n Symptoms may be transient or intermittent, andmay be variable in severity, reflecting thefluctuations in ovarian activity that occur duringspontaneous onset of POI

n Young women with primary amenorrhea rarelyexperience symptoms at presentation, implyingthat these symptoms are due to estrogenwithdrawal rather than estrogen deficiency

n pregnancy occurred in 4.8%n predictive factors included markers of ovarian

activity at diagnosis and the detection ofovarian follicles by ultrasound

How should bone health be monitored?

n Measurement of BMD at initial diagnosis ofPOI should be considered for all women, butespecially if there are additional risk factors

n If BMD is normal and adequate systemicestrogen replacement is commenced, noneed for repeated DEXA scan

n If diagnosis of osteoporosis is made andestrogen replacement or other therapyinitiated, BMD measurement should berepeated within 5 years

Cardiovascular health

n Many cohort studies have shown that womenwith natural POI before the age of 40 yearshave earlier onset of coronary heart diseaseand increased CVD mortality

n A meta-analysis involving 10 observationalstudies comprising 190,588 women followedfor 4-37 years found POI an independentthough modest risk factor of ischemic heartdisease and overall CVD, but not of stroke(Roeters van Lennep, et al., 2014)

n Women with POI should be informed thatHRT has not been found to increase the riskof breast cancer before the age of naturalmenopause
