A feature of Google Docs is being able to create forms.

The first thing you need to do is go to the Google website.

In the black bar above the word Google you will find the more link and click on it. This will then give you a drop-down menu. In this menu you will find Documents and you will need to click on it.3Creating a Form
You will then go to a new webpage for creating documents. Then, you will need to click on the create button where another drop-down menu appears. One of the documents that you can create is the form. You will need to click on that link, so you can begin creating a form which you can use to assess your students knowledge of a skill.

This screen will then pop-up on your screen. As you create the form, you will need to give your form a title in the Untitled form section. Then, you should put a section for the students name as a question and create the rest of your questions. When you click on question type you will get a drop-down menu for text, paragraph text, multiple choice, checkboxes, choose from a list, scale, and grid.Remember, make sure the first question you place is name and you choose the question type to be text.

In order to write all of your other questions you will need to click on the + Add item button at the top of the webpage. Also, make sure to click next to make this a required question box so the students are not able to skip it.
When you finish writing all of your questions you will then need to click on the Done button at the bottom of the page.6

Before you have your students complete the form you can click on a link at the bottom of the page so you can view it.7Preview
When you preview your form you can make sure it looks exactly the way you want it to look.8
Sharing

In order to share this form with the students you will need to open up the form from the Google Docs homepage. First, click on the form that you want to have your students access. Then click on the right side of the page where you will find a share button. You will need to click on it.9

The sharing setting window will popup and you will need to click on anyone with the link option then click on save.10

You will then be given a link to share with your students. This can then be placed in your class wiki so your students can answer the questions on your form.11
