Working together…for healthy people in healthy communities. Supporting children where they live, learn and play: Child & Family Centred Care in an Integrated Services / Rural Outreach Model of Children’s Therapy

description

Sunrise Health Region’s Children’s Therapy Program (CTP) supports children where they live, learn and play. This session will describe CTP’s journey in the development of a community-based, family-centred service delivery model for children’s therapy in rural Saskatchewan. In alignment with recommendations from the RockBank report (2007) and Saskatchewan’s Patient First Review (2009), CTP redesigned services to children and families to increase access, reduce wait times, and improve the family experience. With an innovative approach to service delivery, CTP has established stronger working relationships with parents, community agencies and First Nations partners.

Siloed approach to Children’s Therapy

• Families were accessing a variety of supports with limited coordination among service providers.

• Each discipline working with the family had its own operational procedures.

• Families were affected by inefficient and inconsistent processes.

Wait Time for Service

• The window of opportunity to help children and families is very small; too much time was spent on a waiting list.
