WONDERFUL WEDNESDAY AFTER SCHOOL LETTER AND PERMISSION FORM

Dear Parents:

I hope you all have had a wonderful start of a new school year and are getting back into the fall

routine. Wonderful Wednesday After School Program will start September 5th and we would

like to invite your children and friends to our program. Attached you will find a permission form

(that must be filled out and returned before Sept. 5th) and rules for bus and at the church.

This program is open to all children in kindergarten through sixth grade. The sixth graders will

be helpers. Once again this year we will pick children up from LJBell and Washington Street

Schools. We will follow the school calendar therefore if there is an early dismissal or no school,

we will not have Wonderful Wednesday.

I must have the attached completed permission form for each child in order for them to attend

and ride the church bus. If you need extra copies, you may call the office at 895-4027 ext. 103

or print a form off the church’s website at www.fumcrockingham.org. Please make sure you

send a note to your child’s teacher letting them know he/she will be riding the FUMC bus on

Wednesday afternoons. The bus will pick them up in the bus parking lot on Wednesday

afternoons. If your child is absent from school on a Wednesday, please call the church office to

let us know he/she will not be riding the church bus. Car riders are welcome and will meet in

the Fellowship Hall after school.

Upon arrival at the church the children will have a snack and playtime. We will have Bible Story,

craft, and music. Our program ends at 5:00pm. You will pick your child up at the playground or,

if raining, in the Fellowship Hall. Please go over the rules that are included in this mailing with

your children.

We are still in need of volunteers to help. Please consider the opportunity to make a difference

in our children by volunteering on Wednesday afternoons. You can call me at the church 895-

one time, once a month, or every Wednesday, we can promise you Wonderful Wednesday will

be a blessing for you and this ministry of our church. Your help means, giving one hour and a

half to our children from 3:30-5:00. We appreciate all our volunteers from the past and hope to

see lots of new faces.

Medical Authorization/ Permission Form Children’s Ministries

I, the undersigned, being the parent or legal guardian of the following child, give permission for said

child to participate in the Children’s Ministries, including the After School Program, at First United

Methodist Church, Rockingham, NC. I confer upon First United Methodist Church the authority to sign

on my behalf for any and all medical and dental treatment. Any contracts and treatment authorization

forms without limitation the guardian deems necessary for the benefit of my child in the event of injury

or illness. I agree to pay for all such treatments and not to hold First United Methodist Church, its

employees or volunteers, responsible for injury or any associated expenses. I understand the temporary

guardian will attempt to contact me before the treatment.

These rules are for your safety!

Please sit in the first available seat

Face forward

Keep hands and feet out of aisle

You must talk quietly to the person sitting with you

No changing seats or moving on the bus

No inappropriate words or actions

Respect the bus driver, chaperones, and fellow students

Consequences:

1st warning – Talk between Director and Student

2nd warning – Talk between Director, Student, and Parents

3rd warning – Will not be able to ride the bus

Bring book bags to the choir room and leave in a chair.

Go to the playground get snack, use the bathroom, run, play, and talk with

friends.

Once we enter church use your inside voice

Keep hands and feet to yourself

Be respectful to All the Leaders

No inappropriate behavior or words will be tolerated

We will go by The Golden Rule:

“Do unto others as you would have them do unto you.”

Consequences:

3rd warning – Will not be able to participate in activities

Rewards:

If we all go one month without any warnings—then we will have a

special snack on the last Wednesday of the month!
