¡Feliz año 2015 para todos! Esperamos que todos hayan disfrutado de unas maravillosas fiestas de fin de año y estén ansio‐ sos por lo que puede ser un largo invierno... si creen en el Almanaque del Agricultor. Estamos a la espera del nuevo año y tenemos la esperanza de que continuaremos teniendo grandes avances para curar la Hepatitis C para muchos de nues‐ tros pacientes, así como mejorar la vida de aque‐ llos que se benefician con el regalo de los tras‐ plantes. En el próximo año, CLDT espera seguir adelante y que muchos cumplan algunos de nuestros obje‐ tivos de educación. Por ejemplo, estamos en las etapas iniciales de la planificación de una feria de salud de hígado graso, mayor alcance comunita‐ rio para tratar la Hepatitis C y mejorando los módulos educativos en línea así como las herra‐ mientas de evaluación para atender mejor a nuestros pacientes y a sus familias. Para poder hacer esto, continuaremos examinando oportuni‐ dades para recaudación de fondos. Si alguien tiene alguna excelente idea para recaudar fondos o sabe de algún medio para este fin (juego de bolos, campos de golf, etc.), sírvase enviar sus ideas a [email protected]. Con frecuencia, los pacientes y familiares nos preguntan si hay opciones para hacer donativos directamente al Centro de enfermedades y tras‐ plantes hepáticos. Si se dirige a nuestro sitio Web en www.livermd.org, verá una ficha “Ways to Give” (Formas de dar) en la parte izquierda. Cuando haga clic en esta ficha, se le llevará a una página que describe las diferentes opciones. También puede comunicarse con la Dra. We hope you all had a wonderful holiday sea‐ son and are gearing up for what may be a long winter…if you believe the Farmer’s Almanac. We are looking forward to the year ahead and are hopeful that we will continue to make strides in curing Hepatitis C for many of our patients and improve the lives of those benefit‐ ing from the gift of transplantation. In the upcoming year, the CLDT hopes to move forward with accomplishing some of our educa‐ tion objectives. For instance, we are in the early stages of planning a fatty liver disease health fair, more community outreach in treating Hepatitis C, and improving online educational modules and assessment tools to better serve our patients and families. To do this, we will be looking into fundraising opportunities. If any‐ one has any great fundraising ideas or are aware of any potential venues (bowling alleys, golf courses, etc), please forward your ideas to [email protected] . We are often asked by patients and family members whether there are options for directly donating to the Center for Liver Disease and Transplantation. If you go to our website at www.livermd.org , you will see a tab “Ways to Give” on the left hand side. When you click on this tab, you will be directed to a page outlining different options. You may also contact Dr. Sil‐ via Hafliger to explore ways to support patients and families struggling with costs associated with transplant or direct patient education. We hope you all have a wonderful, healthy year. We will have our annual organ donation ta‐ bling event April 6‐10th when we hope to sign‐ up new organ donors. The Liver Walk NYC is scheduled for June 7th in Clinton Cove. Our educational workshops began at the Columbia campus on Thursday, January 22 nd at 4:30pm. Please note the new time and location. Successfulorgantransplantationonlybecamepossiblewiththeintroductionin1984ofmedicationsableto"tame"ourimmunesystem.Themomentyournewliver,whichcomeswithdifferentDNAandproteins,isplacedintoyourbody,yourbodywageswaronyourimmunesystem.Weprescribemedicationstopre‐ventthecellsofourimmunesystemfromcausingdamage.
Theimmunesystemisahighlyorganizedteamofcellsprotectingourbodiesagainstbacteria,viruses,fungusandfor‐eignproteins.Youhavemanyteamshelp‐ingyouduringyourjourneyoflivingalongandhealthylife:yourhometeam(youandyourcaregivers),themedical,psychosocialandsurgicalteamandtheimmunesystem.Asyouknow,communi‐cationamongteammembersisessentialforagoodoutcome.
Medicationstopreventrejection,alsocalledimmunosuppressants,alterandsilencesomeofthecommunicationsig‐nalsbetweenyourimmunesystem’scells.Theimmunesystem’sdefenseteamhastwobroadcategories:agroundarmyofsoldiersstationedattheborderofyourskinandmucousmembranes(calledtheinnateimmunesystem)andmorespecial‐izedsoldiers,thenavysealsandmarinesthatcanbecalledintoassist(theadap‐tiveimmunesystem).Thegroundarmy(innateimmunesystem)hasbloodcellsthatarecapableofeating,punchingholesand,thereby,killingbacteria,virusesandsomecancercells.Macrophages(sentinelcells),neutrophils(footsoldiers)andnaturalkillercellsaresomeofthemaincellsoftheinnatesystem.Thesecellsarealwaysreadytodefendus.
Theadaptiveimmunesystem(thespe‐cializedsoldiers)containswhitebloodcellscalledlymphocytes.BLymphocytesarecellssecretingantibodiesthatcoatbacteria,virusesororganicmaterialtag‐gingthemfordestruction.Tlymphocytes(Thelpercells,CD4andTkillercellsCD8)assistinkillingvirusesinsidecellsandrecognizeforeignproteinpresentedtothem(proteinfragmentsformyourdonorliver).Theadaptivesystemtakestimeto
mature,hasmemoryandisthesystemmanipulatedbyimmunosuppressivemedicationslikePrograf,Neoral,MyforticandPrednisone.
PrografandNeoralinterferewiththeproductionofasignalingproteincalledInterleukin2.Interleukin2islikeacellphone.ItisusedbyTlymphocytestoalert,activateandtraintheadaptiveim‐munesystem.Ifthereisnocommunica‐tion,thereisnoproductionandexpan‐sionofthesespeciallymphocytes.
PrografandNeoralneedtobetakenevery12hrstoeffectivelyinterferewithcellphoneuse.IfyouonlytakethesemedicationinthemorningandroutinelyforgettheeveningdoseyougiveyourbodyachancetoproducesignalstowakeupthespecialTlymphocytes.Itisalsoimportantthatthedoseofmedicationissufficienttointerferewithcellsignals.
Prograflevelsinthefirstmonthsaftertransplantare7‐10,Neorallevels100‐200.However,yourmedicalteamwilladjustthedosetoyourbodyandyourunderlyingillness.Myforticpreventsthebuildingandassemblyoflymphocytes.Myforticdoesnotdependonadruglevel.Prednisoneinterfereswithboththeinnateimmunesystem(reducingthefunctionofneutrophilsandmacrophages)andadap‐tiveimmunesystem(reducinginflamma‐torysignals).Prednisonebroadlyaffectsalllevelsofimmuneresponsiveness.
Rejection,ortheprocessofattackingyournewliver,hastwotimecourses:acuteandchronicrejection.Acutecellularre‐jectionoccursinthefirstseveralweekstomonthsaftertransplantandisseeninabout25%ofallpatients.Acutecellularrejectiondoesnotgenerallyleadtolossoftheorgan.Acuterejectionisusuallyas‐ymptomaticandonlydetectedbyvigilantbloodtesting,dailyinthehospital,weeklyandthenmonthlyafterdischarge.
ElevationintheASTandALTliveren‐zymefunctiontestsindicateslivercelldeath.Ifthereispersistentelevationintheseliverenzymes,youwillbemoni‐toredwithaliverbiopsy.Theresultsofthebiopsywilldeterminedegreeofrejec‐
tion(inflammation)andismeasuredbyaBanffscore.ThehighertheBanffnumbertheworsetheinflammationorinjury.Thetreatmentofacuterejectionisusuallyashortcourseofintravenousprednisoneandincreaseinyourbaselineimmunosup‐pression.
Chronicrejectionusually,butnotalways,happensafterthefirstyearandisseeninabout5%ofpatients.Itisusuallyaresultof“forgetting"ornottakingyourmedica‐tionasprescribed.Theliverenzymetests,ASTandALTareelevatedinchronicrejec‐tionbutlesssothaninacuterejectionbuttotalbilirubinandalkalinephosphataselevelswillbemuchhigher.Onliverbiopsy,weseelossofbileducts,called"thevanish‐ingbileductsyndrome."Chronicrejection,likeacuterejection,isoftenasymptomatic.Thereisoftentoomuchdamagetoyourliverwhenyouareabletoseethesymp‐toms.Forinstance,whenyoureyesturnyellowmanybileductshavebeende‐stroyedandcannotregenerate.
Generally,acuterejectioncanbewelltreatedbyaddingandadjustingim‐munusuppressionanddoesnotcauseper‐manentdamagetotheliver.Unfortunately,chronicrejectionisdifficulttotreatandoftenleadstocirrhosisanddeathinmanypatients.Itisimportanttoknowthatchronicrejectiondoesnotjusthappen.Chronicrejectiondoesnothappen"outofnowhere."Ifyouhaveyourbloodtestsregularly,communicatewithyourmedicalandpsychosocialteam,anddon'trepeat‐edlyforgetorrunoutofyourmedication,thisfearedcomplicationdoesnothavetohappen.Ifyouhavecirrhosisasaresultofchronicrejectionduetonottakingyourmedicationornon‐compliance,asecondlivertransplantislikelynotanoptionatourcenter.

SedebetomarPrografyNeoralcada12horasparainterferirenformaefectivaenelusodelteléfonocelular.Sisolotomaestosmedicamentosporlamañanayseolvidaenformarutinariadeladosisdelatarde,ledaalcuerpolaoportunidaddeproducirseñalesquedespiertenloslinfocitosTespe‐ciales.Tambiénesimportantequeladosisdemedicamentoseasuficienteparainter‐ferirenlasseñalesdelascélulas.
LosnivelesdePrografdurantelosprimerosmesesdespuésdeltrasplantesonde7a10,losnivelesdeNeoral,de100a200.Sinem‐bargo,suequipomédicoajustaráladosisasucuerpoysuenfermedadsubyacente.ElMyforticevitalaconstrucciónyelensambledelinfocitos.ElMyforticnodependedeunniveldefármaco.Laprednisonainterfieretantoenelsistemainmunitarioinnato(reduciendolafuncióndelosneutrófilosymacrófagos)comoenelsistemainmunitarioadaptativo(reduciendoseñalesinflamato‐rias).Laprednisonaafectaampliamentetodoslosnivelesderespuestainmunitario.
Elrechazooprocesodeatacarelnuevohígadotienedosetapas:rechazoagudoycrónico.Elrechazocelularagudosepro‐duceenlasprimerassemanasamesesdespuésdeltrasplanteyseveenalrededordel25%delospacientes.Elrechazocelularagudo,porlogeneral,nollevaalapérdidadelórgano.Elrechazoagudoesnormal‐menteasintomáticoysolosedetectamedi‐anteanálisisdesangredemonitoreo,diaria‐menteenelhospital,semanalmenteyluegomensualmentedespuésdelalta.
UnaelevaciónenlosanálisisdefuncióndelasenzimashepáticasASTyALTindicalamuertedelascélulashepáticas.Sihayunaelevaciónconstantedeestasenzimashepáticas,selerealizaráunabiopsiadehígadoparamonitorearlo.Losresultadosdelabiopsiadeterminaránelgradoderechazo(inflamación),elcualsemidemedianteelpuntajedeBanff.Cuantomayoresel
númeroBanff,peoreslainflamaciónole‐sión.Porlogeneral,eltratamientodelre‐chazoagudoesunaadministracióncortadeprednisonaporvíaintravenosayunau‐mentoenlainmunosupresiónbasal.
Porlogeneral,aunquenosiempre,elre‐chazocrónicoseproducedespuésdelprimerañoyseveenalrededordel5%delospa‐cientes.Normalmente,eselresultadode“olvidar”onotomarsusmedicamentoscomoseleindicó.LosanálisisdeenzimashepáticasASTyALTsonelevadosenelre‐chazocrónicoperomenoresencomparaciónconelrechazoagudo,aunquelosnivelestotalesdebilirrubinaydefosfatasaalcalinasonmuchomayores.Enunabiopsiadehígado,vemosunapérdidadeconductosbiliares,loquesedenomina“síndromededesaparicióndeconductosbiliares”.Elre‐chazocrónico,comoelrechazoagudo,esamenudoasintomático.Amenudohaymuchodañoenelhígadoparacuandosepuedenverlossíntomas.Porejemplo,cuandoseponenamarilloslosojos,muchosconductosbiliaressehandestruidoynopuedenregenerarse.

medicine.Mycuriositywasbeingsatis‐fied,myknowledgewasgrowingandIwasnowdevelopingarespectforthosewhocuredthediseased,comfortedthesickandwhoperformlife‐savingopera‐tions.Thatadmirationgrowsbytheday.IoftenfeeldwarfedwhenIaminthepres‐enceofthestaffatTheCenterForLiverDiseaseandTransplantation.Howfortu‐
nateIhaveexperiencedthisfeelingofawe.
Onequieteveningthatphonecallcameinformingmethattherewasaliveravail‐able.MywifeandIwererelaxed,incon‐trolandabitanxioustohavetheopera‐tion,recoverquicklyandreturntoanor‐mallife.Theoperationandquickrecov‐erycameoffwithoutmuchfanfare.Wewerebothsurprisedandfeltveryblessed.ThelovingeffortsofmycaregiverbroughtmetoaplacewhereIthoughtIwouldpickuponmylifeasbefore.HowfortunatethatIwasonceagaininvolvedwithlovingfamily,friendsandenjoyingmyfivegrandchildren.InsideIstruggledwithfeelingsthatIsomehowhadchangedbutcouldnotreallyidentifythisunknownME.Iwasexperiencingmoresensitivity,aneedtoshowmyoverwhelmingapprecia‐tionandstrugglingwiththerealitythatsomeonediedinorderformetolive.HowdoIthankmydonorfamilyandhonormydonor?Iwrotemy“ThankYou”lettertothedonorfamilywithinonemonth.

Idon'tknowtherightwordstosumupmystory.Icanonlyreflectonthedivineinterventionthatsavedmylifeandpermittedmetogrowandreflectonwhathasbeensomeofmybestyears.Ilookforwardtocelebrating50yearsofmarriagetomywifeandcaregiverMary.IcannowmarvelatthegrowthanddevelopmentofmyfivegrandchildrenwhoweretoddlerswhenIfirstgotsickandarenowincollegeandhighschool.Thedesiretowatchthemgrowwasamajorfactorinmyrecoveryandinkeepingmyselfhealthy.Ofcourse,TheCenterforLiverDiseaseandTransplantationwherethosetalentedhands,brilliantmindsandfiercelydedi‐catedhumanshaveproventhat"AmazingThingsAreHappeningHere."
