May 1987 thru March 1988Corporate Safety Director ofORANGE BLOSSOM GARDENS and FLEETLINE HOMES of FLORIDA, INC.Lady Lake, and Tavares, Florida

I served as Safety Director for both Corporations, which actually were co-related Corp., Orange Blossom Gardens (now “The Villages”) is a rapidly expanding “Adult Modular Home Community”, with 3,000+ residents, and plans for 30,000 homes to be developed, along with shopping centers (now there) and other support facilities, etc. The Modular Homes are manufactured by two factories, of the Corporate name of Fleetline Homes of Fla., Inc. There were some 700+ employees, with anticipation’s for some 1,000+ in the near future.

All forms of construction from Commercial Structures to building roadways, Engineering and Architectural planning, Concrete Plant, Woodworking and Metal Working and Welding Shops, Sewer and water Utilities and Waste Disposal, a full variety of stores, Shopping Center, Gas Station, operation of several Restaurants, operation of several Golf Courses, a Fleet of 150 Vehicles of all types, etc., along with two large Factories and all of the full scale Industrial Manufacturing machinery and heavy equipment involved in constructing homes, from the ground up, as well as over-the-road delivery and set-up operations involved, as well as

community relations involvement which goes along with such a complex operation, gives some indication of the vast array of activities involved in this Corporate position of Safety Director.

My responsibilities as Safety Director involved all phases of Loss Control from accident and injury prevention programming, to fire prevention and overall property protection, for the residents as well as the employees. Also, my duties involved reviewing & evaluating blueprints and plans for various construction and remodeling of buildings and facilities throughout this large development, to help bring about conformity with Fire and Safety related codes & standards. All of my past thirty+ years of Safety and Security, and Insurance Involvement, training and experience, was actively utilized within my position as Safety Director for Orange Blossom Gardens and Fleetline Homes of Florida, Inc.

Summary - in this position as Safety Engineer & Instructor, dealt with most all forms of Businesses, involving Loss Control functions of most all categories, including that of working directly with manufacturers of various products, and equipment, in assisting them with professional advice and guidance in relation to designing, installation, operation, and maintenance, in a manner which helps to maintain Safety as an integral built in part of the end product, which thus minimizes the liability which may be incurred by the producers, and dealers, as well as the Insurers of the operation.

Performed numerous varieties of Safety related training functions, various certified courses, programs on Agricultural Safety, programs for youth groups, schools staff & faculty as well as business organizations, etc. and served as guest lecturer and instructor at Colleges and Universities, National Safety Council seminars, and various Safety specialty courses.

Farm Bureau numerous duties involved Safety Inspection of Facilities and submittal of reports of findings and corrective suggestions, to assist the underwriting department in making appropriate and well informed decisions relative to suitable coverage’s to be issued for insurance policies.

Served as the Florida representative on the Business and Industry Committee of the U.S. Civil Defense Council in the Development of Loss Reduction Contingency Planning, and Coordination of Fire and Rescue Services for private business and industry interest. Most of the details outlined in the following employment outlines, have been involved in this position with the Florida Farm Bureau Safety and Loss Control Department.

Summary - served as Accident Prevention & Safety Training Officer, as well as Civil Defense

Coordinator, which involves approx. 16,000 employees, 27,000 students, and over 100 different outlying facilities extending through-out the entire State of Fla. Operations include a vast Medical Center & Hospital, Agricultural activities of all kinds, Forestry, Marine Life, Boating, Aircraft, Transportation fleets, shops - maintenance, Construction, Electrical utilities, Nuclear sciences, Aero space activities and machinery associated with each type of operation, as well as research labs of all kinds, operations in practically any imaginable realm, including coordination of Safety needs with the numerous Private Contractors who are continually operating through-out the various facilities.

Reviewed and evaluated all the Blueprints and Plans for all new Construction & all Remodeling of Buildings and Facilities throughout the Statewide University Complex to Enforce Compliance with Fire & other Safety related Codes, Standards, Statutes, & Advisory Guidelines, etc., with follow-up Inspections of the various completed projects, to determine adequate Compliance with Safety related Requirements.

Presented Safety Training in connection with other Courses being taught on Campus, in the numerous departments, in connection with these various statewide operations. Was responsible for coordinating & conducting all types of Safety Training Activities, Safety Surveys of facilities, the development of Safety guidelines and rules, compiling of Safety statistics on employees and students as well as the general public around the University, monitoring of Safety equipment programs, maintaining a Safety reference library, Investigation of accidents, etc., as well as organizing & coordinating Fire & Rescue Services and other Civil Defense related Emergency Preparedness programs, along with continually auditing the effectiveness of these interrelated Loss Control associated programs. Summary - Served throughout these 16 years as Business & Industry Safety Advisor, Agricultural Safety Specialist, Safety Instructor, and as Deputy to the Administrator of Bureau of Workmen’s Compensation.

My Industrial Commission of Ohio duties & activities were numerous and diversified, working with all manner of associations, committees, groups, and agencies that represent Safety interest in all manner of businesses, Implementing Safety and Industrial Hygiene programs for Industry, Agriculture, and Public Agencies. Was active in Safety Training programs throughout Ohio, along with developing training aids, movies, slides, conducting courses and presentation of Safety demonstrations and informative programs at National meetings, Safety Congresses, along with programs on Radio and TV, and preparation of articles and items of literature, in the overall promotion of Safety and Industrial Hygiene.

I was one of the originators of the widely utilized “Supervisors Safety Training Course”, and the now universally utilized triangular red & orange “Slow Moving Vehicle Emblem”.

This position regularly included making on-site Safety and Security surveys, inspections, and investigations of working conditions and environmental factors, home and Public safety activities, advising as to suitable corrective measures, with follow up re-checks to determine adequacy of corrective measures employed, as well as advising on legal provisions of Codes, Standards, rules and regulations, engineering of Fire Preventive procedures, mechanical guarding of hazards, etc., and advising on occupational disease preventive methods, personal protective equipment, etc., as well as conducting various programs of information about the Workmen’s Compensation provisions of the State.

Summary - Over these 28 years, served as Aircraft Maintenance Officer & Instructor Supervisor in the U.S. Army Transportation School; as U.S. Air Force Personnel Officer, Education & Training Officer; Personal Affairs & Benefits Officer, Ground Safety & Range & Explosives Safety Officer, & Disaster Preparedness Officer, including Emergency Response Training Coordinator, and Personnel Management Officer.

My Military experience has been in such realms as: trucks, trains, harbor craft, stevedoring, pipelines, aircraft - fixed wing & helicopters, etc. Received training at the Interagency Nuclear Weapons Training Center in New Mexico, & participated in numerous Safety training activities and courses in management, job analysis, maintenance, radiological monitoring & defense, shops & occupational Safety, as duties as Instructor & Education Supervisor.

Among various Reserve duties, have served as U.S. Air Force Reserve Civil Air Patrol Liaison Officer, & as State Safety Officer on the Ohio Wing Staff of the Civil Air Patrol, & Composite Squadron. During this time I was conducting Safety Programs (and Supervision), teaching Safety classes, making Safety surveys, investigating accidents and preparing Training aids & Safety surveys etc.

In more recent years, served as Reserve Disaster Preparedness Officer at Patrick Air Force Base & the associated Cape Canaveral Air Force Station, the Cape Canaveral Launch Facilities, Kennedy Space Center, & the overall Eastern Test Range, which includes Radar Sites throughout the Atlantic Ocean & down to South Africa.

Also, was selected to serve as an individual Auditor, as well as a member of a team of Auditors, to evaluate & made recommendations, relative to the Loss Control effectiveness of Disaster Preparedness Programs on Air Force Reserve Bases throughout the Eastern States of the Country

At Patrick Henry Airport, Newport News, Virginia, & at Lunken Airport, & Xavier University Cincinnati, Ohio; as F.A.A. certified Aviation Ground School Instructor with ratings in: Aircraft Engines, Aircraft Engines, Aircraft (Airframe & Principles of Flight). Navigation, Radio (& Radio Navigation), Meteorology, & Federal Air Regulations.

Conducted Aviation courses at Patrick Henry Airport while in the Army at Ft. Eustis, Va. I conducted regular courses in the Cincinnati area of Ohio, at Lunken Airport & other nearby airports, & at Xavier University in the R.O.T.C. Military Pilots Training program, as well as giving private training to interested individual students.

Safety naturally has always been the primary purpose for all such training courses, for the life & safety of the pilot, the aircraft, & the passengers involved, as well as the conditions which might otherwise exist.

In this connection, have been active in the coordination of Flight Safety Clinics, Management of the Aviation section programs of the annual ALL-OHIO Safety Congresses, etc.

Also active as a Civilian Pilot, with Ratings in both Land and Sea Planes.

1945 - 1974Agricultural Experience Background:

Grew up as a “farm boy” - on our Family Farm of 190 acres in Williamsburg, Ohio (East of Cincinnati). Was a student of Vocational Agriculture and active in the F.F.A. (Future Farmers of America) at Williamsburg High School, and was active in 4-H Clubs with Beef Cattle, Farm Machinery, Tractor Maintenance and Farm Safety as projects.

Was State of Ohio Champion in the 4-H Clubs Tractor Maintenance and Tractor Safety programs, and winner of trip to the National 4-H Club Congress, in Chicago, in 1949. 24.Additional Agricultural Experience Background - - - of Herbert T. Owner and Operator of the family 190 acre general purpose farm, until 1974, when the Federal Government acquired the farm as part of a large Flood Control Reservoir project.

Results from nearly 30 years of direct Farm Involvement, has been a very diversified personal experience with Farm Machinery and Equipment, and General Farm Conditions, and first-hand Safety Involvement.

Commercial Campgrounds and Recreational Park Experience - Background; for 10 years our Farm was partially operated under the business name of “Cabin Run Lakes Parks”, as Commercial Campgrounds, with two Fishing Lakes, Fishing Bait and Tackle Shop, Picnicking, Hiking, and General Outdoor Recreational Area, as well as available Camper Trailer Rental Units, etc.

This business, and affiliations with outdoor rural recreational and campgrounds owners’

association, has provided first-hand experience with the Safety related details which are necessarily an integral part of public exposure business operations.

As a result of these personal involvement’s with Farming & outdoor Rural Recreational businesses, my duties with the Industrial Commission of Ohio and its Division of Safety and Hygiene, also included serving as State Farm Safety Specialist, and Rural Recreation Safety Specialist.
