• Showcase property’s roof must be at least 30 squares. • Maximum size of rebate allowed is 250 squares

per property. • One rebate allowed per showcase property.

• TruSlate™ Systems must be purchased between June 1, 2009 and December 31, 2009, and installed on the property no later than January 31, 2010.

2. Fill out the form below... • Maximum of 1 property per builder/contractor • Up to 2 properties per SlateCrafter™ Specialist

3a. Attach original distributor invoices… showing purchase(s) between June 1, 2009 and December 31, 2009
