Historical Accident Causes

• Turkish Airlines Flight 981 in 1974 – A cargo door blew off in flight causing cabin pressure to drop and eventually leading to a section of the cabin floor to collapse. This accident lead to an industry wide design change that limited the possibility of depressurisation

Lessons Learned

• In 1956, a TWA plane crashed into a United Airlines flight above the Grand Canyon . The incident was the first of many that illustrated the need for increased communication between planes, including a 1996 collision near New Delhi that resulted in 349 casualties -- emphasized the need for advanced anti-collision technology.

• In 1991, the FAA implemented the Traffic Alert and Collision Avoidance System, which monitors the airspace around an aircraft independent of air traffic control.

Lessons Learned Cont.

A 1985 incident when a British Airtours 737 caught fire before take-off at Manchester International Airport. Unfortunately, the lack of space between seats meant that bottle-necking occurred at the doors, and some passengers couldn't escape. Now, there are limits in terms of the minimum space between seats as well as the distance to an emergency exit.

• In 1981, the FAA required all turbine-powered commercial planes be installed with wind shear detection systems by 1993.

• Aloha Airlines Flight 243- 1988 while flying at 24000ft, a large section of roof blew off. The FAA created the National Aging Aircraft Research program, which monitors the structural integrity of older planes

• In 1985 Japan Airlines Flight 123, which crashed in the mountains 62 miles from Tokyo. The crash was the result of a poorly executed repair made several years earlier. Boeing responded by monitoring its repairs more closely.

Preventative Actions

• It took two years for search parties to recover the black box from Air France Flight 447, which crashed into the Atlantic Ocean en route to Paris from Rio. Airbus and Boeing have both changed their operation procedures and have put more focus on manual training.

• Gatekeepers are tasked with contacting the flight crew in the event of an incident and providing pilots with on-the-spot training, tools and advice.

Preventative Actions

• Malaysia Airlines Flight 370 (MH370/MAS370) was a scheduled international passenger flight that disappeared on 8 March 2014. The aircraft, a Boeing 777-200ER, was carrying 12 Malaysian crew members and 227 passengers from 15 nations.

• Analysis of satellite communications between the aircraft and Inmarsat's satellite communications network concluded that the flight continued and flew south into the southern Indian Ocean, although the precise location cannot be determined.

Current Accident Causes

24 March 2015 Germanwings Flight 4U9525 crashed 100 kilometres (62 mi) northwest of Nice, in the French Alps, after a constant descent that began one minute after the last routine contact with air traffic control and shortly after the aircraft had reached its assigned cruise altitude. All 144 passengers and six crew members were killed.

Current Accident Causes

• The 2-persons-in-the-cockpit recommendation is maintained. Its benefits should be evaluated after one year.

• All airline pilots should undergo psychological evaluation as part of training or before entering service.

Current Accident Causes

• Mandate drugs and alcohol testing as part of a random programme of testing by the operator and at least in the following cases: initial Class 1 medical assessment or when employed by an airline, post-incident/accident, with due cause, and as part of follow-up after a positive test result.

• The establishment of robust oversight programme over the performance of aero-medical examiners including the practical application of their knowledge.

Conclusion

• Historically from the first accident that was recorded in 1918 the machine was the cause of accidents

• Currently the machine is all but perfect and the cause of almost all accidents is due to the man machine interface.

• This has led to various requirements to combat this new problem: – SMS – CRM – Human Factors Awareness – Operators should introduce appropriate supplemental measures

including training for crew to ensure any associated risks are mitigated.
