In her thesis, Lung microbiome of chronic obstructive pulmonary disease patients with and without HIV

infection in Pretoria, South Africa, the promovenda identified and determined variations in the lung

microbiome (using targeted metagenomics and the IS-Pro method) and virome in chronic obstructive

pulmonary disease (COPD) patients. The researcher demonstrated that the sputum microbiome is diverse

and that there were no significant differences between the stable and exacerbated COPD states. Although

the two methods provided similar results, the IS-Pro method showed differential abundance with key genera

of clinical importance. The virome in this study was unique with a high prevalence of BeAn 58058, a relative

of the smallpox virus. As one of the first studies on the lung microbiome and virome in Africa, this study

showed that the microbiome of COPD patients in Africa has similarities to the microbiome of COPD patients

in Europe and the USA and identified possible treatment areas for COPD exacerbations.

In her thesis, The anatomical description of the erector spinae, paravertebral and epidural block for post-

operative pain management in paediatric care, the promovenda aimed to evaluate the anatomical

differences between existing neuraxial techniques, the paravertebral- and epidural block, as well as the

novel interfascial erector spinae plane block in a paediatric population. Using fresh cadavers, ultrasound

images and computed tomography scanning, she demonstrated the extensive craniocaudal spread of the

erector spinae plane block, covering multiple dermatomal segments, while targeting nerves that are

involved in both the paravertebral- and epidural blocks. The study further substantiated using the erector

spinae plane block as an alternative for neuraxial techniques due to its safety profile. This paediatric study

is at the forefront of paediatric anaesthesia research, contributing substantially to the literature.

Milehna Mara Guarido

In her thesis, Identification and phylogenetic analysis of Aedes species (Diptera: Culicidae) and arboviruses

associated with them across tropical and temperate regions of South Africa (2015-2018), the promovenda

conducted a mosquito-based surveillance in the north-eastern provinces of South Africa from 2014 to 2018.

The study provided a new look of the Aedes species as potential vectors for arboviruses and screening using

PCR assays for the genera Alphavirus, Orthobunyavirus, and Flavivirus. Aedes species composition and

diversity were determined and the impact of the trap types and climatic conditions on Aedes species further

investigated. Additionally, blood meal sources of the engorged female were analysed for host specificity

determination. Finally, the research also used DNA barcoding as a confirmatory tool of the morphological

identification of the aedine mosquitoes found in South Africa and to assess the phylogenetic relationship

among these species. The study highlights the importance of a continued mosquito-based surveillance for

arboviruses in South Africa, and the role that Aedes species might play in the circulation of these arboviruses.

In her thesis, A multidisciplinary approach to investigate the manifestations of Diffuse Idiopathic Skeletal

Hyperostosis (DISH) in modern South African skeletal remains, the promovenda utilised both macroscopic

and biochemical analysis to evaluate the presence of this disease in human skeletal remains. Variations in

the δ¹3C and δ15N values found between the DISH and control groups most likely reflected different dietary

patterns, associated with temporal changes, cultural practices, and religious and economic factors,

independent of DISH. While the findings from the stable light isotope analysis yielded no tangible evidence

for a dietary aetiology for DISH, observations from the micro-CT scans corroborate some new clinical

interpretations from the literature, exposing a possible erosive or inflammatory process preceding new

bone formation in the spine. This study has implications for the direction of future research in human

skeletal remains in South Africa and the way DISH and disease in general are assessed.

In his thesis, Ga-68 pentixafor PET/CT imaging of vascular expression of chemokine receptor 4 (CXCR4) as

a marker of arterial inflammation in patients with human immunodeficiency virus infection and its

comparison with F-18 FDG PET/CT imaging, the promovendus showed the presence of arterial inflammation

in HIV-infected patients without known cardiovascular risk factors. These results showed that arterial

inflammation, the precursor lesion of atherosclerosis, is present at a young age in HIV-infected patients,

indicating that they may benefit from an early institution of preventive therapy. The researcher further

validated FDG-PET/CT imaging criteria for arterial inflammation. The results showed that unique imaging

characteristics were needed when applying FDG-PET for arterial inflammation imaging. The candidate found

an adequate level of agreement in the quantification parameters obtained in a head-to-head comparison

of FDG-PET and 68Ga-pentixafor-PET/CT for arterial inflammation imaging indicating that 68Ga-pentixafor-

PET/CT can be used in place of FDG-PET with reasonable accuracy while avoiding the challenges associated

with FDG-PET.

In his thesis, 18F-PSMA-1007 in recurrent prostate cancer, a new frontier in prostate cancer PET imaging,

the promovendus who used 68Ga-PSMA imaging was not able to determine a significant difference in

imaging findings between black and white patients presenting with biochemical recurrence suggesting that

the tumour burden and growth rate in androgen-dependent prostate cancer may be similar between the

two racial groups. In a comparison between 68Ga-PSMA PET-CT and bone scan, he found that in the initial

staging of high-risk prostate cancer patients, the imaging modality of choice should be 68Ga-PSMA PET-CT

due to its superior specificity and accuracy. Overall, the researcher found no significant difference in the

performance of 18F-PSMA-007 as compared to 68Ga-PSMA PET CT in the detection of recurrence of prostate

cancer. However, 18F-PSMA-1007 outperformed 68Ga-PSMA in the detection of intra-prostatic recurrence

in low PSA settings.

In her thesis, A comparison of arterial measurements between a South African cadaver and living sample

as affected by age, sex, height and weight, the promovenda took 15,255 measurements on 321 formalin-

fixed cadavers, and 213 retrospective CT scans and the ultrasound investigation of 81 living volunteers.

Measurements were taken at 20 different arterial sites. The goal was to establish whether there was a

statistically significant difference between the three samples and examine the effects of demographic

parameters. This resulted in one of the most comprehensive examinations of arterial morphology in a South

African sample and confirmed age to be a vital parameter when assessing arterial changes - a useful

indicator of the vascular ageing process. As a demographic parameter, age was more influential when

compared to sex or body size. Cadaver measurements compared well to CT and ultrasound measurements

proving the validity and reliability of future cadaveric studies to examine vascular morphology.

In her thesis, Assessment of macrophage and granulocyte cell-enhanced phagocytosis of Mycobacterium

tuberculosis by novel monoclonal antibodies, the promovenda demonstrated the ability of a novel IgG1

mouse monoclonal antibody (JG7) to enhance opsonophagocytosis of susceptible M. tuberculosis strains at

different growth phases by human U-937 macrophage and HL-60 granulocyte cell lines. Effects were

concentration-dependent and particularly evident at lower bacterial dilutions. JG7 also induced a pro-

inflammatory effect, driven by IL-8 in neutrophils and a significant anti-inflammatory response in

mononuclear cells, shown by the considerable decrease in IL-12p70 and increase in IL-10 respectively. The

research methodology allowed for assessing the magnitude of M. tuberculosis clearance, thereby offering an

approach for determining the efficacy of potential candidates for host-directed therapies against

tuberculosis.

Riaan van Coller

In his thesis, A clinical, neurophysiological and genetic study of South African familial combined myoclonic

syndromes, the promovendus aimed to demonstrate the clinical features and neurophysiological and

genetic findings in families with rare myoclonic syndromes. The promovendus discovered a South African

pedigree with familial cortical myoclonic tremor with epilepsy. This rare condition showed a non-progressive

course during a 30-year follow-up, with typical neurophysiological findings of cortical origin myoclonus.

Genetic analysis revealed a pentanucleotide repeat mutation in the intron of the STARD7 gene. A novel

mutation in the epsilon-sarcoglycan gene was discovered in a family with myoclonus dystonia syndrome,

possibly with the escape of maternal imprinting and an unusual combination of myoclonus and dystonia in

the same body region. Deep brain stimulation surgery was put forward as a treatment modality in myoclonic

syndromes with long-term sustained improvement shown in patients with myoclonus dystonia syndrome

and paroxysmal non-kinesigenic dyskinesia.

In her thesis, The application of two capability models to support 4th year medical students' learning, the

promovenda applied the models of capability to define student learning elements within the Longitudinal-

Community Attachment programme for Students (L-CAS). A mixed-method approach was employed in the

analysis. The individual models demonstrate strengths, challenges and potential areas for improvement in

the L-CAS curriculum. A novel model was derived from both the capability models and aspires to facilitate

the learning process before, during, and after L-CAS visits by scaffolding learning and promoting

autonomous learning. When students arrive at the learning site,, they should have a “CHILL” mindset—

Changing cHallenges into Learning Loops. This should ensure best use of available resources and learning

opportunities at the community learning sites.
