improvement for me and a focus this
year.

“ I understand the focus on continuous improvement, but I believe we are positioned
to be efficient as we currently are. This is at odds with the desires of management
and one area which I understand I am at conflict with coworkers. I understand I need
to change in this area.
“ One of my focuses has been on major, breakthrough changes to the detriment of
allowing small changes to take hold. This has resulted in a year of missed
opportunities. This next year, I will focus on making small changes that will make
major changes – over time.
“ I do not feel like I have ownership of my work and do not feel the motivation to
make changes to how I work. I understand this isn’t a popular opinion, but one
which I hold. I would like to feel more ownership over what I do so the changes I
make hold value.
“ I am at a loss for coming up with new ideas for continuous improvement. I don’t
know how to creatively think up ways to improve and struggle with decision making
when presented with different options to potentially improve. This is something I
struggle in and would like more assistance finding ways to improve my work.

COOPERATION
COOPERATION: EXCEEDS EXPECTATIONS
“ I find it very easy to adapt to changing circumstances. Our environment is always in
flux and I try to handle the changes well and find ways to help others accept change.
My goal is to make a positive contribution to team morale and I believe I am
successful at this.
“ Through my leadership, our work environment is positive and inclusive. I create an
atmosphere of mutual teamwork and support. The team cooperates well with one
another and makes sure the job gets accomplished.
“ One of my strengths is offering assistance to coworkers in need. By making sure
everyone understands this is a team effort, we have been able to accomplish more
together than any of us could individually. I will continue to contribute in this area
over the next year.
“ My focus this year has been to be an encourager of my fellow team members. I try
to always be willing to lend a hand in times of need and bring teams of people
together around a common goal. This has led to a team-centered environment.
“ I believe I am an asset because I can effectively work with people to complete a
project or successfully achieve a goal. I understand how to bring disparate groups of
people together and promote cooperation.
Simple Evaluation Page 31
COOPERATION: MEETS EXPECTATIONS
“ I cooperate well with coworkers, staff, managers, and members of other teams. I
find it easy to ensure that we are focused on team goals and help each employee
both accomplish their individual goals, and also their team goals.
“ I give of myself to make sure the job is well done. I encourage others around me to
work towards a common goal. I believe our team is a model for teamwork and
cooperation.
“ I coordinate individual workers together to meet deadlines and work effectively as a
team. This is an area which I believe I am very good at. I am always willing to lend a
hand and find creative ways to ensure others will as well.
“ One of my strengths is finding ways to make a positive contribution to teamwork. I
make sure projects are a team effort and am always willing to lend a hand when
needed. This has created an inclusive and cooperative environment.
“ At all times I try to be an encourager and make sure everyone is able to effectively
contribute to the team’s goals. I work to coordinate individual team members
together to ensure we work well together and accomplish much together.
COOPERATION: BELOW EXPECTATIONS
“ At times, I have struggled collaborating with others. This has been an area I have
been working on, but I have not always been successful with. I have been very good
with individual tasks, but fall short in cooperating on group projects and in
partnership with staff members. This will continue to be an area of focus for me.
“ I can learn and take on new tasks with ease, but find it difficult to train and
communicate these tasks to team members. I have not been consistent enough with
effectively involving others in learning new systems or processes.
Simple Evaluation Page 32
“ Unfortunately, a perception has arose that I am unwilling or unable to offer help in
times of need. I need to manage this perception and demonstrate a renewed effort
to cooperate with team members and make sure the job gets done.
“ When I moved into this role, I came from one which was very individual-focused and
demanded a great deal of focus. In this new job, I am expected to perform a much
more team-focused effort and I have frankly struggled to make the transition. I will
continue to work on focusing my efforts towards the team.
“ Some believe I am very territorial with my work, but I believe I am just better at
accomplishing tasks alone. I do share crucial information and ideas with coworkers,
but the actual tasks I prefer to work with alone. I understand the team nature of our
work environment and will continue to make efforts to be more inclusive.

Chapter 9
CREATIVITY AND INNOVATION
CREATIVITY AND INNOVATION: EXCEEDS EXPECTATIONS
“ I display signs of originality everyday with the fresh ideas I bring to the table. My
unique and helpful suggestions have been used on multiple occasions to increase
office productivity and improve the overall work environment at the office. I know
these creative contributions have proven to be an indispensable part of conducting
innovative business.
“ I exemplify a refined standard of creativity and innovation, skillfully adapting to new
information and facing challenges that arise with spirit and determination. I
successfully funnel the creative recommendations of staff into practical applications,
proving my ability to productively contribute to the team. Additionally, I am
constantly questioning common practices and regulations in order to refine and
improve office standards.
“ I easily imagine new solutions and then put them into practice. When faced with a
difficult problem, I am often approached to develop a solution no one else has
thought of. I am very visual and can see the solution before anyone else. I will
continue to excel with my creativity.
“ I show consistent strength in creativity and innovation, frequently contributing new
ideas at meetings. I pull the team together to focus on a common goal, and offers
alternative suggestions when obstacles arise.
“ I foster creativity on my team by holding brainstorming sessions and encouraging
new ideas. This has led to a number of improvements to work processes and
Simple Evaluation Page 34
increased the productivity of my team. I encourage thinking of alternatives when my
team runs into a wall and have built a collaborative work environment.
CREATIVITY AND INNOVATION: MEETS EXPECTATIONS
“ I like to provide a new perspective on difficult projects, thinking creatively and
providing fresh ideas. At times others see my approach as aggressive and I am
working to soften how I say things.
“ I have proven to be consistently creative and innovative, actively participating in
meetings and making suggestions for improvement. I offer a creative touch to an
otherwise monotonous role. This artistic flair has provided a number of creative
ideas for our team to implement.
“ I satisfy the expectations for creativity and innovation, consistently providing useful
feedback to improve project strategies. My suggestions have been put to use on
many occasions. I also show ingenuity when facing difficult situations. I will continue
to work on offering creative solutions.
“ I satisfy all standards for creativity and innovation. I show consistent progress in my
work, and makes continued efforts to actively participate at meetings, offering new
ideas and perspectives. Whenever we need a fresh look at a problem, I try to
provide a novel perspective.
“ Although I have struggled with originality in the past, I have shown great
improvements over the past year. My recent assignments have exemplified signs of
creativity, proving my ability to explore new perspectives in order to approach a
project differently. I show great potential in creativity and innovation.
CREATIVITY AND INNOVATION: BELOW EXPECTATIONS
“ I believe I have shown great potential in the areas of creativity and innovation, but
there are times when I struggle to complete things. I have a tendency to develop an
Simple Evaluation Page 35
exciting new idea, but lose confidence as the project faces problems. Instead of
tackling the problem productively in these situations, I look to others to solve them.
“ I know I have struggled lately to remain focused enough to innovate. In the past, I
found it easy to be creative, but since moving into a manager role, I have struggled
to stimulate ideas among my staff and peers. I know I have the potential to excel in
this area and will work to rediscover my past talents.
“ I find it easiest to be creative when working with a team, but honestly I lack
confidence to be creative on my own. This appears on the outside as being reluctant
to explore new approaches and alternatives. I understand my work would improve if
I were more willing to take risks and face challenges.
“ I am moving in the wrong direction when it comes to creativity and innovation.
Lately, my contributions have decreased and at times, I lack motivation to face
challenges with initiative, choosing to let others offer solutions instead.
“ The biggest challenge I have found in being creating is trying to create original ideas.
I am hesitant of risks, which is causing my work to suffer. This is by nature – I am not
a risk taker and like to play it safe which has resulted in an appearance of a lack of
creativity. I plan on working to improve in this area this year.
Simple Evaluation Page 36
Chapter 10
CRITICAL THINKING SKILLS
CRITICAL THINKING SKILLS: EXCEEDS EXPECTATIONS
“ I have a very analytical mind. I understand how to logically examine something and
provide a well reasoned response. My ideas are well grounded and practical. I
believe critical thinking skills is one of my strengths.
“ One of the areas of focus this year has been training the team to think about the
long term implications of short term actions. This has created an environment where
calculated risk taking is rewarded and understanding the long term needs of the
business is almost second nature. The methodical nature my team uses to pursue
long term ideas is refreshing. I believe the attention I placed on this area has really
paid off this year.
“ I have been very good this year at analyzing situations and discovering the right
solutions. There have been a number of difficult issues to examine and resolve and I
believe I have handled them all very well. I will continue to work in the area of
critical thinking this year.
“ I have excelled at reviewing a situation and quickly identifying the key elements of
the situation, what potential paths we can take, and how to summarize it. This skill
has led to a number of situations where we have been able to find different paths to
take and generate additional revenue.
“ There is often little middle ground when it comes to critical thinking skills: you either
have it or you don’t. This is one area where I believe I have performed excellently
this year. Critical thinking skills have been key to our team’s success and I have been
the highest performer in this area.
Simple Evaluation Page 37
CRITICAL THINKING SKILLS: MEETS EXPECTATIONS
“ Critical analysis skills are in short supply and I have made this an area of focus this
year. On a number of occasions, I have applied critical thinking to situations and we
have reaped good results. I will continue to focus on improving my critical thinking
skills.
“ One of the key challenges in our department is being able to properly analyze a
situation and discover all of the factors involved and offer a good diagnosis. This is
one area I have paid particular attention to this year and believe my results speak
for themselves. I plan to continue to contribute in this area.
“ I have an analytical mind and enjoy dissecting a situation to discover root causes,
analyze data, and offer suggestions for the proper direction to go. These skills have
resulted in excellent resolutions for many situations this year.
“ I am very good at recognizing problems and creating a prioritization of the steps to
solve it. My research skills are excellent and have, many times, identified unstated
assumptions and information. I am able to discern the right solutions and draw good
conclusions from all of the information presented.
“ Connecting the dots from A to B is a difficult skill to master, but one which I believe I
excel at. I am able to marshal pertinent information and properly interpret the
information to find the connection between seemingly unrelated issues. My
judgment of potential resolutions has been very good.
CRITICAL THINKING SKILLS: BELOW EXPECTATIONS
“ I am good at analyzing a situation and providing alternatives and ideas. I do well with
short term problems and situations, but struggle to think more long term and how
the short term actions could affect long term decisions. This is an area of focus: how
to strategically think about the long term business.
Simple Evaluation Page 38
“ Sometimes, I find I struggle to develop good conclusions from data provided. There
are times when I am not entirely sure of the best solution and do not have adequate
time to fully analyze the situation. I am working on providing more well-reasoned
analyze and learning how to discard irrelevant information.
“ I am good at analyzing a situation, but struggle to help my team develop these skills.
Unfortunately, I have dismissed a number of good analyses in favor of my own,
when I probably should have spent more time reviewing and critiquing them. This
has resulted in a couple of situations where my team has become upset with me.
This is an area of focus for improvement this year.
“ On occasion, I find myself convinced a particular solution is correct without
reviewing all available options. I realize this has caused a number of problems or
delays that could have been avoided. I plan on working to be more critical of my
ideas to ensure I am seeing the entire field of options before selecting a path to
pursue.
“ Though I believe I am very good at acquiring and interpreting information, I struggle
when it comes time to analyze the information to provide potential options for
moving forward. There have been times where I have spent far too much time
analyzing a situation. This is an area I will work to improve in this year.

CUSTOMER SATISFACTION
CUSTOMER SATISFACTION: EXCEEDS EXPECTATIONS
“ I understand the latitude available to ensure customer satisfaction and do
everything necessary to retain customers. My customer service ratings are stellar
and I have very satisfied customers. I understand how to deal with difficult
customers and resolve major problems.
“ A focus on the customers is the quickest way to generate good returns for a
company. I believe I am deeply focused on the customer experience. I want our
customers to love our brand and our company and through this dedication to the
customer, increase our revenue.
“ Customer service has been a focus of mine this year. My internal customer surveys
have been excellent and I have received excellent ratings from my manager and
coworkers. Impressing customers with high quality, unique service is my specialty
and something I will continue to perfect.
“ I believe I offer excellent customer service to meet the varied needs of my clients. I
listen to customers, understand how to deal with difficult ones, and aim to have a
satisfied customer with each experience.
“ Without customers, I wouldn’t have a job. I understand the importance of the
customer relationship and how cultivate a happy and satisfied client. This is crucial
for the success of our team and our company. I will continue to provide excellent
customer service.
Simple Evaluation Page 40
CUSTOMER SATISFACTION: MEETS EXPECTATIONS
“ Though my customer service ratings are high, I have missed a number of
opportunities to up sell clients. Up sells make up a good portion of our net sales and
I understand I am required to meet a sales quota. Unfortunately, I have frequently
failed to meet the sales quota.
“ My customer satisfaction numbers met goal this year. I have a very good approach
to handling customers and believe this has resulted in an excellent image for the
company. I will continue to work on improving the customer experience.
“ I am good at handling customer interactions – and very good at handling difficult
customers. I understand how to listen to and calm down customers in stressful
situations. I believe this has resulted in a number of great customer experiences and
saved the company several customers. I will continue to improve with my customer
service.
“ I have built a number of great relationships with our clients and focus on customer
service with each interaction. This is an area of strength for me, I am very good at
reading the customer and crafting a solution that is right for them. I can improve in
follow-up communications with clients this next year.
“ I believe I am good at handling customer situations and resolving problems. My
customer surveys have been consistently good. I value our customers and work hard
to make sure they are pleased with the company and my service.
CUSTOMER SATISFACTION: BELOW EXPECTATIONS
“ I have been made aware that the company has received a number of complaints
about my customer service. These complaints center around follow up to customer
issues and has resulted in a number of unsatisfied clients. This has resulted in a
number of problems which must be resolved. I will focus on improving my customer
service.
Simple Evaluation Page 41
“ My customer service ratings have not been up to par this year. I have struggled to
retain customers and have found dealing with difficult customers to be frustrating.
This is an area I understand I need much improvement in and plan to participate in
new training and coaching.
“ Though I don’t have an external customer facing role, I do deal with a number of
internal customers, many of whom are upset with my performance this year. I have
struggled with some of the relationships of key customers for our department. A
focus this year is to rebuild these relationships and improve my customer service
ability.
“ There are times when I have struggled with difficult customers. I do not always know
how to handle the situation and frankly do not enjoy angry customers. These
situations are out of my comfort zone and I struggle to find a resolution. This is an
area I will work to improve this year.
“ I understand not all of my clients have been happy with my work, though I am trying
to mend these relationships and deliver stellar results. I believe additional training
and coaching will help me learn how to properly handle these customers and ensure
a disagreement doesn’t turn into a lost client.

DEPENDABILITY
DEPENDABILITY: EXCEEDS EXPECTATIONS
“ I faithfully commit to get the job done and consistently perform above expectations.
This dependability and willingness to get things accomplished is a great asset to my
team. When something has to get done, I am someone who will get it finished and
delivered.
“ There are times when we have tasks that absolutely must get done. I have shown
that I will do what it takes to get things done. I do so with a positive attitude and
willingness to complete it cheerfully.
“ I believe I am one of the most reliable employees at our firm. I demonstrate this
through a commitment to meeting or exceeding deadlines for projects and goals. I
will continue to improve in this area and continue to be trustworthy.
“ I am committed to success in this organization! I find a way to get the job done,
despite any obstacles in the way. I believe I am responsible, trustworthy, and
dependable. I will continue to grow in this area.
“ There have been a number of times this year when I have worked late, weekends, or
changed my schedule to meet the demands of a project and deliver it on time. I am
willing to do whatever it takes to get the job done in an effective manner. I believe I
am a very reliable and committed member of the team.
Simple Evaluation Page 43
DEPENDABILITY: MEETS EXPECTATIONS
“ I have demonstrated that I am one of the hardest workers and I care about my job,
my team, and the company. I believe this has shown my reliability and I have
contributed as required to the needs of the team. I will continue to grow with my
dependability.
“ I understand I am relied upon to provide great performance each day. I believe I
have achieved this. I believe my steadfastness on the job is admirable and I work
without ceasing on major projects or if we have a problem.
“ I am very consistent with my work product and deliver on time, each time. I believe I
am a responsible employee who completes tasks in a timely and efficient manner. I
will continue to work on improving my dependability.
“ I reliably complete my work on time and within budget. I consistently perform well
and receive high marks for my productivity. I work well with others and respond to
requests for help. I work late as required and am willing to alter my shift to help my
coworkers or management.
“ I am committed to getting the job done with whatever effort, scheduling, and
demands required. I do not complain when asked to work overtime and offer to help
when others are swamped with work. I find time to make sure my staff are not
stretched too thin and offer to help to alleviate stress.
DEPENDABILITY: BELOW EXPECTATIONS
“ There have been times this year when I have struggled to complete tasks on time.
This has caused my dependability to be questioned, but I have worked to improve in
this area and will continue to find new ways to demonstrate my reliability.
Simple Evaluation Page 44
“ Since moving into management, I have struggled to keep up with my own duties as
well as to help when the team gets behind on work. Some believe this has reflected
negatively on my dependability. I will work to balance the demands of my
supervisory role and the needs of my team.
“ Due to personal needs, I have not been able to work overtime or weekends as
sometimes required. This has offered an impression that I am not committed
entirely to getting the job accomplished. I am committed, I just cannot meet all of
the demands on my time. I will work to free up my schedule this next year.
“ Though I believe I am mostly reliable, I understand there have been a number of
projects I have been late with and have not effectively communicated before the
project was late. This is an area of focus for me and one which I will improve in.
“ A number of times I have failed to help my teammates as they struggled under the
weight of the work. I have my own workload which is cyclical in nature, yet I should
be more considerate of others when they have difficulties. This year, I will work to
improve my dependability.

ETHICS
ETHICS: EXCEEDS EXPECTATIONS
“ I believe I am a morally sound person who stands on conviction. I have a solid base
of principles and will not compromise them.
“ In an international role, it can be easy to find your ethics compromised, but I have
stood firm and have not violated company policies, US, international, or local laws. I
demand this level of ethical values among my staff.
“ Not only do I stick by the letter of the law, but also the intent of the rules. There is
little gray to live in, the rules are meant to protect us and the firm and I follow them
very precisely.
“ I am deeply concerned about business ethics and interested in how it applies to real
world situations. In each action, I think about what consequences could occur and
how to manage those through a lens of morality.
“ I like to look at each major decision from different perspectives: moral, ethical,
philosophical, legal. I believe I have a very good sense to make solid decisions which
not only are legal, but are also highly ethical. This is one of my strengths in business.
ETHICS: MEETS EXPECTATIONS
“ I meet all standards for ethical behavior and do not have any lapses in this area. I
follow all company policies, as well as local laws. I continually demonstrate a high
standard of ethics.
Simple Evaluation Page 46
“ I understand the reasoning behind ethics, laws, and regulations, and how a
particular decision fits within this. Being ethical is extremely important for our
business to succeed and I believe this is an area I am good at. I will continue to keep
up to date with changes in this area.
“ I have high morals and am consistent in my ethics and values. I believe I am a
trustworthy employee who follows the rules very strictly. I hold my team to a high
moral standard.
“ In a position like mine, my moral convictions are on display and I believe I pass the
test. I provide a good example for those around me and place a particular emphasis
on ethical behavior with my team. I will continue to work in an ethical manner.
“ I have high ideals and expectations for myself and the employees that work for me. I
consider myself a highly ethical employee and I am consistent in demonstrating
those ethics.
ETHICS: BELOW EXPECTATIONS
“ On occasion, there have been gray areas which I may have dabbled in. This isn’t
where I want to be all of the time. I do not think this is a significant ethical violation,
though I will continue to improve in this area.
“ I have had some difficulty getting my head wrapped around some of the rules and
regulations in our industry and how ethics applies. I don’t think I have been
unethical, but I know questioning some of these rules have raised some eyebrows.
“ Unfortunately, a number of times this year, I lied in a situation and was caught in the
lie. This turned into a larger problem and one which needed gentle negotiations to
resolve. I understand the ramifications of my actions and what is required to fix
these actions.
Simple Evaluation Page 47
“ There are times when I don’t understand why a particular rule is in place and have
violated them for expediency’s sake. I understand this is not acceptable and should
not be repeated.
“ Though I don’t believe I have been unethical, my manager has noted a number of
times where my team feels I have been backhanded or undercut them. This has
negatively affected team dynamics and my own position as their leader. This is an
area which I am acutely aware of and will work to resolve.

Chapter 14
GOAL AND OBJECTIVE SETTING
GOAL AND OBJECTIVE SETTING: EXCEEDS EXPECTATIONS
“ I always meet predetermined targets and deadlines. When I set a goal, I make sure
the goal is accomplished, despite what obstacles may be in the way. This determined
performance has led to major accomplishments this year for our team.
“ I have a driven personality which thrives on goal setting. I understand how to frame
my work within objectives and then create proper goals to achieve them. I
understand my purpose at this company and strive to succeed in each task I
undertake.
“ I structure much of my work and my team’s work around goal accomplishments. We
regularly meet to set team goals, be it daily, weekly, monthly, quarterly, or annual
goals. We set them together and receive everyone’s buy in. This helps drive
achievement by moving the team the same direction together. Goal and objective
setting is something I enjoy and strive to instill in all of my employees.
“ One of the primary drivers behind my success in this department is my use of daily
task lists which are part of a master list of goals. Not only do I work towards
company and department goals, but I have also defined a number of personal and
professional goals. I build a task list each day to achieve these goals.
“ Goals are the driving force behind our productivity, teamwork, and achievements.
Personally, I contribute greatly to our department and company goals, but I have
also achieved (and in many cases, exceeded) my professional goals. I will continue
delivering excellence in this area.
Simple Evaluation Page 49
GOAL AND OBJECTIVE SETTING: MEETS EXPECTATIONS
“ I am proficient in setting goals based on objectives. I understand how to break these
goals down into steps to achieve them. I work on these elements of the goal until
the goal is achieved. I will continue to work diligently in goal and objective setting.
“ I understand how to create SMART goals and effectively manage my work to achieve
them. I believe goal setting is important to getting things accomplished and place
the proper emphasis on it with me team. I know how to set specific goals which are
measurable and achievable in the timeframe allotted. I will continue to grow in this
area, setting good goals which move the business forward.
“ We can achieve more together and I build my team around goals and objectives.
First we set the objectives for the team and then develop the goals together to meet
those goals. At times, we struggle to meet some of the stretch goals, but overall, we
have been much more productive and happy since placing this into our regular work
process.
“ I have a number of goals defined for me each year and I work hard to achieve each
one. This past year, I achieved all of my goals through hard work, extensive planning,
and dedication. I will continue to work hard to achieve the goals we set.
“ Early on in my career, I didn’t think much about goals or objectives, but now that I
have used them in my day to day work life, I don’t know how I would operate
without them. Setting goals and working through tasks is an essential part of my
every day work and something which drives me to success.
GOAL AND OBJECTIVE SETTING: BELOW EXPECTATIONS
“ I have struggled to achieve my goals this year. I think the root cause of this has been
a lack of focus on the goals and a general atmosphere of “putting out fires” instead
of proactively working towards the future. This year, I plan to set aside time to
achieve these goals and be proactive instead of constantly reacting to emergencies.
Simple Evaluation Page 50
“ I have not followed a systematic process to achieve my goals. This has resulted in
lower results than could otherwise have been achieved. I understand the process is
in place for a reason, but have not found the time to implement it for goal setting. I
plan to use it from now on and achieve my goals next year.
“ I set many stretch goals for myself, and unfortunately, did not achieve them. I
spread myself across all of the goals and did not focus on any individual goal, causing
all of them to fail. I would have been better off focusing on a few of them than all of
them, and will work to accurately set goals this year which can be achieved.
“ I had poorly defined goals this year that in the end weren’t measurable. This left goal
achievement open to interpretation. Since my goals were so poorly constructed, I do
not believe I did well in this area and will better define my goals next year.
“ Though I achieved my personal goals, we did not achieve our department goals.
Since I was an integral part of these goals, not achieving them was a failure for me.
This next year, I will better manage the team to achieve both our personal goals and
our department goals.

INITIATIVE
INITIATIVE: EXCEEDS EXPECTATIONS
“ When I see an opportunity for improvement, I take it. I enjoy finding new ways of
increasing efficiency and productivity. This is a definite area of strength and one
which I feel contributes significantly to our company.
“ Finding new ways to do things is exciting for me. Taking on a new project and seeing
it to fruition is why I work here – I want to succeed and help the company find
success through me. I believe I am very resourceful when it comes to taking on new
work and I always find a way to get it done.
“ There are many opportunities to take the lead in ventures and I frequently step up
to assist. I think this is one of my strengths: finding new ways of doing things and
then executing on them. If I see something that needs done, I do it.
“ I learned a long time ago that sitting static in a comfort zone won’t work. I like to
step out of my comfort zone and accomplish new things by taking initiative. When a
task needs done, I volunteer for it. When a project needs help, I step up. When a
customer is repeatedly having problems, I help fix the situation. Taking initiative is a
passion for me.
“ I believe I am a resourceful employee who finds ways to get work done. I believe it is
advantageous to step forward and take on new challenges in the workplace. I take
on these challenges with ingenuity and a passion for accomplishments.
Simple Evaluation Page 52
INITIATIVE: MEETS EXPECTATIONS
“ I understand what work needs to be accomplished and I know how to get the work
completed. When I see work that must be done, my natural inclination is to get it
done. I will continue to work hard in this area.
“ When I have available time, I ask for additional work or opportunities to grow. If a
coworker is struggling, I find time to help them. Initiative isn’t just something to do,
it’s way of approaching work and one which I think I do well with.
“ I believe I am a resourceful person who finds new ways of accomplishing things,
while taking on additional work and helping alleviate the backup of work we have on
our team. There is plenty of work to get completed and I take the initiative to
resolve it myself.
“ When I see something needed fixed or done, I immediately seize upon it as an
opportunity to accomplish something, or learn something new. I work to expand my
knowledge and job skills and also to get things accomplished for our department.
“ I love to take the bull by the horns and get things done. I feel good when something
is accomplished and the only way to get things done is to continually seek out new
opportunities for improvement. This is one of my strengths.
INITIATIVE: BELOW EXPECTATIONS
“ Though I believe I am a hard worker, I struggle to take the initiative to ask for more
work. At times I have some down time, but I do not offer to take on more work or
help others who are struggling. I will work to improve in this area this year.
“ At times I feel I am overwhelmed with work and find it difficult to take on new
assignments. I do not always take the initiative to ask for more work when there is
downtime. This is one area which I understand I must do better in.
Simple Evaluation Page 53
“ I understand that I require more direction than some of my coworkers, yet I believe I
am still new to this group and want to make sure I do exactly what is expected of
me. Initiative has been difficult for me as I just do not know what I could take on
without some direction. With time, I will improve in this area.
“ When new tasks need to be done, I am usually asked to do them. I don’t ask for
more work assignments or for additional work. If someone needs something done, I
expect that they will ask me for help.
“ I am intimidated by some of my coworkers and find it difficult to approach others for
help or to find additional tasks I could take on. Instead, I wait for others to ask for
help if they require it. I would like assistance overcoming this.

INTERPERSONAL SKILLS
INTERPERSONAL SKILLS: EXCEEDS EXPECTATIONS
“ I have a natural rapport with people and am at ease in business and personal
situations. I relate very well to people and make them feel valued. I try to make
people feel important and know I appreciate them.
“ I am a positive influence to those around me and I build confidence in them to
achieve more. I have built solid relationships throughout the organization and
understands organizational dynamics. I have a passion for my team and want them
to succeed.
“ I believe I connect well with coworkers and management. I understand relationship
dynamics and how to build high quality relationships. I try to be a pleasure to work
with and will continue to connect with my team mates.
“ I believe I am very good at making a connection with just about everybody. I have a
friendly personality that people enjoy. I make people feel good when working with
them and build quality relationships with my coworkers, direct reports, and
managers.
“ I find it easy to relate to people and find a connection. I try to show people I truly
care about them as individuals and want to help them find success, while still
managing high productivity. I believe I have a gift in bringing people together around
a common goal.
Simple Evaluation Page 55
INTERPERSONAL SKILLS: MEETS EXPECTATIONS
“ I have an even personality which helps to stabilize our team. My friendly and
professional manner helps coworkers relate, as does my high quality work and
pleasant interactions. This is an area of focus for me which I believe I have done well
in.
“ I am a nice and affable person who works well with others. I am gracious and
responsive most of the time and am open and honest with feedback. I plan to
continue building strong relationships with my management team.
“ I am an affable fellow, but on a number of occasions, I have been overrun by
coworkers with strong personalities. I need to learn how to push back on these
individuals and understand that I do not need to say “yes” to every demand they
make.
“ I like to think I am an amiable and charming person who understands how to relate
well with those around me. I find it easy to discover things I have in common with
people and build a professional and productive working relationship. I will continue
to work to improve in this area.
“ Working with people doesn’t take a particular expertise, it boils down to whether or
not you care. I believe I care about people and understand how to motivate them to
excellence through the relationship we’ve built.
INTERPERSONAL SKILLS: BELOW EXPECTATIONS
“ I have a very strong personality which has caused a rift on the team. I know I have a
tendency to “steam roll” fellow employees to the point that it has affected team
dynamics. I am a very productive person who has a driven personality, but this has
negative consequences as I have detailed. I need to work on toning down my
interpersonal dealings.
Simple Evaluation Page 56
“ I have struggled building quality relationships with my coworkers. The feedback I
have received is that I am rigid and impersonal. I understand I have not responded
well to attempts to be included and instead I have alienated many of those around
me. I plan on opening up and building professional relationships with his coworkers.
“ I do not cultivate crucial good working relationships with my coworkers. There is an
impression that I can be very difficult to work with and on a number of occasions,
my outbursts have disrupted work. I understand this is an area I must improve in.
“ I know this is one area that I definitely need improvement in. Though I find it easy to
relate well with others, I am often too shy to carry on strong relationships with
people I work with. Some people think this is aloofness, but instead it’s just shyness.
I will work on coming out of my shell and working with others.
“ There are groups of people on our team who have formed cliques that I have a
difficult time breaking into, so this year I have severed many of those relationships
and it has negatively affected my team. I will work to improve this situation this
year.

JOB KNOWLEDGE
JOB KNOWLEDGE: EXCEEDS EXPECTATIONS
“ I act as the resident specialist for administrative issues because of my excellent job
knowledge. I lead our training seminars and ensure that everyone is acting under
protocol at all times. I also makes useful suggestions on how to improve existing
protocol.
“ I take a very strategic approach to work, incorporating my extensive business
knowledge into each assignment. My awareness of competing firms in the industry
keeps us ahead of the game in many departments, and I am a key contributor to
successful planning sessions.
“ I stay up-to-date about all the latest trends and advancements in my field, sharing
them with my colleagues to ensure maximum productivity. I keep the team strong
by staying informed and sharing my knowledge. Additionally, I document important
progresses and changes in an internal guide that can be used throughout all
departments.
“ As far as job knowledge goes, I believe this is a strength. I keep my extensive
network of industry connections accurately maintained for utilization, and I am
always on the lookout for new information. My understanding of company policy
and procedure exceeds that of the majority of my colleagues and I am always
interested in learning more.
“ I understand company processes and how to follow them to precision. I recognize
inefficiencies and work to improve them. I am very knowledgeable with my daily
tasks and perform them with ease. I will continue to grow in my job knowledge.
Simple Evaluation Page 58
JOB KNOWLEDGE: MEETS EXPECTATIONS
“ I show good judgment when handling routine problems. I am proficient at my job
and demonstrate this through continued excellence at accomplishing my tasks. This
year, I am going to improve by sharing more information with my coworkers to cross
train them on my tasks.
“ I have proven my job knowledge through all of my completed assignments. My
wealth of information relevant to my field and specific responsibilities makes me
one of the most valued team members. Unfortunately, I struggle with keeping my
subordinates up-to-date on this knowledge, so I will work on that this year.
“ I am technically efficient at all aspects of my work. I stick to company policy and
protocol through every step of an assignment, and try to stay abreast of key
advancements relevant to her field.
“ I approach each challenge that arises with a unique strategy. My success can be
attributed to my knowledge of the field, and my expertise at finding hidden
information. I will improve by taking more initiative to include my peers in this
process, helping them to advance to my level.
“ I understand what is expected of me and perform my job well. I have a sufficient
appreciation for the gaps in my expertise and work to improve those. I will focus on
on-the-job training and increased exposure to other work teams to grow in my
career this year.
JOB KNOWLEDGE: BELOW EXPECTATIONS
“ I have proven in the past to have extensive knowledge in my field, completing
assignments with expertise and efficiency. Unfortunately, I have failed to produce
the same results from my subordinates, who have complain in surveys that I am
unhelpful and unsupportive to problems that arise. I will improve in this area this
year.
Simple Evaluation Page 59
“ I have a tendency to want to rely on my own knowledge to complete assignments
and struggle to ask for help when it’s needed. Unfortunately, I’ve been told my work
product is short of impressive in these instances. I need to work on changing my
approach, and I need to be more willing to learn from my peers.
“ While I make consistent promises to put more effort into my work, my actions
haven’t lived up to these promises. I am prone to making the same mistakes
multiple times. I believe that with more effort, my work can be much improved,
even exceptional. I know I have the potential for good things and I just need hard
work to achieve it.
“ I depend too much on the instruction of my superiors, and need improvements to
efficiently work alone. While I follows direction well, my hesitancy to take on
independent assignments shows a lack of confidence in my knowledge, and an
inability to meet new challenges head-on.
“ I have little awareness of our competitors, and have made little effort to remedy
this. My manager has advised me to educate myself on key issues relevant to work
and I have begun this process. I understand that I need to adjust my attitude and
approach work with a new perspective.

Chapter 18
JUDGMENT AND DECISION MAKING
JUDGMENT AND DECISION MAKING: EXCEEDS EXPECTATIONS
“ I believe I have excellent decision making skills. When presented with a difficult
situation, I can easily review all of the options and make an educated decision. These
decisions are based on experience, expertise in the industry, and a “gut” feel. I
believe these decisions have been shown to be excellent.
“ I am a shrewd decision maker with the wisdom to discover the right answer to a
problem. I use common sense and experience to determine which is the best path to
travel down. This is an area I will continue to work hard in.
“ The ability to make a firm decision does not come easy to me, but instead, it’s
something that I have spent years crafting and refining. I believe this has resulted in
excellent decision making skills and an ability to make a judgment call that is
unsurpassed by my peers.
“ I am judicial with how I respond to questions and situations. I understand words
have an impact and ensure that I choose the right ones before making a decision or
giving direction to my staff. This is an important distinction between leaders and
managers and one which I believe I excel in.
“ My decision making process is very deliberate. Some find it slow, but in the end, the
time it takes to make a decision pays off if the decision is a correct one. Time has
shown that despite the delays, my decision making is sound and has saved the
company a tremendous amount of time and money.
Simple Evaluation Page 61
JUDGMENT AND DECISION MAKING: MEETS EXPECTATIONS
“ I have an even keel personality which has worked well when it comes to decision
making. I weigh all of the options and provides a well reasoned response. I can
continue to improving in this area.
“ I have the right way of thinking about problems: weigh the information presented
and make a wise decision based on the potential outcomes. This has served me well
over the years and I believe I have made difficult, yet discriminating, decisions.
“ I make well reasoned decisions from the facts presented, with the likely outcomes
describe. Decision making is neither all art, nor all science, but a combination of the
two. I believe I have a good mix of the two to make intelligent decisions considering
all of the circumstances.
“ Decision making is a particular strength of mine. I am very focused and determined,
yet my mind can be changed with the right information. I have used this mix to make
quick and effective decisions which have proven to be good ones.
“ At times when difficult decisions must be made, I step forward and make the tough
calls. These may not always be popular decisions, but they have to be made and I
have the fortitude to make a decision and stick with it.
JUDGMENT AND DECISION MAKING: BELOW EXPECTATIONS
“ This is one area which I know I need to improve in. I find it difficult to review all of
the options and discover the “right” answer to the problem. Sometimes, I find
myself indecisive and unable to make the right decision. I will continue to work on
improving in this area.
“ I am frequently indecisive when presented with a difficult situation. I do not believe
this is because of a lack of knowledge, but likely a lack of confidence in my skills. I
Simple Evaluation Page 62
am continuing to work in this area to improve my experience and ability to make a
quality judgment on problems.
“ I am often too quick to make decisions and a few of those have backfired. This has
resulted in some lost revenue to the company and is something I have struggled
with. To remedy the situation, I am pausing and weighing the consequences before
making a decision.
“ My judgment has been questioned on a number of decisions due to what might be
considered faulty login in decision making. I am learning to better justify my
decisions and completely document the nature of my decision making process. This
will help me improve in this area tremendously.
“ Though I have struggled making sound decisions, I think that my process to get to a
decision is detailed and well grounded. It takes me longer than some to arrive at a
decision, though I eventually reach one. This is an area that I will work to improve
this year.

LEADERSHIP
LEADERSHIP: EXCEEDS EXPECTATIONS
“ If there is one area on this evaluation that I believe I excel in, it’s leadership. As my
360 degree reviews have shown, my team respects me and enjoys working for me.
This is an area of strength: influencing those around me to deliver great results. I
enjoy leading a team and hope to be blessed with the opportunity for years to come.
“ There are two types of leaders: safe on the stage and guide by your side. I am a
guide by your side, moving along with people to achieve their goals and ultimately
the company’s goals. I understand how to work with people to get the best out of
them.
“ I am an excellent coach and regularly receive maximum performance from the
people that work for me. I couldn’t succeed as a leader without a high performing
team and my team performs above par.
“ I understand how to use daily tasks and motivate employees to meet a long term
goal. I understand different attributes of leadership and how to apply them with
different types of employees.
“ I am very good at influencing people to achieve great things – not just work a job.
People that work for me have indicated they trust and respect me. I trust in the full
capabilities of my team to achieve something that individuals could not achieve
alone.
Simple Evaluation Page 64
LEADERSHIP: MEETS EXPECTATIONS
“ I have performed well in leadership, directing a team to meet goals and maintain
high levels of productivity. I understand how to influence team members and help
lead them to meet expectations. Leadership is not given, its earned, and I believe I
have earned it.
“ I am very good at leading others through the maze of our organization. I can help
others set a course and navigate through it. This is my strongest part of being a
leader and one which has proven a great asset to this organization.
“ I understand different types of people and the different ways to motivate them to
get the job done. Leadership is not a part of a job description, it is something earned
and I believe I have earned it from my team.
“ I am not in management, but I believe I have very good leadership skills. People
around me believe I have a self-less attitude and I work hard to make sure they
know I care. This builds respect and trust – things teams are built on.
“ I delegate well to my employees and build trust in them to take responsibility and
initiative, and then motivate them to respond. I don’t just manage my staff to
accomplish goals, but I lead them to meet objectives and ultimately, success.
LEADERSHIP: BELOW EXPECTATIONS
“ Though I think I have been a good manager, honestly, I have struggled at being a
leader. I find it difficult to inspire my team, though I can create a productive team,
one that desires to be productive eludes me. I will take advantage of the company
mentoring program to improve my leadership skills.
Simple Evaluation Page 65
“ At times, I struggle to understand the full capabilities of my organization and utilize
them to the fullest extent. This has held me back as a leader. I do not get the most
out of my staff and will continue to work on improving my leadership ability.
“ I have done well defining a vision for my organization, but I have struggled to
motivate people to meet this vision. I have provided direction, goals, and objectives,
but do not find people working towards them. I have struggled to gain trust and
motivate staff.
“ Since my transfer, I have not become adequately up to speed in job knowledge and
have found it difficult to provide direction and implement measures across the
team. With more knowledge about how this department works I will become a more
well rounded leader.
“ I am an introverted person and have found it difficult to connect with people in a
manner you would typically think of for a manager. I have a tendency to focus on
small tasks and get lost in the details versus thinking about the big picture and how
to achieve large tasks.

LISTENING SKILLS
LISTENING SKILLS: EXCEEDS EXPECTATIONS
“ I am always looking for new ways to receive and interpret information presented to
me. I believe I am an excellent listener and understand the principles of active
listening. I listen attentively and demonstrate good listening skills.
“ I am a focused, active listener. I am adept at focusing on a speaker and providing the
necessary feedback. I elicit feedback while listening and let people know I value
them and their problems.
“ I understand conversation dynamics, how to read people, and how to intelligently
decipher what they are trying to say. Part of the challenge of conversations is
figuring out what is said and what is unsaid. I am very good about figuring out what
is unsaid in a conversation.
“ I listen carefully during conversations and allow speakers to finish before talking. I
understand how to respond appropriately and involve everyone in a discussion. I
believe I have mastered the art of active and focused listening.
“ I am very adept at comprehending, retaining, and responding during conversations.
These components make up active listening and I am exceptional at active listening.
This is one of my strengths and an area which sets me apart from other employees.
LISTENING SKILLS: MEETS EXPECTATIONS
Simple Evaluation Page 67
“ I understand the principles of listening: understanding, interpreting, and evaluating
what I hear. I believe I am a good listener and provide active feedback to people
during conversations. I absorb the information presented and restate it back to
ensure I understand what is being said. I will continue to improve in this area.
“ I have improved my listening skills significantly over the past year. I have a sincere
interest in others and listen fully before responding. I listen carefully when people
speak to me. I value conversations and continue to grow in this area.
“ I listen, comprehend, and ask the right questions when given direction. I am very
good at listening and conversing with people. When someone is speaking with me, I
respond both verbally and physically to demonstrate I am listening.
“ I understand the difference between being an active listener and a passive listener.
In each conversation, I have learned how to be an active listener and focus on those
speaking with me. I use open ended questions to probe further into conversations.
“ When speaking with someone, I listen, comprehend, and evaluate what is being
said. I am able to effectively form a response and carry on an intelligent and
effective conversation.
LISTENING SKILLS: BELOW EXPECTATIONS
“ The feedback I have received so far in this process is that I have not listened well this
year. I understand that I do not always provide active feedback. I also appear
distracted at times during conversations. I am working on becoming an active
listener.
“ There are times when I have a tendency to drift off during conversations. This
happens most often during long conversations. I have been made aware of this and
so I try to focus on when it happens so I can bring myself back into the conversation.
This is something I will continue to work on.
Simple Evaluation Page 68
“ I appear very distracted during conversations, though I am often still listening to
what is being said. I understand this impression needs to be corrected so others
know that I care about what is being said and care about the conversation I am in.
“ I am inconsistent with my listening skills. I have a tendency to do “all the talking”
during conversations and leave little room for others to participate. I have, on
occasion, talked over others while they are trying to talk. These are all things I will
work on to improve my listening skills.
“ In a problem situation, I have been known to scold instead of listening, empathizing,
and coaching others. When confronted with this type of situation, I can let the
emotions of the moment get the best of me. I will work to control these emotions.

MANAGERIAL STYLE
MANAGERIAL STYLE: EXCEEDS EXPECTATIONS
“ This year has been a difficult one merging two teams together, but I think the results
speak for themselves. We have had the best year yet and I believe my consistent,
sound leadership has helped contribute to that.
“ I am a high performer leading a high performance team. My leadership has
contributed to high productivity in our department and we will continue to perform
at a high level this year.
“ I excel at managing a staff. I believe my staff are happy working for me and we
achieve all of our goals each year. We have a dynamic group of people and one
which excels at taking on new challenges.
“ As this was my first year managing a staff, I didn’t know what to expect. It’s been an
awesome experience! My team has performed much higher than expectations and
we’ve exceed almost all of goals. We’ve had no turnover this year and the staff has
worked hard to create an open and inviting work environment.
“ You can’t be a successful manager without a great team of people working for you,
and I believe I have one of the best. I feel funny about putting this as such a high
strength as its really due to how well they perform that I am able to excel in
management. I will continue supporting my team to achieve great new things for the
company.
Simple Evaluation Page 70
MANAGERIAL STYLE: MEETS EXPECTATIONS
“ I am a consummate professional and manage my staff with ease. My direct reports
have provided me high ratings on 360 degree reviews and our performance level has
been on target all year.
“ I am a very effective manager who understands the balance between family and
work. I act in a professional manner with my staff and they provide excellent results
for me. I will continue to work on improving my management skills.
“ I have been very adept at managing difficult employees and turning their energies
towards higher performance. Though this was an area I struggled with in the past,
this year has seen a renewed effort and the results have demonstrated that my
focus was spot on.
“ Managing is about bringing people together, creating expectations, and helping
them reach those goals. I have a great team that has developed wonderful,
supportive team dynamics. Without this team, we would not be able to achieve the
goals we have set. I have a great team and I’m privileged to be their manager.
“ I understand how to bring a group of disparate individuals together to form a high
performing team. Though we’ve had some struggles, we’ve come through it stronger
and more united than ever. I look forward to all of the great things we will
accomplish this year.
MANAGERIAL STYLE: BELOW EXPECTATIONS
“ There are times when my team members feel I am too hard charging and our
personalities have clashed. There is a general feeling I am too focused on
productivity. This next year, I will work on my soft skills and improving my
relationship with my team.
Simple Evaluation Page 71
“ I have had difficulty making the transition from staff member to manager. I have
several close personal relationships with people who now report to me and this has
caused some friction on the team. I am working to reduce or sever some of these
ties to improve team dynamics.
“ Though I think I have excelled at managing staff members with personalities similar
to my own, I have had difficulty managing team members with other types of
personalities. I understand that survival as a manger isn’t the goal, excellence is. I
will continue to work in this area to improve my relationship with all team members.
“ We’ve struggled merging people into our team. This broke team dynamics and it’s
been difficult getting everyone back onto the same page and performing. I take
responsibility for this and will work as hard as ever to bring everyone together
around a common goal.
“ I have not provided the support or attention my staff members need to be high
performers. I have been asked to participate on several projects which has drawn
me away from day to day management and I have struggled in this area. I hope to
spend more time with the team this next year.
Simple Evaluation Page 72
Chapter 22
ORGANIZATION AND PLANNING
ORGANIZATION AND PLANNING: EXCEEDS EXPECTATIONS
“ I go about my work with a very deliberate and planned methodology. I believe I am
excellent with my organizational and planning skills. This planning results in efficient
work – meeting deadlines and achieving a lot of what we set out to accomplish. This
is an area of focus for me which I think has paid off.
“ I begin each week with a plan and then each day keep my plan up to date to achieve
maximum effectiveness. Planning is second nature to me – not only do I enjoy it, but
I think it is the best way to achieve high productivity.
“ In a project based work environment, sufficient planning is crucial to our success. I
excel at creating plans which are realistic, achievable, and have specific deadlines. A
number of projects I have managed have come in ahead of schedule and under
budget. This is primarily due to excellent plans.
“ My work environment is very organized and clean. I expect this from my team and
they deliver. I believe an organized workspace reflects an organized mind. Teams
that stay organized are able to deliver more and our productivity speaks to this
principle.
“ I have not always been as well organized as I am now. I have taken a number of
courses and read a few books and believe that this is something I now excel in. I now
enjoy having an organized workspace, schedule, and staff.
Simple Evaluation Page 73
ORGANIZATION AND PLANNING: MEETS EXPECTATIONS
“ I have a very methodical way to go about business. I work hard to ensure each
project is well planned and organized. I know that an ordered way of doing things is
the best way to get them accomplished and believe I do well with my organizational
skills.
“ There were times in my career where I struggled to maintain an organized schedule
of work, but through hard work, training, and perseverance, I now believe I am very
good at organization. There are times where I could better plan, but overall, I am
pleased with my progress in this area.
“ I am a detailed person and enjoy making arrangements for events. Scheduling them
to run smoothly is one of my strengths and something I truly enjoy. I would like to
work more in this area, setting up new events and helping to make them run
smoothly.
“ Forecasting needs into the future is a gift I possess. I understand how to effectively
arrange staff members to get the most work out of them and how to find the right
resources to make sure projects and normal day to day work get done.
“ Each day, I start out with a plan for what I would like to get accomplished. Most of
the time, I achieve what’s on my list – and sometimes more. This has shown to be a
very effective way of planning my work and helps ensure I am kept busy and
accomplish many of the tasks expected of me.
ORGANIZATION AND PLANNING: BELOW EXPECTATIONS
“ One of my weaknesses is having a disorganized work environment. I understand that
I do not organize my workspace for maximum effectiveness and frequently have
piles of paperwork that needs to be organized. This is an area I must improve in.
Simple Evaluation Page 74
“ I often find myself disorganized (and as my managed described it, disheveled),
though it is an area I am really focused on. I am unsystematic in how I approach my
job, which in this field is problematic. This is an area for improvement.
“ I don’t know if I would go so far as to say my work style is chaotic, though from the
outside, it could appear to be this way. I find it difficult to fall into the norm of
organization and instead like to work with my own methodology. Unfortunately,
many people find it confusing and off putting.
“ I find it difficult to adequately put together a plan that covers every contingency and
situation. A number of times this year, this has come back to hurt us as my plan was
not sufficient. This is an area I need further training and experience in.
“ A number of work issues have turned negative due to a lack of preparation on my
part. I do not always prepare as thoroughly as I should and this has set us up for
disaster. I do not know precisely what training or education would help me, but I do
know I need more knowledge and training in this area.
Simple Evaluation Page 75
Chapter 23
PROBLEM SOLVING
PROBLEM SOLVING: EXCEEDS EXPECTATIONS
“ I always analyze a decision effectively before implementing it. This analytical
approach to problems has resulted in an excellent closure rate on problems. Over
this next year, I will continue to work to improve my problem solving skills.
“ My investigative skills have provided a key resource for a team focused on solving
glitches. My ability to quickly assess a problem and identify potential solutions is key
to my excellent performance.
“ I have an analytical mind that is quick to discover the root causes for problems. This
is an area which I believe I am a strong asset in. I am able to analyze a situation,
describe it succinctly, discover what is a symptom and what is the actual problem,
and then offer creative solutions to resolve the situation.
“ I am adept at tacking some of the trickiest problems we have faced as a team. I
enjoy solving these types of problems. I understand how to dissect a problem and
discover the core issues so we can make a rational decision on how to move
forward.
“ I have the ability to unravel even the most difficult problems to find potential
solutions. In some teams, problems can be a major setback, but I believe our
collective troubleshooting skills are top notch. I believe I am a big part of that.
Simple Evaluation Page 76
PROBLEM SOLVING: MEETS EXPECTATIONS
“ Though I believe my problem solving skills have been great this year, there are a
number of areas of troubleshooting that I know I can improve in. I am working on
learning how to more quickly analyze a situation and provide multiple options for
solving a problem.
“ I can easily break down a problem into smaller components to analyze the entire
issue in an efficient manner. I strive to find all of the probable solutions to an issue.
From these, I can provide all of the research necessary for decision makers to decide
a path to take.
“ I am an independent thinker who likes to analyze problems to discover the root
cause. I understand the testing process and how to discover a solution to a particular
problem. I would like to improve this year by understanding when to seek outside
opinions to discover other potential options to fix the issue.
“ I believe I am one of the most technical employees on our staff, yet I can also
translate a problem into language others can understand. I have a natural ability to
find a solution to even the most difficult of problems. I am very good at problem
solving.
“ Some of our systems are very problematic, but I believe I have a great ability to find
workarounds and solutions. Through experience and intuition, I have solved a
number of very difficult problems and am always prepared to help out in this area.
PROBLEM SOLVING: BELOW EXPECTATIONS
“ I have excelled at identify a problem, but struggle to discover and offer potential
solutions. Sometimes I become fixated on the problem and can’t see beyond it to
help analyze the potential decisions that need to be made. This is a focus area of
improvement.
Simple Evaluation Page 77
“ I have struggled to narrow down the root cause of problem, often being distracted
by the symptoms. This has made troubleshooting difficult for me. I would like
continued on the job training and a chance to study the knowledgebase to increase
my ability in this area.
“ I have a difficult time troubleshooting problems, but once potential solutions are
offered, I find it easy to pick the right solution. Our lack of excellent troubleshooters
has shown to be a problem in the past. I am working to improve in this area so I can
excel in both aspects of problem resolution.
“ I believe I am good at testing solutions for a problem, but struggle to be creative in
finding new solutions. Not every problem has been documented and I often struggle
at discovering new issues and resolutions. This is definitely an area of focus for me
and one which I will improve in.
“ A number of times, I have been poor at communicating problem status until it
becomes a small crisis. This has caused lost productivity as I need to bring in help on
the team to solve these types of problems. I will work on communicating problem
status sooner in the life cycle to avoid these incidents.
Simple Evaluation Page 78

PRODUCTIVITY
PRODUCTIVITY: EXCEEDS EXPECTATIONS
“ I have made productivity a focal point of my management efforts this year and it’s
paid off. My concentration and focus on productivity has seen tremendous
improvements in our numbers. I will continue to focus on creating new
opportunities for improved productivity on my team.
“ I manage my team to consistently high performance, while still maintaining great
employee satisfaction. I understand how to motivate people to perform and my
team has met all of our productivity goals this year.
“ I am at the top of nearly all performance measures for individuals on my team.
Getting the job done is very important to me, and I believe, one of my greatest
strengths. This diligent attitude towards work has paid off.
“ I understand how to properly set goals and use those as motivating to get the job
done. This results in a very high level of productivity with little downtime required.
Through proper planning, I know this type of increase in productivity can be
duplicated to other staff members and I will work hard to help this happen.
“ I am focused on working smart this year. In a down economy, we are expected to do
more with less and I believe I have been innovative in how I approach work and this
has resulted in great performance for the year. I will continue to work to discover
ways to be more efficient in my job.
Simple Evaluation Page 79
PRODUCTIVITY: MEETS EXPECTATIONS
“ I complete my tasks in a timely and efficient manner. I keep a tight calendar and
make sure this results in a high level of productivity. I will continue to work on
productivity and become a more efficient employee this year.
“ I moved into a new role this year and my productivity dipped as I took on new
challenges and learned my new position. I know I am a hard worker and I have been
working on becoming a “smarter” worker. This next year, I will continue to learn in
my new position and become more productive.
“ One area that I believe I do very well in is working hard to make sure the job gets
done right the first time. Rework can kill productivity and a focus on ensuring things
are correct pays great dividends. I can continue to improve in this area and plan to
focus on it this next year.
“ I set the right tone with my team to balance productivity and personal needs. This
has resulted in a very consistent output from an efficient team. I know hard work
isn’t everything, but I have created an environment where hard work is expected
and rewarded.
“ I have demonstrated I am an industrious worker with high output and high quality
work. I know it’s important to create stretch goals and have almost met them all.
Though managing a project is not my strength, I know how to execute on the
processes required to achieve them.
PRODUCTIVITY: BELOW EXPECTATIONS
“ There is a perception that my work quality has decreased recently and I will continue
to work to improve that. With the increase in workload on our team, I have not been
able to participate in projects or meetings as fully has I have in the past. I will work
to become re-engaged with assignments and balance the work I need to perform.
Simple Evaluation Page 80
“ I have not focused on productivity this year, despite our financial condition, and we
did not achieve the goals desired. I understand I must improve my own productivity
and create a work environment where my team can be more productive as well.
“ At times, my delivery has been inconsistent, but I have been working to improve in
this area. I have taken criticism of my work well and used it to improve how I work
to try to improve efficiency. I will continue to seek feedback and incorporate that
into my daily work habits to improve in this area.
“ Though I have not consistently met quota, I believe I have worked hard. I have not
seen the results I have desired and have been researching ways to improve my
productivity. This is an area of focus for me this year.
“ Productivity has not been my strength in my new role and I have struggled to keep
with the pace of my team. I need to find the right balance in my work environment. I
will continue to seek feedback and work on continuous improvement in the area of
productivity.

PROFESSIONALISM
PROFESSIONALISM: EXCEEDS EXPECTATIONS
“ I am adept at keeping a calm demeanor in all sorts of circumstances. As a steady
force in my group, I help others understand how to professionally deal with
situations and reduce stress and the potential for problems. This is an area I believe I
excel in.
“ One of the key attributes to my success with our company is displaying a
professional attitude. I am an authority in our company’s product and enjoy training
others. In the future, I will continue to excel at professionalism.
“ I work in a number of key customer situations and represent the company with
excellence. I manage my team to display a great attitude and respect towards
coworkers and the customers. As a result, our team is viewed as very professional.
“ I provide excellent support to my customers, display a professional attitude in every
encounter, and support my staff very well. I value the relationships I have built and
act as an even keel for my team, despite the circumstances.
“ One of my strengths is demonstrating excellence in my day to day job. I project an
image of professionalism in each task I undertake and project I work on. My goal for
each thing I do is to make sure it represents myself, my boss, and the company well.
Simple Evaluation Page 82
PROFESSIONALISM: MEETS EXPECTATIONS
“ I have many years of experience in this field and understand how to meet all of the
requirements of the job in a skilled and polished manner. I desire to be known for
being a professional in all I do.
“ I work hard to be professional in every business situation and project a good image
for the company. I maintain a professional image and attitude. Whenever dealing
with external customers, I project a professional image for our firm.
“ I project an image of professionalism in each task and project I work on. I have a
calm demeanor when working in difficult situations and understand how to diffuse
people who are upset. This results in high quality work situations with people I work
with.
“ I work within the stated and informal expectations of my management team. I meet
all of my professional obligations and act according to official codes of conduct. I will
continue to work hard to be professional in all work situations.
“ I conduct myself properly in all situations and act appropriately when conducting
business. I demonstrate awareness of proper etiquette in situations and
competently display skills and behaviors.
“ I am in a position where relationships are highly valued and I try to meet all
standards of moral and ethical behavior. I work well with others and represent
myself and the firm well. I will continue to work to improve in this area.
PROFESSIONALISM: BELOW EXPECTATIONS
“ There are times where I have been told my appearance is unprofessional and I have
worked hard to improve in this area. I understand that appearance can really drive
Simple Evaluation Page 83
someone’s opinion of you and this next year, I will work to improve my business
appearance and be known as a polished businessman in all circumstances.
“ The one area I have struggled in for professionalism is my work area. I understand it
is not clean or tidy and this is an area I am working on. I am organization my work
area and making sure I have enough storage for my files and that I deal with
paperwork as soon as it comes in.
“ I have worked hard to improve how I work with people and control what I say and
do. I know I have fallen short in this area, but continue to work to improve. This next
year, I will continue to seek feedback to improve how I relate with others.
“ There have been times where I have acted unprofessionally and my manager has
helped me identify those and quickly correct them. I will continue to work on how I
behave and how I interact with my coworkers.
“ A number of times this year, I have been involved in situations where my conduct
has been unprofessional. I did not realize the affect this had on those around me
and will work hard to improve in this area.

Chapter 26
QUALITY
QUALITY: EXCEEDS EXPECTATIONS
“ One of my focuses this year has been ensuring a high level of quality from my work
product. I believe this focus has paid off. My productivity has increased and the
overall quality of the product is much higher this year as compared to last. I will
continue to work in this effort for high quality.
“ Quality management is not just a human resources fad – it’s something we should all
have in mind in our day to day tasks. Quality is extremely important to me. Getting
the job done isn’t enough. I continually work towards getting the job done right the
first time.
“ Quality is a measure which has impact beyond just my job. Quality is something I am
focused on in my day to day work and I believe I have had a good impact on quality
this year. By improving processes, I have seen a dramatic quality increase in our
team’s production.
“ One of the challenges to ensuring high quality across the enterprise is to break down
barriers between departments, lead, and improve constantly. I have mastered these
components of quality management and understand how to effectively lead quality
programs within the company.
“ Part of making quality a focus at our company has been to review processes, identify
weaknesses, and improve them. This has been both an area of focus for me and one
which I believe I have excelled in. Continual improvement is an important part of
quality and one which I have been a champion of.
Simple Evaluation Page 85
QUALITY: MEETS EXPECTATIONS
“ I am focused on ensuring our team’s output not only meets quantity standards, but
quality as well. This takes the focus off of just how productive we are, but how
productive we are when delivering an excellent product. I think this focus has caused
us to perform admirably in quality this year.
“ I have been instrumental in implementing ISO quality management and helping the
firm achieve its ISO certification. I have worked through a number of activities and
sought to improve our quality measurement for many processes.
“ The quality of my work is very high and something I have worked on quite a bit this
year. I believe that quality is very important to both internal and external customers.
Improving quality has led to improved productivity as we do not have to fix items
and the original shipped is the best product.
“ I believe my work product has been very good. I have received few complaints and
most of it is error free. This is an improvement over last year and a focus of my
efforts this year. I attended a quality training class and applied its principles to my
work. I will continue to grow in this area.
“ One of my goals this year was to achieve quality on a more consistent basis and we
have achieved this goal. We started by building a culture that supports and desires
quality. I contributed to building this at work, participated in training classes, and
helped audit quality programs.
QUALITY: BELOW EXPECTATIONS
“ My productivity has been very high, but quality has suffered. I understand we should
not sacrifice quality for productivity. I will work to improve quality so both
productivity and quality are high.
Simple Evaluation Page 86
“ I have worked hard to improve my own quality, but this has been at a detriment to
the production efforts of my team. In the future, I will be more considerate of others
when working on improving quality on my own efforts.
“ I have not focused enough on internal customer feedback when designing new
quality control processes and the resulting improvements has not resulted in higher
quality. Since then, I have redesigned how we approach quality and this should
result in better quality management this year.
“ I did not pay appropriate attention to quality management training programs until
late in the year. We had a number of lost opportunities to improve quality with this
delay and I am working to make up for the lost time.
“ I rushed the quality program through without properly gaining acceptance among
staff and helping to build a culture of quality. I learned from this mistake and have
begun taking a much more deliberate approach to quality management
implementation.

SALES
SALES: EXCEEDS EXPECTATIONS
“ In a tough economic climate, I have consistently proven the ability to exceed sales
targets. This has placed me among some of the top salespeople at our firm. I will
continue meeting and exceeding sales goals this next year.
“ There are many aspects to being successful in sales. I demonstrate proficiency in
many of these attributes: assessing needs, finding the right solution, presentations,
influencing people, negotiations, and asking for the business. My techniques have
been used in our training classes for salespeople. I will continue growing as a
salesman and continue finding solutions for our customers.
“ I believe I am an expert salesman. I can look at any problem and see the sales
opportunity. My calm demeanor and firm presence relaxes clients and encourages
them to partner with our firm. I close more high margin, high value clients than any
other salesman on our team.
“ In many ways, sales is about building relationships. I have demonstrated that I
understand interpersonal bonds and how to forge a partnership with our clients.
This has transformed our sales process and made our product an integral part of the
customer’s success – creating an opportunity for our success. I will continue to
contribute to our success in this area.
“ I am very good at selling customers on solutions and not just products. This has
resulted in high margin service sales, increasing company profitability. I am a reliable
resource for my clients, meeting their needs by finding new solutions we can help
them with.
Simple Evaluation Page 88
SALES: MEETS EXPECTATIONS
“ This year, I have met all sales performance targets. I believe I consistently perform
well on sales presentations and working with new clients. My business retention
rate could be improved, but overall, My sales performance has been very good. I will
continue to improve sales retention targets this year.
“ I am a consistent sales performer. I regularly meets sale targets and provide high
quality sales support to his clients. I work well with others and understand the needs
of his customers. I will work on meeting increased sales targets this year.
“ I have performed marvelously under difficult circumstances and place the
customers’ needs above my need to close the sale. I have worked well with technical
sales staff and sales management to enhance the sales process. I can continue
improving, especially in the areas of negotiation, qualifying sales leads, and
generating customer referrals.
“ I have been a very good inbound sales representative this past year. I have handled
the customers’ calls with a professional, respectful attitude. My customer survey
results and supervisor reviews have both been very positive. I can continue to
improve closing inbound sales and up selling the clients on other high margin
products.
“ I moved from internal sales to external sales this year and I believe I have handled
the transition very well. I have expanded my customer base beyond the clients
assigned to me and have served them very well. I have kept my sales expenses
under control and have received very good reviews from my client base. I will
continue growing as a salesperson, taking the coaching and training offered.
SALES: BELOW EXPECTATIONS
“ I understand my sales numbers have dropped off this year. I have a good aptitude
for sales and clients report good interactions. This year, I plan to focus on improving
Simple Evaluation Page 89
sales numbers through additional follow up calls and increasing the number of sales
calls made.
“ I understand how to make an initial sale but I find up-sells and cross-sells difficult to
close. My total revenue per client has been lower than it should be and I know this
places me in the bottom third of salespeople. I plan to improve this through more
interaction with clients after the initial sale and by providing sufficient post-sales
support.
“ I find it easy to sell in one-on-one sales situations but have a difficult time presenting
to groups. When presented with a group sales presentation situation, I know I
become visibly nervous and do not project confidence in the product or in my own
abilities. I plan to take some public speaking courses or join Toastmasters to increase
confidence in speaking in front of large groups.
“ I have made sufficient sales results this past year. Where I have struggled is in
identifying, understanding, and selling against our competitors. I need more
competitive intelligence to achieve much better sales results. I plan to increase my
competitive research and highlight the competitive advantage we maintain over
other firms.
“ I struggle to close sales. I believe I deliver a quality sales presentation and seem to
build rapport with potential clients, but my closure rate is low. This has resulted in a
much lower sales performance when compared to my peers. I plan to improve the
ability to close a sale over the next several months.

TEAMWORK
TEAMWORK: EXCEEDS EXPECTATIONS
“ I have a great willingness and desire to help out my fellow teammates. I have
applied this practically on the job by assisting as required, and frequently offering
assistance even when it’s not asked for. I believe I work well with others and put the
team first in all things.
“ Our company is built on teamwork and I understand how to foster team dynamics in
a high energy, fast moving work environment. My interactions with others can be
seen as contributing to the success of the company, my organizational unit, and my
department.
“ I believe I excel at putting together high performance teams and running them
smoothly. These teams provide a high quality work product and do so with high
productivity and efficiency. I will continue to build great teams at our company this
year.
“ As a matrix organization, it is crucial to be able to work well with a variety of
different people and I believe this is a skill I have mastered. I work to make sure
people feel appreciated and contribute at their maximum when working on a team
with me.
“ I am a master at understanding relationship dynamics and how these translate to
team-based success. I know how to select the right group of people to work well
together, and when handed a team, how to find common ground so we can work
towards success.
Simple Evaluation Page 91
TEAMWORK: MEETS EXPECTATIONS
“ I believe I am a consummate team player and regularly make contributions to the
success of the team. I always help out to achieve the goals of the group and am not
concerned about who gets the credit, just the task gets accomplished.
“ I am not concerned about myself and continually contribute to the success of those
around me. In a dynamic team environment, I work well with others and understand
how this can be beneficial to the company. I will continue to work by contributing to
team efforts.
“ I manage the relationship with my coworkers, managers, and other employees in a
professional and engaging manner. I look to other people for help when required
and offer it as much as needed from others. I believe the success of the team is very
important and work in my daily tasks to ensure it is successful.
“ I am not concerned with who gets the credit: I want our team to be a success in
everything we do. To this end, I contributed to our team’s success with everything
opportunity given. I will continue working hard to find success for our group.
“ I understand how to make proper suggestions and critiques to those working around
me. I also have learn how to take suggestions and critiques from my coworkers and
managers. I regularly contribute to the success of our team.
TEAMWORK: BELOW EXPECTATIONS
“ I have focused on self improvement this year and several of my coworkers believe
this has been to the detriment of the team. I will work this year to be a better
teammate and assist others on the team more frequently.
Simple Evaluation Page 92
“ I am often focused on the task at hand which others see as a lack of teamwork. I am
working on making sure coworkers understand I care and want success for the team.
I will work on making sure my team contributions are more consistent in the future.
“ I understand I have been inconsistent in supporting my teammates this year and will
work this next year to improve this. I will provide support and guidance when asked,
volunteer to work on project teams, and work to participate in team functions.
“ I have worked well on my own teamwork, but have not encouraged my direct
reports to really form a high quality team. This has resulted in less productivity – my
team would accomplish more if they worked better together. This is an area of focus
for me this year.
“ I work well on small teams, but struggle when it comes to participating in larger
groups. I find this intimidating at times. I am working to improve my participating
with larger teams and finding a way to be more productive and inclusive when
working with these larger groups of people.

TECHNICAL SKILLS
TECHNICAL SKILLS: EXCEEDS EXPECTATIONS
“ I have a very analytical mind which has resulted in excellent technical abilities. I am a
skilled troubleshooter – understanding the underlying technical issues of a situation.
I will continue learning over the next year and continue to improve my technical
skills.
“ I am able to grasp complex technical concepts and explain them in easy to
understand manner. I believe I have approachable personality, despite my technical
aptitude. I am a teacher at heart and help raise the technical level of the employees
around me. I can improve my technical skills by taking additional training classes this
next year.
“ There are a lot of intricacies to the technology used by my team, yet I believe I make
it appear very simple to manage. High performance in technical skills is taking a
complex system and making it look easy from the outside. I am excellent at taking
complexity and managing it with precision. This next year, my team will continue to
take on more responsibility and expand its technical experience.
“ I can quickly learn new technology and adapt it to our environment. Whenever we
have a very technical problem, I am one of the first to volunteer to find a solution.
There are a lot of intricacies to our work and I understand how to navigate through
them to find the right solutions.
“ Things change very rapidly with our technology and I am able to quickly grasp the
changes. I am able to take complex topics and make them simple for people around
Simple Evaluation Page 94
me. Technology is something I very much enjoy and analyzing complex situations is
one of my strengths.
TECHNICAL SKILLS: MEETS EXPECTATIONS
“ I understand the technology we implement and how to effectively use it in our
environment. I take advantage of training opportunities. I believe one of my
strengths is applying technology on the job.
“ Some of our older systems are in the process of being replaced and I’ve been a
resource to be relied upon for new systems. I can quickly learn new technology and
explain it to coworkers in a way they can understand. I am able to apply techniques
and ideas we have from the older system into how the new system will work.
“ I am good at absorbing complexities of the job and understanding how to best apply
them to the day to day tasks. I believe I am one of the first people management
turns to when there is a technical issue needing resolved.
“ Effectively using technology is one area I believe I excel at. At times, I have struggled
in troubleshooting technical problems, but overall, I believe I am very technical in
nature.
“ I have demonstrated that I am an accomplished technician who understands how to
best implement systems and processes. There have been a number of opportunities
for me this past year to demonstrate these skills and I believe I have performed well.
TECHNICAL SKILLS: BELOW EXPECTATIONS
“ My current role is very technical in nature, but I have struggled to grasp the basic
technical skills required. I believe more training would help me learn some of the
complex technical skills of my job.
Simple Evaluation Page 95
“ I have struggled to absorb information in training classes and have had difficulty
applying it on the job. I find technology difficult to understand and use. This has
been a problem as we have moved to more automated systems, but I am working to
improve my technical aptitude.
“ I have not yet demonstrated the basic mastery of the machinery and equipment
required in my position. I understand this is a fundamental requirement of my
position, but I have found the on the job training to not be complete enough for me
to learn this position.
“ I understand the basic technical knowledge required, but have had difficulty learning
the nuances of the technology our team uses. I think my business knowledge is very
good, but my ability to communicate with the more technical members of the team
is holding me back.
“ One of my struggles has been how to apply technical knowledge learned in training
classes to the job. I think it’s difficult to think outside of the box, beyond the
manuals, and troubleshoot undocumented technical issues.

TIME MANAGEMENT
TIME MANAGEMENT: EXCEEDS EXPECTATIONS
“ Having a long history in project management has provided me the ability to
accurately assess a timeline and meet a deadline. I am very good at developing a
solid task list towards a dated goal. I am also very good at managing my day to day
schedule amid a hectic time. I will continue improving in time management over the
next year.
“ I am very good at managing my time and my employees’ time. My staff are regularly
among the highest in attendance and productivity. I understand how to schedule
both big and small tasks. Over the next year, I will improve my skills by assessing
new areas my team can work in and meet those needs on-time.
“ I am a very efficient worker. I define a schedule each week and keep to it. I find that
I am frequently interrupted in this role, but I find a way to keep on track. My goal for
the next year is to begin training my team members on time management.
“ One of my strengths is developing project plans which utilize available resources in
an efficient manner. I hold staff accountable for their time estimates to ensure the
work is completed on time, each time. My project completion percentage is perfect.
“ I am very good at managing my time and accomplishing tasks on time. I have
demonstrated that I work well under pressure and can juggle multiple priorities and
still deliver. I will continue to work on improving my time management skills.
Simple Evaluation Page 97
TIME MANAGEMENT: MEETS EXPECTATIONS
“ I have a very focused personality and believe I will accomplish anything I have my
mind set to. I understand and use the tools provided to achieve my objectives. I set
an efficient schedule and meet it.
“ I am a task oriented person. I am proficient at scheduling tasks and accomplishing
them. One of the skills I have developed is predicting how long a task will take and
accurately completing tasks on time. As I grow in experience, my time management
skills will continue to improve.
“ I am able to adequately schedule my time to accomplish my tasks. I understand at
times there have been, on occasion, some scheduling issues, but overall I believe I
handled it well. We’d like to see Ryan manage his time a little better over the next
year by communicating his schedule more effectively with team members and
management.
“ I am good at time management and keep a tight schedule. I understand that some of
my meetings run long which affects others on the team. Though, by and large, I
believe my time management skills are exceptionally good. I will work on keeping
meetings to their allotted time.
“ Though I believe my time management skills have been very good, I understand that
I have not always succeeded at adequately accounting for the time of my team
which has resulted in lower productivity than expected. I will work to improve in this
area and ensure my team’s time is tracked and managed.
TIME MANAGEMENT: BELOW EXPECTATIONS
“ Though I know my tactics have been questioned, there are strengths and
weaknesses to how I manage my time. The strengths have resulted in a large
increase in productivity of my team. The weakness is I have several team members
Simple Evaluation Page 98
who have indicated they are burned out. I will work to temper my time management
to achieve high productivity and high employee satisfaction.
“ For the most part, my time management skills have been good. Unfortunately,
several of my projects came in late with little advanced notice. I will work to improve
by communicating project status more effectively.
“ I am occasionally late to work and I understand it causes the workers in the shift
before me to have to adjust their schedule. Though my schedule has been
inconsistent, I still produce a high volume of work. I will work to improve my
reliability.
“ One of my challenges has been my willingness to say ‘yes’ to too many requests. This
has resulted in a number of times where I am overloaded and ineffectual. I will work
on being more assertive and only take on work that I can complete in a reasonable
time.
“ When I get immersed in work, I have a tendency to lose track of time and not pay
attention to things around me. This level of focus can be a good thing to get
something accomplished, but when it causes my time to overrun and miss
appointments or project deadlines, it can become a problem. I will continue to work
to improve my time management.

TRAINING
TRAINING: EXCEEDS EXPECTATIONS
“ I am one of the lead trainers for our staff. I try to always be an encourager during
training sessions. I am consistently rated as excellent by employees in the training
classes. I will work to continue providing excellent training to new employees.
“ I encourage my employees to attend training sessions and take advantage of the
training courses offered. I have taken a number of soft skills classes this year and
have learned a great deal. Over the next year, I plan to continue learning and taking
advantage of company training programs.
“ I am a consummate learner and excel in the training offered. I am very attentive
during training class and score among the highest in comprehension tests. I have
also taken a number of independent study courses outside of company training to
improve my business skills.
“ We’ve installed a number of new systems this year and I have focused on learning
the new technology. I have enjoyed absorbing the material and training coworkers
on it. I believe this has produced a great return for the company.
“ There is a significant time investment required for sufficient training for staff
members. This can put a strain on teams as they work to absorb the lost hours .I
have worked hard to schedule time for all of my team members to attend training
and have been excellent in working to absorb the hours invested. I will continue
upgrading skills on my team this next year.
Simple Evaluation Page 100
TRAINING: MEETS EXPECTATIONS
“ I have sought out specialized training opportunities. This has afforded me a number
of great training programs where I can continue to upgrade my knowledge and skills.
I believe this helps make me a valuable team member.
“ Not only have I taken the recommended training classes this year, I have also helped
several team mates find the right training classes for their skill sets. I enjoy investing
time in other people and helping put proper focus on training.
“ I take advantage of training courses offered and have attended all mandatory
training this year. I will continue to spend an appropriate amount of time on training
and taking classes as required.
“ I am quick to learn new concepts and apply them on the job. I am interested in on
the job training opportunities and take advantage of them as often as possible. This
next year, I will focus on continuing to take training opportunities as they are
offered.
“ I enjoy investing time in training and believe the training classes I have taken have
benefited the company and myself. At times, I have struggled to maintain my
workload and some of the classes, but overall, I think it has been beneficial. I plan to
continue taking training as feasible.
TRAINING: BELOW EXPECTATIONS
“ As a long time employee, I struggle with some of the introductory courses. Not that I
struggle with the material, but with the point of having to take time out of my
schedule to attend. I can understand taking more advanced courses, but I believe my
long term experience here should exempt me from basic classes.
Simple Evaluation Page 101
“ I have had difficulty applying some of the concepts taught in mandatory HR training
this year and understanding why we’re asked to participate in some of these classes.
This struggle with the applying these new processes has resulted in lower
productivity. I plan to work on improving my application of training principles this
year.
“ I attend all of the required training classes, but have been made aware that the
instructors do not feel that I participate fully during class. I understand the point of
view and will work on being more engaged in training class.
“ Though I have attended all of the required training classes, I have not thoroughly
encouraged my team to attend training and a number of the staff members missed
several training classes. This year, I will ensure my team is taking the necessary
training courses.
“ I understand training is important, but have not had the time available to attend
classes. My goal this year is to schedule training classes and attend them as
required.
