No guaranteeThe project team working on the construction of one of our offshore platforms, Dry Docks World - Dubai (DDW-D), Petrofac and TenneT, had prior to the fatal incident, as described on the front page, achieved an excellent safety performance with 4 million man-hours without a lost work case. This shows that it is possible to achieve a good Safety performance, even in a high risk environment like a shipyard, in a different cultural environ-ment working with many different nationalities. However the fatal incident shows that there is no guarantee for full protection from high impact cases. As it is sadly proved in Dubai.

Good HousekeepingTenneT is making preparations to draw extra attention in the second half of this year to the subject of “Good Housekeeping”. A major cause of building site accidents and incidents is the lack of an orderly and tidy workplace, even though it is often relatively easy to pay attention to these matters. Our Safety Vision 2018 document explains that a good safety performance stems from operational excellence. A tidy building site is part of this effort. The subject of good housekeeping will be integrated in workplace inspections and will receive extra attention during the safety walks that our managers perform. The outcomes of a number of themed toolbox meetings will be published. By talking to each other constructively about good housekeeping, we hope to avoid accidents in this field.

Safety Instructions soon also in GermanyFollowing the Safety Instructions is mandatory for visitors to and employees at high-voltage stations in the Netherlands and for all those visiting or working on power lines and construction sites. This year, the Safety Instructions will also be introduced at Ger-man TenneT sites.

Safety is important for TenneT. For this reason, our high-volt-age stations, power lines and construction sites are subject to specific rules with regard to access and the carrying-out of work. Where TenneT is responsible for safety, following the Safety In-structions is a prerequisite for being admitted.

Available in digital formThe Safety Instructions are available in the form of an e-learning course available on TenneT’s corporate website. The Safety In-structions explain TenneT’s safety and conduct rules and must be studied before going to the site. The instructional film is followed by a 10-question test. Participants who answer 8 questions cor-rectly will receive a certificate via e-mail. This is valid for 1 year. The certificate, or proof of participation, must be shown to the TenneT representative when entering the site.

New: A phone number for all your questions!A phone line has been set up (in the Netherlands) to improve the service of Veilig.tv and to deal with questions and complaints about the technical aspects of the digital Safety Instructions. The line is open on working days from 08:00 to 17:00 and the number to call is: +31 (0) 33 - 450 47 48. For all other questions, please send an e-mail to [email protected].

C-level meeting at TenneTAfter the summer, TenneT will host a high-level meeting with its key suppliers to discuss strategic themes related to our busi-ness. Board members and managing directors will be invited to discuss important developments in the energy market and the impact of those developments on TenneT. Safety will be an important topic on the agenda. More information will follow.

SCL workshops prove a big success!Morethan70suppliersofTenneTmetintheHDIArena(thehome ground of the Hannover 96 soccer club) in Hannover, Germany, at the end of May to learn more about the Safety Culture Ladder (SCL). These first German workshops were highly successful thanks to the large turnout by representatives of TenneT and suppliers.

The workshops are part of the program for introducing the Safety Culture Ladder in our supply chain. We are focusing on approximately 450 suppliers. The SCL is one of the elements of TenneT’s Safety by Contractor Management program aimed at enhancing the safety performance in our supply chain. We are asking suppliers to carry out a self-assessment or to have their safety culture level certificated by an independent body. Exchange of knowledgeThe meeting was structured like a workshop to provide an opportunity for an exchange of knowledge between certification bodies(Dekra,NCI,Kiwa,LloydsandDNVGL),safetyconsul-tants, TenneT and its suppliers. The workshop closed with an

SCL seminar given by Advisafe, a training agency specializing in the Safety Culture Ladder.

Positive feedbackThe suppliers gave positive feedback about the workshop. The remaining workshops in Germany will take place in July (and possibly after the summer as well). All workshops in the Nether-lands have already been held.

Available now: a handy tool for your self-assessment!Looking for a handy tool to perform self-assessments and analyses to identify how far safety awareness has been embedded and is developing at your company? The Netherlands Standardization In-stitute (NEN) has now made available an SAQ online facility in return for a small charge.

Our contractors are expected to perform a self-assessment for the certification schedule, an essential part of the Safety Culture Ladder. The certification schedule consists of numerous questions that correspond to the requirements for obtaining certification and forwhichpointsareawarded.Duringtheaudit,questionsareaskedin interviews. The answers are checked by the auditors against the certification schedule.

Using self-assessment to prepare for an auditThe self-assessment enables you to prepare for an audit. The auditor uses the outcomes to compile an audit plan. For all lev-els, the certification schedule comprises more than 200 ques-tions. Employees must answer multiple-choice questions, but it can sometimes be useful to add a comment to explain why you made a certain choice. There is one staff member who distrib-utes the questionnaire (self-assessment questionnaire = (SAQ)) among colleagues, monitors progress, analyses differences within the organization and, if necessary, changes the recipients of the SAQ.

Using the SAQ web tool you can easily copy an email list, select recipients and send out the survey. The system indicates the minimum number of surveys that must be sent out. The results are set out neatly against the requirements in the manual and you can see at a glance what improvements still need to be made in order to obtain certifica-tion. 3rd European TSO conferenceThe third European TSO (Transmission System Operator) SHE conference was held on Thursday, 8 June 2017. TenneT organ-ized the conference and it was held at the company’s head office at Arnhem in the Netherlands. Seven European TSOs (Elia, Transnet BW, 50Hertz, RTE, REN, Statnett and TenneT) attended the conference. The central theme was ‘How can we improve safety in our supply chain?’

The main subject of the conference was the Safety Culture Ladder (SCL). TenneT is pressing ahead on rolling out the Safety Culture Ladder and system among its suppliers. The conference discussed possibilities for other TSOs to adopt this system.

Time for actionThe European TSOs have been holding meetings since last year to exchange knowledge and experience in the field of safety, health and the environment (SHE). The time has now come to take things further and jointly tackle concrete issues. An exam-ple is a coordinated roll-out of the Safety Culture Ladder.

Accident investigations at VolkerInfra In the previous newsletter, we reported that TenneT wants to raise the quality of its accident investigations. A good investigation iden-tifies the direct and root causes of an accident. Measures can then be taken to prevent more accidents. TenneT uses the Tripod Beta method for this purpose. But what do our contractors do? TenneT visited VolkerInfra.

VolkerInfra is an important contractor of TenneT involved in projects including the construction of the Randstad 380 kV Northern Ring in the highly-urbanized western part of the Netherlands. Mar-tin Brouwer, Head of the Integral Safety department at VolkerInfra, explains how accidents are investigated within Volker-Wessels, of which VolkerInfra is part.

Last year, VolkerWessels decided to use Tripod Beta. This was expected to improve the quality of investigations. And that is precisely what happened. All serious acci-dents are investigated by the company or project where the accident occurred, with support from a Tripod Beta expert. Various safety experts have been trained specially for this purpose within VolkerWessels.

The Tripod Beta method forces investi-gators to work systematically, unravel the

events leading up to the accident and determine the breaches that occurred. Instead of blaming an employee, Tripod Beta searches for root causes that may explain why a mistake was made. These are factors at organizational level. This is precisely the strength of the method, because it allows prevention of similar accidents.

Martin resolutely answered no when asked whether Tripod Beta was an excessive investment in terms of time, money and resources. The results achieved have been so much better than with any other meth-od that this investment pays for itself, he says. It is because of the greater effective-ness of the measures.

Frequently heard criticisms of Tripod Beta are that the method is very complex and that the investigation takes a long time.

Martin waves away such criticisms. A small number of people at VolkerWessels have been trained in use of the meth-od. By keeping the number small, each person gets plenty of opportunities to keep his or her knowledge and skills up to the required standard. Consequently, the investigation team can quickly start and complete a Tripod Beta investigation.

The results of every Tripod Beta investi-gation are always presented in-house at VolkerWessels at a meeting with the man-agement responsible. Since Tripod Beta was first used, it has been a lot easier to

determine the measures that need to be taken. The conclusions are logical and transparent, so the measures follow on from them automatically.

Martin has no hesitation in recommending Tripod Beta to everybody. Asked wheth-er Tripod Beta was an excessive tool for looking into accidents with an apparently obvious cause, he said that in his experi-ence it was possible even in those cases to identify the deeper causes by means of Tripod Beta. They are causes that would not otherwise have come to the surface. The same goes for cases where an em-ployee failed to observe a very basic rule, one you might expect everybody to be familiar with.

Another aspect with which Martin has positive experience is the performance of an accident investigation in cooperation with the client. As a contractor or subcon-tractor, you are always in a situation where you work alongside other parties. Tripod Beta shows that accident causes are not always attributable to just one of the par-ties. Therefore, it makes sense according to Martin to conduct a joint investigation. This is an invitation that TenneT will be pleased to accept if the need arises.

Safety Moments: learning together from incidentsUnfortunately, there have been a number of serious incidents at TenneT despite all our good intentions and initiatives concerning safety.Detailsofafewofthoseincidentsareprovidedbelowandtheyhavealsobeendistributedina‘SafetyMoment’.

The purpose of Safety Moments is to share with each other in plain language information about incidents so that we can all learn from them. We make no distinction between office and field staff, or between TenneT and contractors. This is because we believe it is wise for everybody to take time to reflect: it will make us doubly aware of why safety is so important. Conversely, we also received quite a few Safety Moments and Safety Alerts from you that we have distributed in our organization.

Fatal accident with a sand lorryAn incident in the Netherlands on 3 March 27 showed us how things can unexpectedly go tragically wrong. At the exit of a building site, a cyclist and a lorry carrying sand collided while crossing a cycle path on the public highway. The cyclist, an elder-ly man riding an electric bike, died at the scene of the accident. Eyewitnesses reported that the cyclist and the lorry had both slo-wed down immediately before the collision. The contractor con-ducted an extensive Tripod Beta investigation. Shortly before the collision, the cyclist was in a blind spot of the sand lorry. It was unknown why the cyclist did not stop for the sand lorry. Further investigation into the traffic situation showed that it satisfied the prevailing requirements (CROW96b). The investigation yielded several measures that were discussed with parties including local

neighbourhood councils. Among other things all drivers have now received special instructions about traffic situations while driving into and out of building sites. The contractor has launched a local campaign under the banner of ‘If you see me, I can see you’ to stress the importance of road users making eye contact with vehicles drivers. A number of signs at the place of the accident have been made more prominently visible. The risk analyses of entrances and exits will be evaluated in the interim.

Things went wrong in this instance even though everything oc-curred in conformity with the law and established practices. Buil-ding site entrances and exits continue to pose a heightened risk of traffic accidents even when they are situated along relatively

quiet roads. This incident demonstrates that we cannot suffice with a routine assessment of these traffic situations.

High-risk incident with a craneA high-risk incident (HRI) occurred during the erection of a tensi-on pylon in the Netherlands on 14 February 2017. While the pylon was being put into place, the hoisting sling got stuck. To free it, an employee of the contractor asked was lifted by his safety har-ness by a crane to release the sling. Tension pylons are transpor-ted horizontally, raised by crane using a long hoisting strap and placed on their foundations. After being put in place, the crane hook is lowered and the hoisting belt can be unfastened. Subse-quently, the work cage is connected up so that the employee can be lifted up to the tension pylon to remove the sling. This working method was not followed. Besides breaking the law, this created unacceptable risks. If a person fails to use the correct safety harness, there is even the danger of harness suspension trauma due to the accumulation of waste substances in the blood (as the blood circulation to the legs will be interrupted). The contractor carried out a detailed Tripod Beta investigation and uncover-ed several causes. One of the main causes was time pressure, because work had been delayed due to ice on a concrete balan-cing block and the use of a slower-than-planned crane. Therefo-re, there had been a deviation from the work plan at the building site, without consulting TenneT. Insufficient thought was given to the risks that arise from a changed working method (necessita-ting a thorough risk analysis as part of Management of Change).

There was no approved SHE plan or job risk analysis for the new working method and unfortunately the employee did not perform a Last Minute Risk Analysis (LMRA). High-risk incident during offshore trainingDuringanoffshoretrainingsessionon2December2016atanindoor training centre, two TenneT employees were lifted from the water by crane as part of an exercise. At a height of 3.5 me-tres above the water, the line unexpectedly broke and caused both employees to fall into the water. Fortunately, they did not suffer any serious injuries. An extensive Tripod Beta analysis identified multiple causes of the incident. A direct cause was that the crane did not automatically stop during hoisting when the sling touched the pulley of the crane. The person operating the crane probably continued hoisting unaware of what had happened, which caused the pulling force on the line to become so great that it snapped. Underlying causes identified were the absence of a safety device against overloading and the absen-ce of a second person to provide support during operation of the crane. It also emerged that the crane had not been inspec-ted and should not have been used to carry people. After this incident TenneT prohibited its employees from using the training centre. A Safety Alert was sent out for this purpose. However, the training centre has in the meantime installed a new crane that is suitable for carrying people.

‘We’re never finished with safety’In some respects, the relationship between suppliers and TenneT is very close. Take, for example, the Facility Management department in the Netherlands that is responsible for safety at the company’s office buildings and surrounding areas. To a large extent, this service has been outsourced. Supplier ArrangeGroup is the face of this department for internal TenneT staffers. Supply Manager Andrea van der Meer of ArrangeGroup and TenneT’s acting Facility Manager Suzanne ten Hove-Emmers explain how safety is inter-woven in their day-to-day cooperation.

‘Responsibility for the operational and much of the tactical service provisioning has been outsourced to ArrangeGroup’, says Suzanne. ‘They then issue instruc-tions to suppliers for such matters as cle-aning, catering, maintenance of greenery, logistics and events. Besides hospitality and floor management, our focus is on safety. TenneT employees must be able to work in a pleasant and above all safe working environment’.

Andrea of ArrangeGroup: ‘Safety is not just part and parcel of our contract with TenneT and the close cooperation with the safety department, but also of our way of working. You could say we are the embodiment of this philosophy because our employees are the first point of con-tact at TenneT when it comes to facility

services and safety. Any reports related to buildings and sites are handled with high priority. We must set a good examp-le in our cooperation with the other chain partners, such as Vitam (catering & hot beverages) and CSU (cleaning & window washing). We do this by making arran-gements about safe working practices, agreeing safety targets with each other, ensuring that safety is discussed at team and supplier meetings and agreeing per-formance arrangements with our staff. At TenneT and ArrangeGroup, every emplo-yee has an extra responsibility when it comes to safety’.

Asked how she views cooperation in the light of the Safety Culture Ladder, Andrea said: ‘Wherever possible we work on im-plementation of the Safety Culture Ladder

together with the supply chain partners. We share information and also give feedback to TenneT on how we intend to implement the Safety Culture Ladder and the challenges that we encounter in that regard. The Safety Culture Ladder is a good initiative, but at the same time it’s also a search, particularly for small companies, to discover how it affects them in terms of the time spent on it and the impact on the services they provide. Together, we examine how we can enter into a dialogue with TenneT on questions of this kind. So do it not only for TenneT but also with TenneT. We stress that we do this in addition to all the other efforts that we make for safety.’

Safety is always high on the agenda at Facility Management. Suzanne. ‘The sa-fety of our head office was meticulously scrutinized.Duringtherelocationtothenew building in 2013, employees looked

very carefully at all kinds of safety mat-ters. They examined everything from door handles to power outlets – we received reports on just about everything. That’s good. We take every report seriously. The only way to create a safe working environment is by actively flagging up matters and reporting them. We’re never finished with safety’.

bedoeld om extra aandacht voor het thema Good Housekeeping

te vragen tijdens de thema maanden. Naast dit formulier zijn ook

een speciale editie van het Safety Walk formulier en een thema

Toolboxmeeting beschikbaar, beide met als thema ‘Good

Housekeeping’.

Procedure werkplekinspecties

Het periodiek uitvoeren van werkplekinspecties is een

‘must vraag’ vanuit de VCA norm. Zowel GSN als LPN zijn

VCA gecertificeerd. Het proces is alleen van toepassing op

deze twee organisatie-eenheden van TenneT.

De frequentie van uitvoering van werkplekinspecties is

afhankelijk van de functie. De wijze en frequentie van

uitvoering van de werkplekinspectie is beschreven in de

procedure Werkplekinspecties in het proceshuis.

Het is belangrijk om in acht te nemen dat werkplekinspecties

in de betreffende maanden moeten plaatsvinden. Dat betekent

dat werkplekinspecties niet met terugwerkende kracht op

een andere maand kunnen worden geregistreerd. Ook is het

niet mogelijk vooraf al werkplekinspecties voor toekomstige

maanden te doen. Voor een adequate registratie en monitoring

is het belangrijk om de ingevulde formulieren zo snel mogelijk na

de werkplekinspectie te versturen, zodat deze tijdig kunnen

worden geregistreerd.
