This letter is to inform the Nuclear Regulatory Commission (NRC) that Mr. Scott Cargill the Corporate Radiation Safety Officer is no longer an employee with Appl us RTD USA, Inc (A+) as of July 18th, 2016. Appl us is requesting the following amendment to the license.

Amendment 1 - A+ is requesting the NRC to remove Scott Cargill as the RSO as listed in License condition # 13.

Amendment 2 - A+ is requesting the NRC to name Douglas Miskell as the RSO to the license under condition #13. Douglas Miskell was previously an RSO on NRC license #31-30187-01 out of region 1. If you require supporting documentation it will be submitted as required.

Amendment 3 - A+ is requesting an address change as listed in license condition# 2 and 18, the new address isApplus RTD USA, Inc 34920 Petrol Rd, Bakersfield, California 93308. Attention Douglas Miskell and Stacey Cress.

Jackie,

After more than 23 years, the company and I have come to a crossroads that doesn't include me staying with the company.

So, as of this week, I have left Appl us RTD USA, Inc.

I have informed John Nessel bush the VP of support at applus (my superior) to cease and desist all operations under this license until they can arrange for someone to take over the responsibilities. To the best of my knowledge the license and all activities being conducted under that license are being performed properly and all operations conducted safely. IE. I have no concerns leaving them at this time.
