Referral and Assessment Pathway LOW BACK PAIN CENTRAL INTAKE & RAPID ACCESS CLINIC The Low Back Pain Central Intake and Rapid Access Clinic (RAC) is an inter- professional shared care model for low back pain that is part of a province-wide initiative to streamline referrals and help improve access to quality, appropriate low back pain care in LHINs across Ontario. William Osler Health System (Osler) is launching the Low Back Pain Central Intake and RAC in partnership with the Central West Local Health Integration Network (Central West LHIN) and Trillium Health Partners (THP). Multiple RACS throughout the Central West LHIN will start seeing patients as of September 30, 2019. Patient referrals are now being accepted. Patient Eligibility Criteria Patients must be 18 years of age or older. Inclusion Criteria § Patients with persistent low back pain and/or related symptoms (e.g., sciatica, neurogenic claudication) that are not improving six week to 12 months from onset, OR § Patients with unmanageable recurrent/episodic low back pain and/or related symptoms of less than 12 months duration post-recurrence. Exclusion Criteria § < 18 years of age § Patients with specified red flags § Initial low back-related symptoms < 6 weeks post onset § Constant/persistent low back-related symptoms > 12 months post onset § Unmanaged established chronic multi-site pain disorder § Unmanaged established narcotic dependency § Active low back pain related WSIB claim § Active low back pain related motor vehicle accident claim § Active low back pain related legal claim § Pregnant or post-partum (< 1 year) Steps for Primary Care Providers to Refer Patients Primary care physicians and nurse practitioners must register for privileges to make a referral to the Low Back Pain RAC at http://www.isaec.org/isaec-registration.html. Once registered, you will receive an email within 2 business days that provides you with access to an online module that will take 10-15 minutes to complete. The module will provide further information about the Low Back Pain model of care, and the referral process. Once completed, you will receive the referral form within 2-3 business days via email and may begin referring patients right away. Referral privileges will only be granted to those who have registered and completed the module.

Primarycarephysiciansandnursepractitionersmustregister forprivilegestomakeareferraltotheLowBackPainRACathttp://www.isaec.org/isaec-registration.html.

Onceregistered,youwillreceiveanemailwithin2businessdaysthatprovidesyouwithaccesstoanonlinemodulethatwilltake10-15minutestocomplete.ThemodulewillprovidefurtherinformationabouttheLowBackPainmodelofcare,andthereferralprocess.Oncecompleted,youwillreceivethereferralformwithin2-3businessdaysviaemailandmaybeginreferringpatientsrightaway.Referralprivilegeswillonlybegrantedtothosewhohaveregisteredandcompletedthemodule.
