    It takes courage and strength to make the choice to receive the

    care you need. For many it is difficult to allow someone else to

    provide the care for those needs and to realize the possibility that

    it may be difficult for your loved ones as well. We pride ourselves

    in working diligently to be a team member with all our residentsand their family members.

    We believe that working together as a team means being able to

    communicate openly and honestly. We hope this guide will give

    you and your loved one realistic goals to help you prepare for the

    changes that occur when moving into a assisted living facility.

    In order to be of assistance in this transition process, we havehighlighted generalinformation from each department.

    ADMISSIONS

    Should you choose admission at our facility, we will schedule an

    appointment for an assessment to confirm that we can meet your

    needs. The resident and/or the responsible party will meet with ourAdmissions Coordinator to fill out the admission packet which may

    take up to one hour.

    Care Conferences are scheduled as needed. Care Conferences

    maintain a clear understanding of the resident's plan of care and how

    that may change, especially when a resident experiences a change in

    their condition.

    Activity Department

    have an ongoing program of

    activities to meet the needs of eachresident. Residents are encouraged, but

    not forced, to participate in scheduled

    activities. We want to ensure the best

    quality of life for our residents and

    provide a "home-like atmosphere". We

    have indoor animals and provide pet

    therapy visits by trained professionals. Activities are provided seven

    days a week and include some evening activities as well as outings to

    local attractions.

    A Resident Council meeting is conducted once a month to help ensure

    that the activity schedule reflects the choices and rights of the

    residents.

    A monthly calendar of scheduled activities is posted in designated

    areas for the residents and families to see.

    In order to promote the physical, mental and

    psychosocial well-being of our residents, an

    activities assessment is conducted and

    maintained for each resident. Residents who are

    unable to attend scheduled activities in the

    Activity Room are provided with in-room visits

    for social, mental and physical stimulation. Our

    Activity Program is staffed with personnel who have the appropriate

    training and experience to meet the needs and interests of each

    resident.

    Volunteers at our community are TB tested, supervised and oriented

    before being matched with our residents' needs. If a volunteer or

    family wishes to have a volunteer provide one-on-one time in a

    resident's room, that volunteer must go through a criminal background

    check as required for all employees hired at Gold Country Assisted

    Living.

    Health Center Dietary Department

    All of us in the Dietary Department would like to welcome you to our

    Assisted Living facility. We strive to make a home-like atmosphere

    for you. The Dietary Department has to follow state regulations to

    ensure your health and safety. Questions most asked will be answered

    below.

    We provide three meals a day. The schedule is:

    Breakfast 8:00 AM Lunch 12:00 PM Dinner 4:30 PM

    Meals are served "family style" in Assisted Living. Snacks are offered

    at bedtime, or if you want, in-between meals. We will deliver meals to

    your room on occasion, but we encourage all residents to go to the

    dining room for social interaction.

    Our menus are prepared by a registered dietitian for a four-week cycle

    and changes with the four seasons. All diets are prescribed by the

    physicians. The daily menus and alternate choices are posted near the

    ALU dining room on a bulletin board.

    We use Diet Cards with our meals in ourMemory Care Unit. These diet cards

    will be on the meal tray. Please send the

    diet card back to the kitchen on the tray,

    so we can clean them and use them to

    set up the tray for the next meal.

    Anything written in red on the diet card,

    means that either you do not like, or

    cannot have those food items.

    No outside food can be brought into the kitchen per state regulations.

    All food brought in by families must be labeled, dated and stored in

    the dining room's refrigerator. Food will be discarded after 72 hours.

    Assisted Living Department

    Senior Assisted Living Communities are

    considered a non-medical facility. The Director is

    a Licensed Vocational Nurse and is on duty eight

    hours during the day shift and is on call 24 hours aday. Assisted Living also employs Certified

    Medication Technicians to assist with medications

    and treatment orders. Certified Nursing Assistants

    (CNAs) and Caregivers provide direct resident

    care as regulated by State and Federal Regulations under the direction

    of a licensed professional. There is an Assistant Manager and Shift

    Managers on duty to help you.

    Caregivers receive First

    Aid certificationthrough the American

    Red Cross. All

    Caregivers receive

    medication training

    upon hire and attend

    medication in-services.

    Your Doctor

    You have a choice of physicians. The physician must be credentialedand be willing to visit you in our facility on a regular basis, or you

    must be able and willing to see your doctor on your own. Your doctor

    is responsible for providing the facility and care staff with written

    admission orders as well as updated orders and information as to your

    specific medical needs.

    Medications

    We have a facility pharmacy off site who

    provides deliveries daily, but you may chooseyour own pharmacy.

    Residents are not allowed to keep medications

    of any kind at bedside unless they pass a "Self-

    Administration Assessment" and have an order

    from their physician. The Certified

    Medication Technicians assist residents with

    medications as prescribed.

    Emergency

    Since Assisted Living is a non-medical, care

    community residents will be transferred to the

    nearest acute care hospital in cases of

    emergency, if requested, or if indicated.

    Room Arrangements

    The majority of our rooms are single

    occupancy. Some rooms are double occupancy

    in our memory care unit. We encourage all of

    our residents to have a "home-like atmosphere".

    Room Access

    The resident rooms are not locked for safety reasons. On occasion

    some residents will wander in and out of rooms.

    Room Security

    Open access to the resident rooms means it is

    important to understand there are certain risks

    in living in a group environment. Monies and

    valuables can be kept safe in the business

    office or should be left at home. In order to

    provide a safe and secure environment, thereare alarms on exterior doors to alert the staff

    when they are opened after hours.

    Restraints

    It is our policy not to restrain our residents in

    any way.

    Physical Changes

    Natural and age related changes in a

    resident's flexibility, balance, cognition,

    vision, heart and vascular condition can

    contribute to accidents and falls. We willdo everything possible to prevent these accidents, but cannot promise

    it will not occur. However, if a resident becomes unable to ambulate

    or requires the assistance of 2 people to move from one place to

    another, they may be in need of a higher level of care than what

    Assisted Living is able to provide.

    Infection Control

    Vaccines are offered to our community annually provided the resident

    or resident's responsible party have signed an informed consent.

    A tuberculosis (TB) test or a current chest X-ray report is required

    within the last six months for all new admissions.

    On going in-services, training and audits are performed to ensure that

    staff is practicing effective infection control in our facility.

    Hand sanitizers are placed throughout the building. We encourage you

    to wash your hands when you come and go.

    Staff Development/ Human Resources

    Prior to being hired, all staff are drug tested,

    fingerprinted and have a back ground check. All

    licensed or certified staff are checked for current

    license/credentials and active status to ensure that we

    have hired the most qualified staff and for the safety of

    our residents.

    We have mandatory All Staff In-Services every month that are

    required by State and Federal Regulations. We also have ongoing

    in-services for the staff to increase their current skills and knowledge

    base.

    Business OfficeOur goal is to make the financial liability process as easy as possible.

    Questions can be directed to the business office in the front lobby of

    the Health Center Building between the hours of 8:00 AM and 4:30

    PM, Monday through Friday.

    Assisted Living is private pay and not covered by

    medical insurance or state programs. Billing

    Statements are generated at the end of each month

    for the upcoming month, so room board is billed atthe beginning of the month.

    However, some Long Term Care insurances do

    cover Assisted Living. If you have Long Term Care

    insurance that does cover Assisted Living, we can

    assist you with filling out monthly forms.

    While in our Assisted Living Community, it is the resident's

    responsibility (or their responsible party's) to supply their own

    personal hygiene items as well as establishing phone service should

    that be desired by the resident.

    We offer a Trust Account to keep your money

    safe in an interest bearing account with quarterly

    statements.

    We offer postage stamps and mailing out of

    outgoing mail.

    Housekeeping

    The resident rooms are cleaned daily seven days a week and more if

    needed. Room detailing and heavy duty cleaning are scheduled on a

    routine basis. The disinfection of hand rails, door and handles, tables

    and chairs are ongoing to prevent the spread of illnesses.

    Laundry

    ALL resident's personal clothing are to be labeled with the resident's

    last name and first initial. All the resident's laundry is done within the

    Assisted Living Unit.

    Please report any missing or destroyed items to management and we

    will investigate promptly. Any time a resident or family member tells

    us clothing is missing we take that seriously. We immediately track it

    down and return the missing item to the resident or provide the

    resident with reimbursement.

    Health Information also known as Medical Records

    Your health record begins and ends in the

    Health Information Services Department. Every

    resident is assigned a record number upon theirfirst admission. Discharged health records are

    stored for 7 years after the resident's last

    discharge date before the records are destroyed.

    The health information that is within the health

    record belongs to you, however the facility owns the health record that

    was generated during the normal course of business.

    On admission you will be receiving a

    California Notice of Privacy

    Practices describing how yourprovider uses and discloses health

    information. You will be signing:

    (1) HIPAA Summary Sheet form

    and

    (2) A Use and Disclosure of Health

    Information form.

    There is also a Release of Information form you will sign only when

    someone requests the release of your protected health information.There is an administrative cost for the release of information if it is

    not for a continuum of care with the physician.

    All physician orders written by your

    doctor are entered into the computer.

    Medication and Treatment flow sheets

    are generated by the Medical Records

    Department. These flow sheets are used

    for documentation by the staff when

    assisting residents with those prescribedorders.
