Cholera is an infection of the small intestine that is caused by the bacterium Vibrio cholerae.

The main symptoms are profuse watery diarrhea and vomiting

Transmission occurs primarily when drinking water or food is contaminated by the diarrhea from an infected person or by the feces of an infected but asymptomatic person.

The severity of the diarrhea and vomiting can lead to rapid dehydration and electrolyte imbalance.

Primary treatment is with oral rehydration solution (ORS) and if this is not tolerated, intravenous fluids. Antibiotics are beneficial in those with severe disease

Worldwide it affects 3–5 million people and causes 100,000–130,000 deaths

transmitted by the ingestion of food or water contaminated with the feces of an infected person,

bacterium Salmonella enterica bacteria then perforate through the intestinal wall gastric fever, abdominal typhus, infantile

remittant fever, s low fever, nervous fever , pythogenic fever, etc.

oral rehydration therapy is a way to prevent many of the deaths of diarrheal diseases in general

Typhoid fever in most cases is not fatal. Antibiotics have been used to treat typhoid fever Prompt treatment of the disease with antibiotics reduces

the case-fatality rate to approximately 1%. When untreated, typhoid fever persists for three weeks to

a month. Death occurs in between 10% and 30% of untreated

cases

S. Typhi Whole cell Whole cell heat-inactivated phenol-preserved typhoid vaccine given with tetanus toxoid or DEAE-dextran as adjuvants

Vi Field trial using locally produced Vi: 130,000 people enrolled and protective efficacy 69% after 18 months

Live 80 healthy volunteers given CVD908 htrA. Good seroconversion achievedNo immune responses to urease detected, despite boosting with urease plus heat labile toxin (LT)

Cholera Whole cell 15,000 people given two dose regimen gained 66% protectionWhole cell 14,000 children and adults. Vaccine efficacy up to 82% following three

doses. Only 4% after first doseLive Healthy volunteers given combined CVD103 and Ty21a vaccines.

Serum and mucosal antibody responses, good seroconversion after primary dose achieved

Live Live cholera vaccine administered to 67,000 people. No long-term protection seen

Live Thymidine auxotrophic strain of V. cholerae cholera toxin negative, haemagglutinin/protease negative

Campylobacter

Live, non-colonising

Intramuscular dose with or without adjuvant and with or without oral dose of non-colonising strain failed to give immunity to colonising strain

Vaccinations for water borne diseases costs about $1 per shot

'Nearly all polio outbreaks since 1961 were caused by the oral polio vaccine,'-Jonas Salk, inventor of the IPV, testifying before a Senate subcommittee

“You can fool some of the people all of the time and all of the people some of the time, but you can’t fool all the people all of the time.” Abe Lincoln

Vaccinated 1902-1903 London's case mortality 3 times unvaccinated Leicester, in 1904 mortality rate was 9 times higher

Longterm Study in Guinea-Bissau (1 Kristensen I, Aaby P, Jensen H.:“Routine vaccinations and child survival: follow up study in Guinea-Bissau, West Africa”, BMJ 2000; 321: 1435–41 The children of 15,000 mothers were observed from 1990 to 1996 for 5 years. Result: the death rate in vaccinated children against diphtheria, tetanus and whooping cough is twice as high as the unvaccinated children (10.5% versus 4.7%).

Studies compare Vaccinated with unvaccinated

Salzburger Study Results:of 1004 unvaccinated children, hadAsthma, 0% (8-12% in the normal population)A-topic dermatitis 1.2% (10-20% in the normal population)Allergies 3% (25% in the normal population)ADHD 0.79% (5-10%) in Vaccinated children

Water Filters

The method used in Bangladesh is as follows: an old sari is folded to make four or eight layers.

In the monsoon seasons, it may be advisable to use a cheap disinfectant to decontaminate the material.

Used cloth is more effective than new cloth, as the repeated washing reduces the space between the fibres

By passing the water through an effective filter, most cholera bacteria and other pathogens are removed

An old cotton sari, folded, creates a smaller effective mesh size (approximately 20-μm).

This should be small enough to remove all zooplankton, most phytoplankton, and thus a large proportion of the cholera in the water (99%, according to laboratory studies).

Effective sanitation practices, if instituted and adhered to in time, are usually sufficient to stop an epidemic.

Sterilization: Proper disposal and treatment of infected fecal waste water produced by cholera victims and all contaminated materials (e.g. clothing, bedding, etc.) is essential.

All materials that come in contact with cholera patients should be sterilized by washing in hot water, using chlorine bleach

Sewage: antibacterial treatment of general sewage by chlorine, ozone, ultraviolet light etc….

Water purification: All water used for drinking, washing, or cooking should be sterilized by either boiling, chlorination, ozone water treatment, ultraviolet light sterilization (e.g. by solar water disinfection), or antimicrobial filtration in any area where cholera may be present.

ancient Indian 2500 years with treatment of acute diarrhea with rice water, coconut juice, and carrot soup.

1831, William Brooke O'Shaughnessy prescribed intravenous fluid therapy (IV) to compensate.

The results were remarkable the mortality rate of cholera dropped from 70% to 40% with the use of hypertonic IV solutions.

IV fluid replacement became entrenched as the standard of care for moderate/severe dehydration for over a hundred years.

ORT Oral Rehydration Therapy

the Bangladesh Liberation War in 1971 As medical teams ran out of intravenous fluids to

treat the spreading cholera epidemic, Dr. Dilip Mahalanabis instructed his staff to

distribute oral rehydration salts (ORS) to the 350,000 people in refugee camps. Over 3,000 patients with cholera were treated,

and the death rate was only 3.6%, compared to the typical 30% seen in intravenous fluid therapy

Between 1980 and 2006, ORT decreased the number of worldwide deaths from 5 million a year to 3 million a year

Death from diarrhea was the leading cause of infant mortality in the developing world until ORT was introduced.

ORT Oral Rehydration Therapy

The World Health Organizations salted rice water, salted yogurt drink, and salted vegetable or chicken soup. one liter of plain water with 3 grams table salt and 18 grams common sugar can

also be made. What should be avoided are commercial sof t drinks,

commercial sweetened fruit juices, and cof fee

A basic oral rehydration therapy solution is composed of

30 ml (6 level tsp) of sugar 2.5 ml (1/2 level tsp) of salt, dissolved into 1 litre (4.25 Cups) of clean water

Disinfectant dissolved silver has been used as a water disinfecting agent for , the drinking water

supplies of the Russian Mir orbital station and the International Space Station World Health Organization recommends colloidal silver in water filters provide

safe drinking water in developing countries

US Food and Drug Administration has approved the use of a range of different silver-impregnated wound dressings.

silver-containing fibers provide a sustained release of silver ions when in contact with wound exudates, and are “highly effective against bacteria” have shown silver-containing alginate fibres provide a sustained release of silver ions when in contact with wound exudates, and are “highly effective against bacteria” Biochemical Materials Research and Development Center of J iaxing Col lege, China ,
