Include names, IISE membership numbers, e-mail (school and personal, e.g. g-mail, important we have an e-mail address that the members of the team are responsive to and they will respond to after they graduate, hence g-mail or equiv is probably a better address), cell #, for each team member. (all contact information must be provided for all team members!)

Short ‘team/Individual’ statement explaining why this project deserves recognition.

How was SUCCESS (successful completion-deliverables and outcomes) for this project defined at the outset of the project?

Where are you relative to this definition of SUCCESS? (e.g. where are you relative to DONE on the project) What are the key Metrics of Interest and what positive changes were you able to make in those?

What were your individual and collective key learnings from doing your ISE Capstone Design/Improvement Project?
