The Informal Assessment The informal assessment is not just a task to be completed; it is a tool to help the Case Manager understand the details, strengths, weaknesses and needs of the family. The information collected and documented is not just to assist the Case Manager, but it will also help the customer and other W-2 agency staff.
Case Manager:
• Individual's ability to become employed and remain employed; • Identify strengths and resources of the applicant/participant • Services and activities necessary for the individual to become employed and
remain employed; (such as transportation and child care) • Determine appropriateness for Up-Front Job Search activity assignment • Appropriate placement and activities; • Need for further career assessment and planning; • Existence of potential disabilities • Need for a formal assessment of any disabilities or other employment barriers
Applicant/Participant:
• Identify strengths and resources • Identify potential needs and supportive services necessary • Identify employment goals
Other W-2 Staff:
• Make appropriate job referrals • Develop a strength based resume
