The model is designed to work on the relationship

between employer, employee and healthcare

providers and ultimately will be the most

comprehensive model available in the UK. It

creates a valuable bridge between public and

private health programmes and services, providing

individuals with greater choice to get well, whilst

importantly providing significant and measureable

benefits and savings to the employer.

Our approach can best be described as an

affordable membership service to companies

and individuals providing case management and

guidance to people who require assistance following

referral/diagnosis for management and treatment

options from their GP or consultant. In certain

circumstances, private sector treatment is available

through the NHS, Choose and Book facility.

Patient Advocate provides nurse led case

management services to speed up the delivery of

healthcare to uninsured employees using either

NHS or private healthcare providers. The service is

primarily delivered through contact via telephone

and email although on occasion staff may be

required to attend patient’s homes and employer’s

offices. The business uses the NHS “Choose and

Book” arrangement which allows NHS patients to

receive care Nationwide in either NHS or private

hospitals. The nurse led team will call doctors,

consultants and hospitals to find a provider who

can promptly meet the patient’s clinical and

personal needs. Appealing to both the insured and

uninsured employees, the business aims to reduce

the employee’s absence from work, improve the

wellbeing of the workforce increasing productivity

and reducing the cost of both absence and care.

Chronic conditions such as cancer, cardiac and

mental health are often best delivered through the

NHS, and it is therefore, of great appeal to offer a

service that case manages and fast-tracks members

through their treatment options, directing

members to the most appropriate providers in

both the public and private arenas.

The Patient Advocate proposition is not an

insurance product. It can work on its own or in

parallel with private medical insurance that is

already in place, offering employers direct and

indirect cost savings over existing models. This is

Patient Advocate provides a completely new and affordable corporate healthcare management model, focused on employees outside the scope of companies’ private medical insurance arrangements. Additionally the service can help by reducing the spiralling costs of existing private medical insurance (PMI) premiums, as well as offering a valuable case management service to employees and individuals who elect to use the NHS.

� NHS treatments are always free at the point of need therefore there is no eligibility to

be treated privately, other than through NHS Choose & Book.

� Patient Advocate case managers are fully qualified registered nurses with experience

within primary, secondary and tertiary patient care.

� Knowledge of the NHS, Choose and Book and private access to healthcare is at

the centre of Patient Advocate services. When an individual needs treatment,

understanding medical terminology and assessing the quality of provision, PMI terms,

cost implications and potential delays, can be a daunting task. A Case Manager will

help an individual navigate the healthcare system providing useful and supportive

information based on which an individual can make informed decisions.

� Certain medical conditions are not covered privately through NHS Choose & Book,

therefore case management intervention can support those individuals optimize their

care pathway within the NHS.

Who are Patient Advocate?

Patient Advocate is a company that firmly believes in a better way forward to achieving high

as well as other groups to deliver a wide range of services for which Case Management forms an

integral part.

What is Case Management?

Simply stated, Case Management is a process employed by clinical professionals to assist

individuals in navigating today’s complicated healthcare maze. The goal of case management

is to help facilitate patient centric, safe, timely and appropriate medical care, to optimize ideal

treatment outcomes.

Who are the Patient Advocate Case Managers?

Patient Advocate Case Management are registered nurses who have extensive experience in

providing case management services to individuals, supported by consultants that specialise in

the required field of medicine.

What does a Case Manager do?

A Case Manager acts as a liaison between the patient, the doctor and any other providers

together with the insurer and employer in order to identify what services might be required. A

Case Manager can help to coordinate all services and resources necessary to promote a return

to the best level of well-being. Case Managers usually work via the phone, however, home and

workplace assessments are employed if appropriate.

Case Managers can help the patient and their family to navigate through the healthcare system

by providing resources that may support a healthier and productive lifestyle.

Case Managers are there to help the patient and their family cope with the medical, social,

emotional and vocational problems related to a patient’s injury or illness.

Case Managers can assist a patient in acquiring the care they need. Whilst they do not make

decisions for the patient, they are able to provide the support, assistance and knowledge

that allow the patient to make an informed decision regarding any treatment that might be

required. They can also help in obtaining answers to questions a patient might have about

treatment and rehabilitation.

How does Case Management work?

Where appropriate, Patient Advocate will

engage a Case Manager to work with a

patient in order to facilitate a return to the

best state of health and wellbeing, and work

as is appropriate. They will make contact to

arrange a first contact assessment, and at

this initial stage will explain their role and

the Case Management process. Only after

the patient is satisfied that they have a full

understanding of how Patient Advocate can

assist them achieve the best outcome and

treatment options available to them, through

the use of individual case management do

Patient Advocate obtain a full signed consent

from the patient.

The Nurse Case Managers make a detailed

initial needs assessment, taking into

consideration the health, social care,

educational and employment needs using

communication and available resources to

promote quality cost effective outcomes.

When immediate needs have been

collaboratively discussed, a care plan or

pathway is designed to document the

assessment, planning, implementation and

co-ordination of the case management

interventions. Further monitoring and

evaluation is vital to ensure that achievable

outcomes are set and met along the

way. The care pathway is reevaluated at

regular intervals to ensure that the patient

is compliant and consensual with a full

understanding of their care, whilst any change

to care is considered should that benefit the

patient and outcome long term.
