NACCAS 2016 (Data) Annual Report: Excel Cohort Grid InstructionsFor the 2016 (data) Annual Report, NACCAS requires that the cohort grid be completed in Microsoft Excel format. As a result, NACCAS has published this sample Excel cohort grid for official use by institutions. You are not required to use this sample grid; however, should you use your own alternative, it must (i) be in Microsoft Excel format and (ii) contain all fields present in NACCAS' sample cohort grid. Please note that the 2016 (data) Annual Report Instructions were published in January of 2017, and are available on NACCAS’ website (www.naccas.org) in the “Annual Report Information” section, under the “Accreditation” menu. As several changes were implemented, it is imperative that institutions review the 2016 (data) Annual Report Instructions before completing their cohort grids.

In addition to this Instructions page, there are ten cohort grid worksheets listed in this spreadsheet: “Program 1,” “Program 2,” “Program 3,” “Program 4,” “Program 5,” “Program 6,” “Program 7,” “Program 8,” “Program 9,” and “Program 10.” You may switch program worksheets by clicking on the colored tabs at the bottom of the spreadsheet. You should utilize a separate program worksheet (within this same Excel document) for each campus and each program offered by your institution.

If your institution only offers one program, please use the “Program 1” worksheet. If it offers two programs, please use the “Program 1” and “Program 2” worksheets, etc. (For example: “Program 1” worksheet – main campus cosmetology program, “Program 2” worksheet – additional location cosmetology program, etc.) If your institution has more than ten programs, please contact NACCAS for additional guidance.

In order to assist institutions in the process of completing this cohort grid, helpful reminders will appear when you click on a cell. These reminders will inform you of the required format for the data (e.g., mm/dd/yyyy), and will also advise you of the acceptable types of data entries. If you receive an error message after you have entered data in a cell, please double-check to ensure your entry is in the required format. Additionally, some cells in this workbook are "locked," meaning data cannot be entered into them. If you try to enter data into a locked cell, you will receive an error message that states "the cell or chart you are trying to change is protected."

Note: If you find the helpful reminders annoying, you can move them out of your primary viewing screen by clicking on and holding the reminder box with your mouse and dragging the box to the top-right corner of the screen. (While the reminders will still be on the screen, they will no longer block your view of the cohort grid.)
