Keeping hands clean through improved hand hygiene is one of the most important steps we can take to avoid getting sick and spreading germs to others. Many diseases and conditions are spread by not washing hands with soap and clean, running water, or using an alcohol-based hand sanitizer that contains at least 60% alcohol.

How They workHand sanitizers work on bacteria by stripping the outer layer of oil from your skin, which destroys “transient” bacteria (picked up from various surfaces we’ve touched). Once you’ve used the sanitizer, bacteria will regrow, but more slowly. This keeps the germs you harbor inside your skin from coming to the surface.

Alcohol ConcentrationThe New York Times quotes Allison Aiello, an epidemiologist at the University of Michigan, who says that “it has been

known for decades -- that an alcohol concentration under 60 percent won’t kill the microbes.”

According to a 2003 article in “Infection Control Today,” most alcohol hand sanitizers contain approximately 60 to 95 percent ethanol or isopropanol alcohol. Gels, rinses and foams are different types of alcohol hand sanitizer available. A variety of advantages exist for alcohol hand sanitizers over regular hand washing; yet, you should always wash grossly soiled hands with water and antimicrobial soap, according to the Centers for Disease Control’s hand hygiene guidelines.

Products currently on the market that are considered effective contain ethyl alcohol (62% to 95% concentration), benzalkonium chloride, salicylic acid, pyroglutamic acid, and triclosan. These products are available in fast-drying gels,

foams, wipes, and mixed with moisturizing lotions. According to the CDC, hand sanitizers containing at least 60% alcohol can quickly decrease the number of germs on hands in many situations, but not all sanitizers are equally effective in eradicating all germs. The CDC also reports that hand sanitizers are not effective when the hands are visibly dirty.

Best PracticesPatients should apply a sufficient amount of product to the palm of 1 hand andensure that the sanitizer makes contact with the complete surface of your hands, especially in between your fingers. Pay close attention to you fingertips where bacteria hides close to your fingernails. Rub your hands together until the sanitizer dries.

Traditional antiseptic rinses are engineered to kill the bacteria living in the oral cavity, including the normal flora necessary for a healthy mouth. PerioShield is not an antiseptic; so it works differently. The proprietary active ingredient in PerioShield, delmopinol 0.2%, does not directly kill bacteria; rather, it coats the teeth and gingiva and creates a positive charge that repels plaque thereby reducing bacterial adhesion. Chlorhexexidine Gluconate (CHX) has long been a trusted tool to help prevent infections before and after oral surgery or in patients with periodontal disease. Most CHX products, however, contain alcohol, which may cause pain, irritation and dry-mouth. Furthermore,

certain patients may be opposed to using products that contain alcohol for personal or medical reasons. To overcome the negative side-effects that alcohol can cause, GUM® offers the only FDA-approved alcohol-free CHX product on the market.

Chlorhexidine Gluconate Oral Rinse USP 0.12% is indicated for use between dental visits as part of a professional program for the treatment of gingivitis. This oral rinse should not be used by persons who are known to be hypersensitive to Chlorhexidine Gluconate. The effect of Chlorhexidine Gluconate Oral Rinse USP, 0.12% on periodontitis has not yet been determined. Chlorhexidine Gluconate Oral Rinse can cause staining of oral surfaces, increase calculus formation and cause taste interference. Please see all prescribing information available with the product.

On December 8, 2011, the Supreme Judicial Court of Massachusetts affirmed the final decision and order of the Board of Registration

in Dentistry suspending the license of a dentist who failed to conduct and record weekly spore testing. The dentist’s failure constituted dental malpractice.*

Biological Monitoring and Indicators With the results of that recent case, spore testing (biological monitoring) is more important than ever before. Spore testing uses highly resistant bacterial spores (Biological Indicators) to test sterilizers. Since spores are more resistant than other microbes, killing them should also kill the less resistant microbes on equipment. Biological Indicators are considered the benchmark for biological testing.

From cuts to bloodborne pathogens and

infection control, the reasons to wear

disposable exam gloves are clear. Yet,

choosing the right glove for the job isn’t always easy

with so many variations of latex, nitrile and vinyl

gloves to choose from. It’s important to consider the

positives and negatives of every glove type.

A good quality latex or nitrile glove provides the

highest standard in barrier protection. The soft and

stretchy construction of natural rubber latex lets

the glove move and bend with the lowest risk of

rips and pinhole tears. Many dental professionals

enjoy the comfortable fit and feel of latex while

recognizing it provides the greatest barrier

protection of all gloves. Newer generation nitrile

gloves are thinner, more flexible and still provide

strength against punctures and tears like latex.

To some, eliminating the worry of allergic reactions

to latex proteins in patients and dentists makes

nitrile the more desirable of the two choices.

Unlike latex and nitrile, vinyl gloves do not stretch

and conform easily, causing a dramatic increase

in pinhole tears and rips. Studies have shown

an alarming difference in leakage rates with use

between latex or nitrile and vinyl gloves, from 2%

for latex and nitrile to 50% for vinyl (American

Journal of Infection Control). These studies show

vinyl’s effectiveness as a reliable barrier between

patient and practitioner is greatly reduced through

use and extended wear.

Synthetic gloves are used to provide protection both to the user and to the end consumer. There are currently two main types of latex-free synthetic gloves: nitrile and vinyl. However, not all gloves are created equally.

D isinfectants registered with the Environmental Protection Agency for hospital use are the only disinfectants that

should be used in the dental environment.

EPA RequirementsThe Environmental Protection Agency requires the destruction of Mycobacterium tuberculosis (MTB) as the benchmark for intermediate-level disinfectants. MTB is a highly resistant, vegetative, acid-fast bacillus that is used as the resistance standard for disinfectants because of its innate resistance to chemical germicides.

Mycobacterium tuberculosis is used as a “marker” bacterium not because of any risk of contracting tuberculosis from a contaminated counter top or light handle, but because killing MTB also destroys other patho gens of occupational significance. These include hepatitis B virus (HBV), hepatitis C virus (HCV), human immunodeficiency virus (HIV), herpes simplex viruses, influenza viruses, and Staphylococcus aureus.

In addition it MUST say that it kills TB or on the label to be EPA registered for clinical use, and it has to be EPA registered to be compliant with OSHA.

The labels of most products registered by EPA for use against HBV, HIV, or MTB specify a contact

time of 10 minutes. Such a long contact time is not practical for disinfection of environmental surfaces in a health-care setting because most health-care facilities apply a disinfectant and allow it to dry less than a minute. Currently there are new products that have been developed which have a 1 minute kill time.

Wipe Vs. Spray - Microbial Burden In decreasing microbial bioburden on surfaces a study at the School of Pharmacy at Cardiff University actually found wipes to be superior than

spraying a cloth or paper towel and applying disinfectant as an aerosol when used properly. In addition excessive aerosolization of sprays can cause respiratory, ocular, allergic, and/or dermal irritation problems. For proper use it is vital that you use one wipe to clean and one wipe to disinfect. Most labels will instruct you to use one wipe to clean. Throw that wipe away, and then use one wipe to disinfect.

Have you ever left dirty dishes to sit at home without rinsing or soaking them? If you have, you know that it is much

harder to clean them later. The same is true of dental instruments. If instruments cannot be cleaned immediately after patient treatment, they should be placed in a holding solution. As with your dishes at home, by keeping instruments moist, debris can’t dry and harden. This makes the cleaning process much more effective. Later, when you have time, you can begin the actual processing of the instruments.

Holding SolutionA holding solution is simply a noncorrosive liquid, preferably one that contains a surfactant, stored in a hard walled, spill-proof container with a lid. Because of the potential exposure to infectious material, the container must be labeled as a biohazard. It is not cost-effective nor necessary to use a disinfectant as the holding solution, because contaminated particles on the instruments prevent effective disinfection. In addition, the chemical becomes contaminated with bioburden, rendering the disinfectant ineffective over time. Remember that a holding solution is used only as a way to keep debris moist to facilitate cleaning later.You should clean your holding solution twice a day.

Ultrasonic CleaningUsing a detergent solution and sonic-action, ultrasonic cleaners break up and loosen debris on instruments. To maintain efficiency, ultrasonic cleaners must undergo scheduled maintenance

and function tests, according to the manufacturer’s directions. Ultimately, however, it is your responsibility to maintain the efficiency of the ultrasonic cleaners.

Tips and Hints• Always follow the manufacturer’s directions for use, solutions and maintenance.

• Test cleaning solution for effectiveness.

• Don’t overload the tank.

• Only use solutions formulated for use in the ultrasonic bath.

• Rinse instruments or cassettes both before and after the cycle.

• Once the cycle is complete, allow the instruments to air dry or pat them dry with several thicknesses of toweling.
