It is, therefore, requested to kindly convey your willingness against subject consultancy work as an Individual Consultant by submitting the following documents: 1. Cover letter addressed to the Deputy Managing Director (Engg.), WASA, Lahore containing detailed C.V (duly signed) indicating qualification, relevant experience, list of assiqnments/projects undertaken during last Five (05) years along with time duration of each assignment, description of the nature and scope of work. 2. Attested copy of CNIC, Educational certificates and Experience certificates. 3. An undertaking on a stamp paper worth Rs.100/- that the individual consultant has never been blacklisted/declared ineligible by any government/ semi-government organization. 4. Financial Bid in a separate sealed envelope. The aforementioned documents should reach the office of undersigned not later than 10 th December, 2019 in hard form, not later than 03:00 PM.
