1. EXECUTIVE SUMMARY PSI conducted a National Environmental Policy Act (NEPA) review, according to the 8-point NEPA Land Use Screening Checklist for the Lake Nona Head End Facility – Cell Tower located at 12385 Lake Nona Gateway Boulevard, Orlando, Orange County, Florida. This NEPA screening report has been performed in accordance with our agreement dated October 21, 2013. Information sources used in completing the NEPA review are shown in Appendix D. The subject property consists of one land parcel surrounded by a chain-link fence. The parcel totals 0.41± acres developed with a 2,230 square foot utility building and an emergency generator powered by diesel fuel contained within an approximately 4,200-gallon capacity double walled aboveground storage tank (AST). A 124-foot monopole cell tower is located in the southwest section of the compound. Gravel and grass-covered land are also located within the compound. The surrounding area consists of undeveloped wooded land.
1.1 NEPA CHECKLIST FINDINGS PSI conducted a NEPA review, according to the 8-point NEPA Land Use Screening Checklist. The review of the NEPA Checklist for the 8 items, found that based on information provided, the scope of work and sources documented in the following report, the NEPA Review determined that the preparation and filing of an Environmental Assessment (EA) will not be required. This summary does not contain all the information that is found in the full report. The report should be read in its entirety to obtain a more complete understanding of the information provided and to aid in any decisions made or actions taken based on this information.

2. INTRODUCTION
2.1 PURPOSE AND SCOPE This assessment was conducted pursuant to the authorized scope of services for the completion of the Federal Communications Commission (FCC)/NEPA Checklist. The protocol utilized for completion of the FCC/NEPA Checklist was in general accordance with the requirements of the FCC Rules on Environmental Impact (47 Code of Federal Regulations [CFR] 1.1307). This report includes documentation to support the analysis, opinions and conclusions as presented.
2.2 SPECIAL TERMS AND CONDITIONS
AUTHORIZATION Authorization to perform this assessment was given on October 21, 2013 by a signed Exhibit A, Consultant’s Scope of Services executed by Mr. Robert Wilson, Director of Construction of Lake Nona Land Development Co., and PSI. This assessment was performed in accordance with the scope and terms of the contract.
PROPERTY ACCESS Instructions as to the location of the property, access, and an explanation of the property and facilities to be assessed were provided by Mr. Robert Wilson of Lake Nona Land Development Co.
USE BY THIRD PARTIES This report was prepared pursuant to the contract PSI has with Lake Nona Land Development Co. That contractual relationship included an exchange of information about the property that was unique and between PSI and its client and serves as the basis upon which this report was prepared. Because of the importance of the communication between PSI and its client, reliance or any use of this report by anyone other than Lake Nona Land Development Co. and Lake Nona Property Holdings, LLC, for whom it was prepared, is prohibited and therefore not foreseeable to PSI. Reliance or use by any such third party without explicit authorization in the report does not make said third party a third party beneficiary to PSI's contract with Lake Nona Land Development Co. Any such unauthorized reliance on or use of this report, including any of its information or conclusions, will be at the third party's risk. For the same reasons, no warranties or representations, expressed or implied in this report, are made to any such third party. 2.3 METHODOLOGY AND RESOURCES
RECONNAISSANCE Ground reconnaissance was conducted on October 27, 2013 by Ms. Vicki B. Lewis of PSI. The ground reconnaissance consisted of observing the periphery of the property and viewing the site from accessible adjacent public thoroughfares. Interior portions of the property were systematically traversed to provide an overlapping field of view, wherever possible. The periphery of structures, where present on the property, was observed along common areas. Please note that the utility building located within the compound was not accessed. Visual reconnaissance of adjoining properties was limited to areas that were readily observable from the subject property or from public access areas. Photographs were taken to document the features observed during the reconnaissance and environmental conditions of concern, where possible. A photographic log and copies of the photographs are included in Appendix A.
2.4 LIMITATIONS AND EXCEPTIONS OF ASSESSMENT Along with all of the limitations set forth in various sections of the FCC Rules on Environmental Impact (47 CFR 1.1307) protocol, the accuracy and completeness of this report is necessarily limited to the information obtained and reviewed based on the date of the report issuance.

3. PROPERTY DESCRIPTION AND PHYSICAL SETTING
3.1 LOCATION The subject property is located at 12385 Lake Nona Gateway Boulevard, northwest of Lake Nona Boulevard. It lies in Section 23, Township 24 South, Range 30 East as referenced on the USGS “PINE CASTLE, FLA” Topo map. It lies at Latitude 28° 23’ 22” North and Longitude 81° 16’ 56” West.
3.2 DESCRIPTION OF PROPERTY AND IMPROVEMENTS The subject property consists of one land parcel surrounded by a chain-link fence. The parcel that totals 0.41± acres developed with a 2,230 square foot utility building and an emergency generator powered by diesel fuel contained within an approximately 4,200-gallon capacity AST. A 124-foot monopole cell tower is located in the southwest section of the compound. The tower is recently erected and currently not in use. Gravel and grass-covered land are also located within the compound. Photographs showing prominent features of the site are provided in Appendix A.
3.3 PHYSICAL SETTING
TOPOGRAPHIC MAP REVIEW A current United States Geological Survey (USGS) 7.5 minute topographic map showing the area where the property is located was obtained and reviewed. The 1977 USGS “PINE CASTLE, FLA” quadrangle map was reviewed. According to the contour lines on the topographic map, the property is located approximately 80 feet above mean sea level. The contour lines in the area of the property indicate the area slopes to the west towards Mud Lake. No water bodies are evident on or adjacent to the subject property. WETLANDS RECORDS REVIEW The U.S. Army Corps of Engineers (USCOE) has been delegated authority for determination of wetlands by Section 404 of the Clean Water Act. This section gives the Secretary of the Army authority to issue permits for discharge of dredged or fill material in navigable waters of the Unites States. The COE presently uses the 1987 Wetlands Delineation Manual (Technical Report Y-87-1, Jan. 1987, Final Report) in making their determination. This wetland determination considers soil type (hydric), vegetation type (hydrophytic) and hydrology. The Soil Conservation Service (SCS) provides county soil maps, which delimit hydric soils and assist in making the wetland determination. The U.S. Fish and Wildlife Service (USFWS) maintains the National Wetland Inventory (NWI) maps which presently cover about 75 percent of the United States (excluding Alaska). FWS has a congressional mandate to complete these maps by September 1977. Where available these data overlay the USGS 7.5 minute topographic quadrangles.

An EDR database search was conducted for this NEPA Screening. EDR defines a positive occurrence as having any of the subject elements within 1/8 mile of the specified site location. The USFWS NWI Map was reviewed for this project. According to this map, no designated wetland areas are indicated on the subject property.

4. NEPA CHECKLIST PSI conducted a NEPA review, according to the 8-point NEPA Land Use Screening Checklist for the Lake Nona Head End Facility – Cell Tower located at 12385 Lake Nona Gateway Boulevard, Orlando, Orange County, Florida. This NEPA screening report has been performed in accordance with our agreement dated October 21, 2013. Information sources used in completing the NEPA review are shown in Appendix D.
WILDERNESS AREA RECORDS REVIEW According to information obtained from the USFWS, the subject property and adjacent land is not a designated State or Federal wilderness land, national park, or state park.
WILDLIFE PRESERVES RECORDS REVIEW According to information obtained from the United States Department of the Interior, USFWS, and the Florida Natural Areas Inventory (FNAI) the subject property and adjacent properties are not designated as a State or Federal wildlife refuge. An official wildlife preserve is located within 1/8 mile.
THREATENED OR ENDANGERED SPECIES RECORDS REVIEW The subject property is currently developed. According to the USFWS, and the FNAI, three are no listings of documented threatened or endangered species identified to be located on the subject or adjacent properties.
HISTORICAL BUILDINGS AND STRUCTURES RECORDS REVIEW Research of the National Register of Historic Places and Historical National Landmarks information provided by EDR revealed the subject property is not listed in the National Register of Historical Places.
NATIVE AMERICAN RELIGIOUS/ARCHAEOLOGICAL SITES Research of the National Register of Historic Places and Historical National Landmarks information provided by EDR revealed the subject property is not a Native American religious or archeological site.
FLOOD PLAINS RECORD REVIEW According to the Federal Emergency Management Agency (FEMA) Digital Flood Insurance Rate Maps (DFIRM), the subject property is not located within a 100-year or 500-year flood zone.
SURFACE FEATURES According to the USFWS NWI map, there are no designated wetland areas or water bodies located on the subject property. The subject property is currently developed with a communications building, emergency generator, and cell tower.

5. SUMMARY AND CONCLUSIONS
NEPA CHECKLIST
A NEPA Review of the Lake Nona Head End Facility-Cell Tower site, located at 12385 Lake Nona Gateway Boulevard, Orlando, Orange County, Florida was performed by Professional Service Industries, Inc. in conformance with the FCC rules and regulations for implementing NEPA; 47 CFR 1.1301 to 1.1319.
Based on the data obtained during the Site visit, the review of readily available information from commercial services, government agencies and/or other sources, the preparation and filing of an EA will not be required.

6. WARRANTY
NEPA CHECKLIST PSI warrants that the findings and conclusions contained herein were accomplished in accordance with the methodologies set forth in the CFR 47 CFR 1.1307 protocol. These methodologies are described by the standard as representing good commercial and customary practice for conducting an Environmental Compliance Checklist of a parcel of property. However, these findings and conclusions contain all of the limitations inherent in these methodologies which are referred to in the protocol and some of which are more specifically set forth below.
UNIDENTIFIABLE CONDITIONS There is a possibility that even with proper application of these methodologies, conditions may exist on the property that could not be identified within the scope of the assessment or that were not reasonably identifiable from the available information. PSI believes that the information obtained from the record review and the interviews concerning the property is reliable. However, PSI cannot and does not warrant or guarantee that the information provided by these other sources is accurate or complete. The methodologies of this assessment are not intended to produce all inclusive or comprehensive results, but rather to provide the client with information regarding apparent suspicions of existing and potential environmental conditions relating to the subject property. No other warranties are implied or expressed.

If the screening investigation for any of the above categories results in a positive screening, an Environmental Assessment must be prepared and filed with the FCC. Construction may not start on any positively screened site prior to receipt of a finding of no significant impact (FONSI) from the FCC.

National Wetland Inventory Maps are produced by the U.S. Fish and Wildlife Service, a sub-department
of the U.S. Department of the Interior. In 1974, the U.S. Fish and Wildlife Service developed a criteria for
wetland classification with four long range objectives:
· to describe ecological units that have certain homogeneous natural attributes,
· to arrange these units in a system that will aid decisions about resource management,
· to furnish units for inventory and mapping, and
· to provide uniformity in concepts and terminology throughout the U.S.
High altitude infrared photographs, soil maps, topographic maps and site visits are the methods
used to gather data for the productions of these maps. In the infrared photos, wetlands appear as
different colors and these wetlands are then classified by type. Using a hierarchical classification,
the maps identify wetland and deepwater habitats according to:
· system
· subsystem
· class
· subclass
· modifiers
(as defined by Cowardin, et al. U.S. Fish and Wildlife Service FWS/OBS 79/31. 1979.)
The classification system consists of five systems:
1. marine
2. estuarine
3. riverine
4. lacustrine
5. palustrine
The marine system consists of deep water tidal habitats and adjacent tidal wetlands. The riverine
system consists of all wetlands contained within a channel. The lacustrine systems includes allnontidal wetlands related to swamps, bogs & marshes. The estuarine system consists ofdeepwater tidal habitats and where ocean water is diluted by fresh water. The palustrine system
includes nontidal wetlands dominated by trees and shrubs and where salinity is below .5% in tidal
areas. All of these systems are divided in subsystems and then further divided into class.
National Wetland Inventory Maps are produced by transferring gathered data on a standard 7.5
minute U.S.G.S. topographic map. Approximately 52 square miles are covered on a National
Wetland Inventory map at a scale of 1:24,000. Electronic data is compiled by digitizing these
National Wetland Inventory Maps.

Orlando ROGERS BUILDING (English Club) 37-39 S. Magnolia Ave. 1886+. Eclectic. 2 stories, exterior fabric is pressed zinc cladding, main facade has heavily embossed friezes. One of the most distinctive late 19th-century buildings in Central Florida and one of the best-preserved examples of sheet-metal construction in the state. Second floor served for a time as a club for English immigrants. Private. N.R. 1985.

Orlando DR. P. PHILLIPS HOUSE. 135 Lucerne Circle, NE. 1893. Shingle style. L.M. Boykin, architect. 2 and a half stories, frame, 3-story tower and monumental Ionic portico. Owned by Dr. P. Phillips, one of Florida's most successful early citrus growers. It is the only Shingle-style residence in the city. Phillips added the Ionic portico. Public. N.R. 1979.
Orlando FIRST CHURCH OF CHRIST SCIENTIST (St. George Orthodox Church) 24 N. Rosalind Ave. 1926-1927. Classical Revival. George Foote Durham, architect. 2 and a half stories, masonry, the main facade has a Doric portico, copper dome. The Christian Scientist congregation abandoned the church in 1975 and it was acquired by the Greek Orthodox congregation. Private. N.R. 1980.
Orlando J.J. BRIDGES HOUSE 704 S. Kuhl Ave. 1916. Colonial Revival. Wilson C. Ely, architect. 2 stories, frame, gabled roof, main entrance has portico, on west facade is portico with balcony. The first of the highly academic, Colonial-Revival-style homes built in the area. Built for the Rev. J.J. Bridges, a retired New York clergyman. Private. N.R. 1984.
Orlando JOHN N. HUTTIG ESTATE 435 Peachtree Rd. 1934. Tudor Revival. James Gamble Rogers, architect. 1 and a half stories. Large house on 2-acre lot. Formal entrance gate. The architect had a national reputation. Private. N.R. 1993.
Orlando OLD ORLANDO RAILROAD DEPOT Depot Pl. and W. Church St. 1889. Eclectic Victorian. 2 and a half stories, brick, hipped roof section, 3-story corner tower with 3-story open porch. Built for Henry B. Plant, 19th-century railroad and hotel magnate, during the large-scale development of the area. Private. N.R. 1976.
Orlando TINKER BUILDING 16-18 W. Pine St. 1925+. Masonry Vernacular. 2 stories, brick, detail on front facade of brick, terra-cotta, and tile. Associated with Joe (Joseph B.) Tinker, one of baseball's legendary personalities, member of the Baseball Hall of Fame and a major Orlando developer. Private. N.R. 1980.
Windermere WINDERMERE TOWN HALL 520 Main St. 1922. Colonial Revival. 1 and a half stories. First used as a woman's club. Moved to its present site in 1938 for use as a community center. Public. N.R. 1994.
Winter Park EDWARD HILL BREWER HOUSE (The Palms) 240 Trismen Tr. 1899+. Colonial Revival. 2 and a half stories, enlarged in 1923 to include porticos with fluted Ionic columns, an altered veranda, balustraded 2nd-story decks.
Edward Hill Brewer was a prominent Cortland, New York, manufacturer who used the house as a winter residence. Private. N.R. 1982.
