Overview
• To support an integrated approach to patient care across Dudley,
• To prevent A & E hospital admissions
• To reduce frequent users of GP practices
Dudley - Social Prescribing
Purpose
• Those affected by social isolation and loneliness in later life
• Those patients with mild/moderate depression
• Patients aged 60 years or over, who are identified as meeting the referral criteria, can be referred by their GP practice or the CRRT to the Gateway Service.

Outcomes
The service will support the CCG to:
• Reduce the no. of appointments of frequent users at GP• Reduce inappropriate A & E hospital admissions The service will aim to benefit patients by:
• Reducing feelings of social isolation and loneliness• Improving emotional and physical well being• t to maintain independence Additional benefits:
•More appropriate use of clinician’s time. •Improved links between practices and VCSE Sector
