Section 1: Welcome from the Chief Executive Welcome to Warrington and Halton Hospitals and thank you for showing an interest in coming to work

Trust Profile Warrington and Halton Hospitals NHS Foundation Trust provides services to an area of approximately 100 square miles with a population of 309,295 (Halton 118,215 and Warrington 191,080) within Cheshire and Merseyside on two hospital sites. There are multi-disciplinary Post-graduate Centres on both sites, to which all staff have access. A library with internet access, lecture hall, various meeting and study rooms and a dining area are also available at the Warrington site. Other initiatives to benefit staff include flexible working hours, childcare vouchers, fast track staff physiotherapy access, onsite fitness and relaxation classes, monthly health and wellbeing initiatives, staff counselling service, family friendly policies and shopping discounts. Warrington Hospital (578 beds & 2,979 staff)

Warrington Hospital focuses on emergency and specialist care and has all the back-up services required to treat patients with a range of complex medical and surgical conditions. Many new departments and facilities have opened at the hospital over the last few years and it provides a full range of expert inpatient and outpatient services.

Warrington Hospital is home to our accident and emergency department and maternity services. If surgery or care might require extra support or a high level of specialist care it is likely to be carried out at Warrington. The hospital is also home to specialist critical care, stroke, cardiac and surgical units.

Services provided include – Emergency Care, Critical Care, General Medicine, General Surgery, Orthopaedics, Urology, ENT, Ophthalmology, Paediatrics, Elderly Care, Obstetrics and Gynaecology.

Halton Hospital (42 beds & 987 staff)

A range of care for medical and surgical conditions is provided at Halton General Hospital

in Runcorn and it houses a mix of inpatient and outpatient services. It provides a fantastic, friendly environment for expert surgical care.

If surgery is non-complex and does not require a long hospital stay it is likely to be carried out at Halton Hospital. There are low operation cancellation rates at the hospital as routine surgery is not as threatened by emergency work. The hospital is home to an Urgent Care Centre which provides a range of minor emergency care services for local people and the hospital provides x-ray facilities. A step down ward at the hospital is designed for patients who have had surgery or emergency medical care but who require some further support before going home. We provide chemotherapy services on site and the hospital is home to the Delamere Macmillan Unit which provides cancer support and advice.

The site is also home to our orthopaedic facility - the Cheshire and Merseyside NHS Treatment Centre - a standalone operating and clinical facility for orthopaedic surgery services across our hospitals.

Services provided include Elective Surgery and Urology, Rehabilitation, Outpatients, Urgent Care Centre (UCC), Programmed Investigations Unit

Trust Organisational Structure

The Medical Director is supported by a Medical Cabinet, comprising a Deputy Medical Director, Associate Medical Directors and Clinical Directors. The Trust operational structure is defined by 8 Clinical Business Units (CBUs); the Respiratory service sits in the Medical Care CBU. Each CBU has a Triumvirate leadership team of a Clinical Director, a CBU Manager and a Lead Nurse, who work closely with the Speciality Leads, Matrons and Ward Managers.

Section 3: The Services Other Specialties/Services relevant to Acute Medicine Aside from specialty wards mentioned:

There is an 8 bedded CCU

There is a dedicated Endoscopy Unit.

Inpatient psychiatry is offsite, but there is a mental health liaison service situated within the ambulatory care area. This is provided by the Brooker Centre at Halton & Hollins Park at Warrington.

Specialty nurses are readily available: in cardiology, diabetes, respiratory, elderly care, rheumatology & palliative care

There are close working relations with Emergency Care.

A cardiac catheter suite is on site & pacing is done locally.

There are visiting Neurologists from the Walton Centre.

A renal physician visits weekly & is available for case discussion.

Intravenous antibiotic therapy is available in the community.

DVTs are managed on an outpatient basis according to protocol.

Critical Care Unit There is an 18 bedded Critical care Unit with 8 Level 3 beds and 10 Level 2 beds. The unit provides two Consultants each weekday between 08:00 and 17:30 and single handed Consultant cover 17:30 to 20:00 weekdays and weekend. The unit is supported by a team of 8 anaesthetic Speciality Doctors 24/7 and junior and trainee anaesthetists. There is also a program to develop Advanced Critical care Practitioners (ACCP’s) with three staff at present due to qualify in 2020.

Organ Donation Service The unit is an active participant in the Organ Donation service with a Consultant Lead and dedicated network Organ Donation lead Nurse. Diagnostic Radiology Warrington has two CT scanners, MR and Nuclear Medicine. CT & MR are also available in a dedicated orthopaedic unit attached to the Halton site. The chest radiologists support a weekly chest radiology meeting (Thurs), a weekly (Mon) pre-MDT meeting for patients on CFT pathway and the actual lung cancer MDT meeting (Wed) for diagnosed cancers. PACS has recently been upgraded with imaging access across most of the Mersey Region. There is diagnostic access to MR 7 days a week and CT 24/7; we are part of the Mersey Radiology on-call service promoting collaborative working across the region. Theatres Warrington has 7 Theatres in the main theatre block and two theatres in maternity and Halton houses 4 theatres in Halton Hospital and 4 Theatres in CMTC (Orthopaedic Treatment Centre). There is 24/7 Anaesthetic and Theatre cover. Acute Care Team There is a 12 strong team of Acute Care nurses who provide a critical care outreach service and provide the Night Practitioner service out of hours. The service is also developing Advanced care Practitioners with a trainee due to qualify in 2020.

Section 4: The Team Come and join the Medical Care Team, the Trust has just received its 2019 CQC Ratings and achieved the rating of Good across all Medical Care Services and an Outstanding for Caring in Critical Care We have been on a journey of quality improvement over the last two years as demonstrated by the improvement ratings below but this is just the start! We are developing plans across all of your services with the intention of achieving Outstanding ratings across all domains when we are next inspected.

Section 5: Job Description

Title: Consultant Anaesthetist with sub-specialty interest in Intensive Care Responsible to: Clinical Business Unit Manager Accountable to: Clinical Director Duties of the Post Warrington & Halton Hospitals NHS Foundation Trust wishes to appoint a full time Consultant Anesthetist. The person appointed will be based at the Warrington hospital site This is a full time post in Critical Care amounting to 10 Pas; following induction additional PA’s are paid for participation in the On-call which is a core component of the post. Applicants are required to hold MB ChB or equivalent qualifications, to be fully registered with the General Medical Council and listed on the Specialist Register or due to be registered within six months of the date of appointment. Responsibilities of the Post The post holder will join a team of 7 Consultant Intensivists to provide 7 day cover to the Critical Care Unit; at present weekend cover is split meaning Consultants work a 1 in 4 rota. Along with the existing Consultants the post holders will contribute to the delivery of twice daily consultant led ward rounds on the Critical care ward and Critical care Outreach. Anaesthetics sessions are allocated by the Anaesthetics department. Following induction, the post holder will be expected to join the ITU on-call with a commitment of 1: 8 for weeknights and weekends. This is remunerated in addition to the standard 10 PA’s; the intensity of this rota will be reviewed as the number of consultants increase. (CAT A); Service Development The Intensive Care unit have recently been awarded “Outstanding” for Caring by the CQC and “Good” in all other domains; the service is working hard to achieve Outstanding across all domains and new appointees are encouraged to actively participate in the quality improvement agenda. Service Objectives for the New Post Holder Support the development of the Advanced Critical Care practitioner role on ICU, providing clinical leadership and development Actively participate in the quality improvement agenda adopting an initiative of choice to lead on Provide active teaching and supervision to anaesthetics and critical care trainees both on the ICU and during theatre sessions

Appraisal & Mentoring This will be conducted annually to support the clinician in his/her current work duties and incorporating methodical planning for the future with support to ensure the consultant meets revalidation. Supporting Staff The ICU team are supported by a team secretary, an audit clerk and an Assistant CBU manager. It is expected that the appointee will, with colleagues, undertake general professional supervision and management of Junior Medical staff, undertaking training in Anaesthetics and Critical Care as well as support supervision of medical students passing through the department. Research The research and audit interests of the successful applicant will be encouraged. Educational Activities and Training The Trust supports the requirements for Continuing Medical Education as laid down by the Royal College of Anaesthetists and is committed to providing time and financial support for these activities. Personalised developmental budgets can be discussed & supported for developing specific specialty and/ or leadership capabilities of successful candidates. Clinical and seminar teaching for Junior staff preparing for the MRCP examinations is organised through the Postgraduate Medical Centre and all Consultants are invited to contribute to this teaching. The trust holds mock PACES courses and the appointees will be encouraged to contribute towards that as well. There is established local departmental weekly teaching on Tuesday lunchtime and the appointee shall be encouraged to participate in delivering that. The appointee will share responsibility for educational and clinical supervision of junior medical staff with other physicians. There are regular attachments of 4th year Medical Students to the wards/clinics, with 5th years often involved in shadowing F1s and SAMP attachments. The appointee will be encouraged to support these medical students in both teaching and appropriate supervision. Audit / Clinical Governance The department has an active interest in clinical audit including contributing data for national audits and NCEPOD surveys. All medical staff are expected to participate in Clinical Audit. Regular audit meetings are organised both within the department and within the larger CBU structure, and the appointees will be expected to participate. Fully informed and documented consent will be undertaken for procedures and body tissue samples.

Section 7: Example Job Plan The job plan will be a prospective agreement that sets out a consultant’s duties, responsibilities and objectives for the coming year. It should cover all aspects of a consultant’s professional practice including clinical work, teaching, research, education and managerial responsibilities. It should provide a clear schedule of commitments, both internal and external. In addition, it should include personal objectives, including details of their link to wider service objectives, and details of the support required by the consultant to fulfill the job plan and the objectives. A formal job plan will be agreed between the appointee, the CBU Manager and Clinical Director, with final agreement by the Deputy Medical Director. It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Medical Director or Clinical Director & adjusted accordingly to the agreement of both parties. Provisional Timetable The following provides scheduling details of clinical activity and clinically related activity components of a provisional example job plan. This is liable to change depending upon the needs of the department and division and interests of prospective appointees. Agreement should be reached between the appointee and the Lead Clinician with regard to the scheduling of all activities, including the Supporting Professional Activities.

Section 8: Terms and Conditions of Service The appointment is subject to the Terms and Conditions – Consultants (England) 2003. 1 General Provisions 1. This post is subject to the Terms and Condition of Service for Hospital Medical and Dental staff

(England and Wales) and will be offered on a whole time or maximum part-time basis. 2. The candidate must be registered with the General Medical Council with the appropriate Specialist

Registration 3. The doctors are covered by the Crown Indemnity Insurance but candidates are advised to subscribe

to one of the defence organisations. 4. It is a non-resident post but accommodation for single or married persons may be available on

request. 5. The successful candidate will be expected to work with local managers and professional colleagues

in the efficient running of services and will share with consultant colleagues in the medical contribution to management. Subject to the provision of the Terms and Conditions of Service, he/she is expected to observe the Trust’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instructions of the Trust. The successful candidate will be expected to follow the local and national employment and HR policies and procedures. He/she will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of their patients to be able to contact an appropriate Consultant when necessary.

6. All medical and dental staff are expected to comply with appropriate Trust Health and Safety policies.

7. The successful candidate will have responsibility for the training and supervision of junior medical staff that work with him/her and will devote time to this activity on a regular basis. If appropriate, he/she will be named in the contracts of doctors in training grades as the person responsible for overseeing their training and as a main source of advice to such doctors on their careers.

8. It is mandatory for the successful candidate to attend the Trust’s Corporate Induction Programme and ensure that mandatory training is up to date at all times.

2 Residence 1. The successful candidate will normally be required to live within 10 miles of his/her base hospital (the

hospital where the principal duties lie) and in a location offering easy access to the other hospitals as required.

2. Where, however, the successful candidate already resides within 15 miles by road of his/her base

hospital, he/she will not be required to remove his/her home nearer the hospitals. 3. Where the successful candidate’s present residence is more than 15 miles by road from the hospital

following discussion with the Trust he/she will be required to remove his/her home to meet the residential clause of his/her contract, unless he/she has the written consent of the Trust to the contrary. They must also ensure that their residence is within a reasonable commute from the hospital in order to carry out on call duties.

3 Disclosure & Barring Service (previously Criminal Records Bureau) The successful applicant will be required to undertake an enhanced DBS Disclosure check. Further information on the DBS is available on their website at http://www.dbs.gov.uk/ The Appointee will be required to pay for the DBS Enhanced Disclosure. 4 General Statement & Obligations

a) Duties of the Post The duties above indicate the post holder’s main responsibilities but are not exhaustive. They may be jointly reviewed with the post holder according to experience and to support service development. b) Business Continuity Plan The Trust is required to have robust emergency preparedness plans in place, to ensure the continuity of service in certain circumstances, e.g. major civilian disaster, business continuity incident. The Trust expects the post holder to be aware of their role in the event of such a significant disruption to Trust services. As a consequence of such circumstances the post holder may (within their scope of competence) be required to undertake a variation to their normal duties, normal base and / or hours of work as are reasonable and appropriate to maintain essential service provision. c) Policies & Procedures All employees must follow the Trust’s policies and procedures. These cover all areas of work, both clinical and non-clinical. Some important areas applying to all employees are highlighted below. Your induction will cover key post-related policies. 5 Safeguarding Children and Vulnerable Adults All employees of Warrington & Halton NHS Foundation Trust are required to act in such a way that at all times safeguards the health and wellbeing of children and vulnerable adults. Familiarisation with and adherence to the Trust’s Safeguarding policies is an essential requirement of all employees as is participation in related mandatory / statutory training. 6 Infection Control All staff have both duty and responsibility to prevent the spread of infection within the hospital. The post holder must:-

Take all reasonable care to minimise the likelihood of healthcare-associated infections occurring to patients, visitors and staff (including you) as a consequence of actions and/or omissions at work;

Co-operate with the Trust in ensuring that all statutory and other requirements are complied with – in particular, compliance with the Health Act 2006: Code of Practice for the prevention and control of healthcare – associated

To be knowledgeable and compliant with the Trust’s infection control policies and procedures, which includes attendance at mandatory training. 7 Health and Safety/Risk Management Employees must take reasonable care of the health, safety and welfare of themselves and others. Employees must report promptly any risks, including all incidents, near misses and hazards. All staff must be aware of their responsibilities under the Health and Safety at Work Act 1974.

8 Confidentiality of Information & Data Security Strict confidentiality rules apply to patients, colleagues and Trust business. Confidentiality for patients lasts indefinitely and continues even if you leave the employment of the Trust. Halton sits astride the River Mersey to the east of Liverpool and south of Manchester. Runcorn and

Sports Teams

Warrington is proudly home to the Warrington Wolves, a professional rugby league football club that successfully competes in Super League.

If football is more your thing, then you couldn’t have more choice on your doorstep if you tired. Four giant clubs in Manchester United, Liverpool, Manchester City and Everton are all within easy reach, not to mention a number of smaller clubs.

Transportation

Warrington and Halton are situated next to three major Motorways, M6, M62 and the M56 so driving anywhere isn’t a problem.

The area is blessed with fantastic public transport links around the areas and if you wish to travel further the train station is located 10 mins walk from Warrington Hospital with direct trains to Manchester and Liverpool passing through very regularly.

If that wasn’t enough, 30 minutes in either direction will take you to either Manchester or Liverpool Airport which support flights to all over the world.
