Data Quality Issues Around New Omission Question

•August collection much better than July in terms of data quality.

•Still a few data quality issues around the new collection form.

Example

Patient A, was prescribed an opioid on his admission to hospital. However there is now a valid clinical reason why staff have omitted to administer it. Patient A was also prescribed ‘other medication’, but as he was absent for the meds round this has also been noted as an omission.
